Treatment of metabolic bone disease in patients with chronic renal disease: a perspective for rheumatologists.

As glomerular filtration rate (GFR) declines from age-related bone loss or disease that specifically induces a decline in GFR, there are a number of metabolic bone conditions that may accompany the decline in GFR. These metabolic bone conditions span a spectrum from mild-to-severe secondary hyperparathyroidism in early stages of chronic kidney disease (CKD) to the development of additional heterogeneous forms of bone diseases each with distinctly quantitative bone histomorphometric characteristics. Osteoporosis can also develop in patients with CKD and end-stage renal disease (ESRD) for many r

Nephrotoxicity related to new therapeutic compounds.

Toxic nephropathy is an important cause of reversible renal injury. This article focuses on the nephrotoxicity of several new therapeutic compounds. Selective cyclooxygenase-2 inhibitor is associated with sodium retention, hypertension, ankle edema, and acute renal failure. The incidence of renal complication is similar to conventional nonsteroidal anti-inflammatory drugs. Bisphosphonates, especially when used in high dose for prolonged duration, can cause toxic acute tubular necrosis and renal failure. Pamidronate is also associated with a specific form of collapsing focal segmental glomerulo

Treatment of postmenopausal osteoporosis.

Osteoporosis is a major public health burden. The most devastating outcome of osteoporosis is fracture, which results in increased morbidity and mortality. These fractures most often occur in the vertebrae and indicate an increased risk of future vertebral and hip fractures. Consequently, it is important to identify patients at risk for fracture and to intervene with pharmacologic therapies, lifestyle changes, or both to reduce the frequency of the first or subsequent fracture. Moreover, because osteoporosis is a chronic condition requiring long-term therapy, factors that increase compliance a

[Osteitis deformans--Paget's disease].

Paget's disease of the bone is a disorder in which there is marked increase in bone turnover by activated osteoclasts in localized parts of the skeleton. The newly formed bone is abnormal in structure and mineralization resulting in deformity and increased risk of fracture. The prevalence of the disease has decreased over the last 20 years to 2% in patients over 55 years of age. The etiology of the disease is still unknown. There is strong evidence for viral infection, but recently a genetic disposition has also been discussed. There are important advances in the understanding of the pathophys

[Bisphosphonates in the prevention of vertebral and non-vertebral fractures in postmenopausal women with osteoporosis].

Bisphosphonates (risedronate and alendronate) inhibit bone resorption, increase bone mass and reduce the risk of vertebral and nonvertebral fractures in postmenopausal women as has been shown in a number of clinical trials. The concept of bone quality has recently been actualized with evidence that antiresorptive drugs reduce the occurrence of fractures to a greater extent than would be predicted by the amount of the increase in bone density. Concern about possible negative effect on fracture resistance with prolonged treatment with bisphosphonates has no evidence from clinical studies. Both p

Intramedullary rodding and bisphosphonate treatment of polyostotic fibrous dysplasia associated with the McCune-Albright syndrome.

The authors report the results of the management of five consecutive children with McCune-Albright syndrome. These children were treated with a combination of drug treatment (bisphosphonates) and surgical treatment with elongating intramedullary rods (Sheffield) for management of femoral and tibial lesions. This treatment was successful in all patients, as judged by improvement in their quality of life and in clinical parameters, such as decreased bone pain and fracture rate, and improved walking ability. Two of the five children had been wheelchair-bound before treatment. All children are now

Zoledronic acid significantly reduces pathologic fractures in patients with advanced-stage prostate cancer metastatic to bone.

The skeletal complications of metastatic bone disease secondary to advanced prostate cancer result in significant morbidity. In particular, pathologic fractures often require clinical intervention and are independent predictors of mortality in men with advanced prostate cancer. Before the introduction of zoledronic acid, bisphosphonates had been shown to provide pain palliation in patients with prostate cancer and bone metastases but were not efficacious in preventing skeletal complications. Zoledronic acid is the first bisphosphonate to show efficacy in reducing skeletal complications associa

Nonoperative management of hyperparathyroidism: present and future.

Parathyroidectomy provides effective treatment for primary and secondary hyperparathyroidism with a predictable response of symptoms related to hypercalcemia and elevated parathyroid hormone. Calcium and vitamin D supplementation has reduced the need for parathyroidectomy in dialysis patients with secondary hyperparathyroidism. However, surgery continues to be the only effective treatment of primary hyperparathyroidism. Potential nonoperative treatments for hyperparathyroidism have included the use of estrogen replacement, bisphosphonates, and a new class of drugs known as calcimimetics. Hormo

Preventing glucocorticoid-induced osteoporosis.

Glucocorticoids are a potent cause of osteoporosis and thus a potential source of substantial morbidity in patients who are already significantly disabled by their underlying condition. These agents interact with calcium metabolism at many levels; in particular they reduce the osteoblast's synthesis of the principal proteins of bone matrix. They also reduce circulating sex hormone concentrations, particularly in men and postmenopausal women. Glucocorticoids cause rapid bone loss within weeks of their introduction, but this loss continues even after many years of chronic use. About one third of

[Distribution of bone metastases of cancers. A scintigraphic study of 376 cases].

Technetium 99m methylene bisphosphonate bone scans of 376 patients with cancers of breast, prostate, lung, kidney, colon, and bladder and ENT cancer were reviewed, and the distribution of skeletal metastases was analyzed. Differences were not significant for rank order of metastatic involvement in 9 selected regions in any cancer, but the breast carcinoma. Patients with breast cancer had less pelvis and more skull involvement. The rate of skull metastases was significantly higher in breast cancer than in prostatic (p < 0.001), lung (p < 0.01) and kidney (p < 0.05) cancers. These results are we

[Tolerability of oral bisphosponates in patients with osteoporosis and other osteopathies].

Oral nitrogen containing bisphosphonates (NCB) are effective drugs to inhibit bone metabolism turnover in osteoporosis and other bone diseases. Notwithstanding, some digestive disturbances create concern on the long term acceptance of the oral route. Side effects are mainly caused by low absorption and poor solubility in digestive content. Therefore the compound may precipitate and irritate the mucosas. Furthermore, the administered amount of a particular molecule, its intrinsic potency to irritate digestive walls and the degree of exposition to such sensitive tissue are other facts that combi

Monostotic Paget's disease involving the calcaneus. Diagnostic and therapeutic problems. Two case-reports.

The authors report the cases of two patients who had chronic incapacitating heel pain unresponsive to standard therapy. They were both found to have Paget's disease of the calcaneus. The diagnosis was difficult because the typical roentgenological changes required time to develop and because no other sites were involved. Local corticosteroid injections, elimination of weight-bearing, and standard analgesic therapy were ineffective. Bisphosphonate therapy (pamidronate) given as intravenous infusions ensured prompt lasting pain relief, making ambulation possible.

Bisacylphosphonates inhibit hydroxyapatite formation and dissolution in vitro and dystrophic calcification in vivo.

Some geminal bisphosphonates are used clinically for a number of important bone/calcium related diseases; however, side effects and lack of selectivity impede their wide use. This work reports the synthesis and evaluation of bisacylphosphonates (e.g., adipoyl- and suberoylbisphosphonate). These compounds were found to inhibit significantly hydroxyapatite formation and dissolution in vitro and the calcification of bioprosthetic tissue implanted subdermally in rats. These are the first instances of nongeminal bisphosphonates [P-(C)n-P, n greater than or equal to 2] that have been reported to be

Rationale for the use of bisphosphonates in bone metastases.

Neoplasia affecting the skeleton is an important cause of morbidity, which includes hypercalcaemia, bone pain and fracture. In most instances these events are mediated by an increase in the resorption of bone which decreases bone density and disrupts skeletal architecture, either at focal sites or generally throughout the skeleton. Neoplastic activation of bone resorption in heterogeneous, but there is now good evidence that this is due to the increased activation of osteoclasts, the cells which mediate bone resorption in health. Bisphosphonates are specific inhibitors of osteoclast-mediated b

The detrimental effect of aminohydroxypropylidene bisphosphonate (APD) in otospongiosis.

Two middle-aged female patients with active stapedial otosclerosis and sensorineural hearing loss have been treated with aminohydroxypropylidene bisphosphonate (APD). No effect on the Schwartze sign was observed either during or following therapy. No changes were seen in the audiometric data of the patients during the 1st year. Then, within a few weeks, both patients complained of sudden deterioration of the existing hearing losses in both ears, with an alteration of tinnitus. One patient became totally, bilaterally deaf, while the other retained minimal auditory function in the low frequencie

[Treatment of patients with trigeminal neuralgia by xidifon electrophoresis].

Fifty-two patients with trigeminal neuralgia were treated by electrophoresis with a 2% solution of ksidifon. 23 patients demonstrated a remarkable analgetic effect in the form of disappearance of painful paroxysms and activity of the trigger zones, which enabled a considerable reduction of the daily dose of carbamazepine (finlepsin). In 14 patients, the rate and severity of algic attacks noticeably decreased without the lowering of the finlepsin dosage. Therefore, the suggested method of the treatment of trigeminal neuralgia, being simple in use, effectively removes the painful syndrome, contr
