Guidelines for diagnostic testing in adults with presumed atopic dermatitis refractory to treatment.

BACKGROUND: While many adults diagnosed with atopic dermatitis (AD) achieve disease control with standard treatments, a subset of patients remains refractory to optimal management. In these cases, misdiagnosis or the presence of concomitant conditions may be contributing to treatment failure. OBJECTIVE: To provide evidence-informed guidance for the diagnostic workup of presumed adult AD unresponsive to optimized treatment. METHODS: An expert multidisciplinary workgroup applied Grading of Recommendations, Assessment, Development, and Evaluation methodology for issuing guidance on approaching

Maternal Anxiety and Psychosocial Burden in Childhood Atopic Dermatitis: A Comparative Study.

BACKGROUND: Atopic dermatitis (AD) is among the most prevalent chronic pediatric skin diseases. Beyond its cutaneous manifestations, AD imposes substantial psychosocial and economic strain on families. We quantified this burden-i.e., maternal anxiety and quality of life (QoL)-and identified its clinical and sociodemographic determinants, comparing families of children with AD to those of healthy peers. METHODS: This cross-sectional study enrolled 84 mothers of physician-diagnosed patients with AD aged 3-144 months and 90 mothers of age-matched healthy children attending routine visits. Diseas

Atopic Dermatitis: A Disorder of both Adults and Children with Varying Longitudinal Course.

The longitudinal course of atopic dermatitis (AD) is heterogeneous and complex. While previously thought to be a childhood disorder, recent studies demonstrated that childhood-onset AD may take several different courses that may involve persistence into adulthood becoming a lifelong condition. Other patients only demonstrated adult-onset AD. Different factors may play a role in the timing of AD onset. Assessing the longitudinal course also involves understanding the changing temporal pattern of AD. Understanding the dynamic course of AD is important in identifying individualized treatment reco

Efficacy of abrocitinib in patients with atopic dermatitis resistant to Janus kinase inhibitors.

Abrocitinib, a selective Janus kinase 1 (JAK1) inhibitor, has demonstrated efficacy in moderate-to-severe atopic dermatitis (AD), although data in patients whose AD is resistant to multiple systemic therapies remain limited. We report the case of six patients with moderate-to-severe AD who were unresponsive to previous treatments, including dupilumab, tralokinumab, lebrikizumab, upadacitinib and/or baricitinib. All patients received abrocitinib 200 mg daily. After only 16 weeks, five patients achieved a good clinical response, defined as an Eczema Area Severity Index score of ≤7 and a reductio

Predicting Role of Interleukin-33 in Determining the Development and Severity of Atopic Dermatitis.

INTRODUCTION: Atopic dermatitis (AD), also referred to as atopic eczema, is one of the most common immunological disorders in children. Previous studies have suggested potential roles of interleukin-33 (IL-33) in the onset and progression of AD. This study investigated whether serum IL-33 levels in children with AD are associated with disease severity, immunoglobulin E (IgE) sensitization to food or environmental allergens, and serum IgE concentration. METHODS: The study included 62 children with newly diagnosed AD and 30 age-matched healthy controls. Following an interview and confirmation o

Thermogelling Hydrogel for Sustained Topical Delivery of Quercetin for the Treatment of Atopic Dermatitis.

Atopic dermatitis is a chronic inflammatory skin disease clinically characterized by pruritus, dryness, and eczematous lesions, resulting from a combination of environmental, immunological, and genetic factors. In this study, the quercetin-loaded thermosensitive hydrogel (QU-THG) was developed using the cold method and optimized through a Box-Behnken design. The concentrations of Pluronic F-127 and DMSO, and stirring speed were selected as independent variables, and sol-gel transition temperature, gelation time, and viscosity were evaluated as responses. The optimized formulation contained 18%

Serum LDH, eosinophils, and IgE in atopic dermatitis severity: a retrospective study.

AIMS: To evaluate serum lactate dehydrogenase (LDH), eosinophils, and total immunoglobulin E (IgE) as accessible adjunctive markers of atopic dermatitis (AD) severity. MATERIALS & METHODS: This retrospective study included 265 AD patients and 97 healthy controls. SCORAD scores and laboratory biomarkers obtained within 48 hours were analyzed using ordinal logistic regression. A binary logistic model was used for ROC analysis of severe AD. RESULTS: Severe AD was associated with higher LDH, eosinophil counts, and total IgE than mild-moderate AD. LDH and eosinophils correlated with SCORAD scores

Janus kinase inhibitors enhance the efficacy of dupilumab in refractory atopic dermatitis.

OBJECTIVES: Patients with moderate-to-severe atopic dermatitis (AD) often experience intense pruritus and extensive skin lesions, which markedly impair quality of life. Current treatment options, including biologics and small-molecule agents, may provide limited efficacy and are associated with adverse effects. For patients who respond inadequately to dupilumab, particularly those with insufficient control of pruritus, more effective therapeutic strategies are needed. This study aims to evaluate the efficacy, safety, and clinical value of combining dupilumab with Janus kinase inhibitors (JAKis

A Comprehensive Review of Lipid-Based Topical Nanocarriers for the Treatment of Atopic Dermatitis.

Atopic dermatitis (AD) is a chronic, relapsing inflammatory skin disease affecting 1-3% of adults and 20% of children. It is characterized by type 2 immune dysregulation and epidermal barrier dysfunction. The pathophysiology involves loss-of-function mutations in the filaggrin gene (FLG), tight junction defects, and altered intercellular lipid composition, leading to increased transepidermal water loss (TEWL) and sensitization to environmental allergens. Current treatments, including topical corticosteroids (TCS), calcineurin inhibitors (TCI), and emerging biologics, are limited by cutaneous a

From Barrier Disruption and Immune Dysregulation to Targeted Therapies in Atopic Dermatitis.

Atopic dermatitis (AD) is a heterogeneous inflammatory skin disorder characterized by recurrent eczematous lesions and persistent pruritus. Its pathogenesis involves epidermal barrier dysfunction, immune dysregulation, microbial dysbiosis, and altered neuroimmune signaling. Genetic defects in structural proteins, particularly filaggrin, and altered epidermal lipids increase allergen and microbial penetration, perpetuating itch, scratching, tissue injury, and inflammation. Molecular evidence further reveals AD endotypes beyond conventional clinical phenotypes, with heterogeneity in immune polar

The Effects of Walk-In Dermatology Appointments for Atopic Dermatitis and Hidradenitis Suppurativa Management.

Atopic dermatitis (AD) and hidradenitis suppurativa (HS) are two inflammatory skin conditions characterized by intermittent flares that may lead some patients to seek treatment in urgent care or emergency departments (ED). At our institution, we implemented a walk-in appointment process for patients seen in the ED for urgent dermatologic concerns. After implementation of this program, patients received treatment by a pediatric dermatologist faster: 75.7 days versus 19.1 days for AD pre- and post-implementation, respectively, and 89.9 days versus 9.33 days for HS pre- and post-implementation, r

[The mechanism and research progress of vitamin A in the prevention and treatment of atopic dermatitis in children].

Atopic dermatitis (AD) is a chronic, recurrent, inflammatory, and pruritic skin disease associated with genetic predisposition to allergies. As one of the most common chronic allergic diseases in children, the global incidence of AD has been on the rise in recent years, imposing a significant burden on the quality of life of affected children and the economic status of their families. Vitamin A, as an essential fat-soluble vitamin, not only participates in the visual cycle and maintains epithelial homeostasis but also plays a crucial role in the immune regulation of allergic diseases such as A
