Arthroscopic and open management of posterolateral rotatory instability of the elbow.

Posterolateral rotatory instability (PLRI) is the most common cause of residual instability following a simple elbow dislocation. PLRI may result from trauma or iatrogenic injury to the radial ulnohumeral ligament during treatment for other conditions, such as lateral epicondylitis. PLRI can be identified through a combination of history and physical examination, and confirmed with magnetic resonance imaging arthrography. Once diagnosed, surgery is necessary to correct persistent instability. Instability can be confirmed arthroscopically through several findings, including subluxation of the r…

Electrodiagnosis of ulnar neuropathy at the elbow (Une): a Bayesian approach.

INTRODUCTION: In ulnar neuropathy at the elbow (UNE), we determined how electrodiagnostic cutoffs [across-elbow ulnar motor conduction velocity slowing (AECV-slowing), drop in across-elbow vs. forearm CV (AECV-drop)] depend on pretest probability (PreTP). METHODS: Fifty clinically defined UNE patients and 50 controls underwent ulnar conduction testing recording abductor digiti minimi (ADM) and first dorsal interosseous (FDI), stimulating wrist, below-elbow, and 6-, 8-, and 10-cm more proximally. For various PreTPs of UNE, the cutoffs required to confirm UNE (defined as posttest probability = …

Clinical presentation of posterolateral rotatory instability of the elbow in children.

BACKGROUND: Posterolateral rotatory instability is a type of ulnohumeral instability seen following elbow trauma. It is caused by a deficiency in the lateral collateral ligament complex that allows the radius and ulna to subluxate as a single unit with respect to the distal part of the humerus. There are few studies on this type of instability in children. Our purpose was to evaluate cases of posterolateral rotatory instability in children to better understand its presentation and manifestation as compared with those in adults. METHODS: This was a retrospective chart review of patients from t…

Dislocations of the elbow in children: long-term follow-up.

BACKGROUND: Dislocation of the elbow in children is considered to be a benign injury in most cases. The aim of this paper is to evaluate whether this condition has late sequelae in the adult age. The study consisted of a retrospective evaluation of patients under 15 years old at the time of injury. METHODS: 40 patients were selected for the follow-up evaluation. The mean age of the children at the time of injury was 10.9 years old (range 5-14 years). Four patients were lost to follow-up, so they were not considered for final clinical evaluation. For the other 36 patients, the follow-up examin…

Minimal-incision in situ ulnar nerve decompression at the elbow.

With initiatives to decrease operative morbidity, complications, and associated costs, minimalincision techniques have found an expanding role within multiple specialties. Minimal-incision in situ open techniques for ulnar nerve release at the elbow provide adequate exposure and reproducible, satisfactory outcomes. Furthermore, there is no need for endoscopic equipment and the resultant dependence on staff adequately trained to operate and troubleshoot equipment. More robust research with a focus on complications and standard outcome measures will be required to further define the role of mini…

Maximum force levels in different positions of shoulder and elbow.

The number of occupational diseases in Mexico is alarming and clearly it is important to do studies with the purpose of improving the design of workstations. The objective of this research is to determine the maximum force levels in different positions of shoulder and elbow. An experiment was conducted with 16 subjects between 18 and 28 years old: 8 male and 8 female. We considered 16 different positions, working with the right and left arm to perform the tasks of pull and push. The tasks consisted of pushing or pulling a dynamometer for a period of 3 seconds as hard as possible. The results w…

Evaluation and treatment of failed ulnar nerve release at the elbow.

Failure after ulnar nerve decompression at the elbow can be defined as either no change in the patient's symptoms or an initial improvement with recurrence, making the patient history essential in the work-up. Failure may be due to diagnostic, technical, or biologic factors. Technical errors and the development of perineural fibrosis necessitate revision surgery, while nerve damage due to chronic severe compression should be observed. We do not believe any one procedure is superior in the revision setting as long as a complete decompression is achieved with a compression free, stable transposi…

An uncommon cause of elbow synovitis in an adult haemophilia patient.

We present the case of an adult haemophilia patient that at clinical examination showed intense painless synovitis in his left elbow, with full range of movement, which was unresponsive to 1 month of on-demand haematologic treatment at home. Radiological examination demonstrated a spontaneous displaced fracture of the olecranon secondary to severe osteolysis, due to advanced haemophilic arthropathy. Then, the patient was treated with a posterior plaster splint to immobilize the joint for 3 weeks together with secondary prophylaxis (3000 IU twice a week for 3 months). One month later the patien…

Evaluation of semi-quantitative bone scintigraphy in canine elbows.

Scintigraphy is an extremely sensitive tool for the detection of early changes in bone metabolism. Sixty-eight lame dogs underwent a scintigraphic examination. For each elbow lateromedial (LM), caudomedial (CdM) flexed and caudomedial (CdM) extended scintigraphic views were obtained. Semi-quantitative analysis was performed to determine radiopharmaceutical uptake at the medial coronoid process (MCP) and at the attachment of the flexor muscles at the medial humeral epicondyle, normalised to activity registered in either the total elbow joint or the radius/ulna. MCP pathology/flexor enthesopathi…

MRI and arthroscopy correlations of the elbow: a case-based approach.

The number of elbow arthroscopies and indications for the procedure have increased significantly since the advent of modern elbow arthroscopy in the 1980s. In addition to the patient history, physical examination, and plain radiography, MRI is an important tool for the clinician in diagnosing several pathologies within and around the elbow. Understanding the pathophysiology and clinical presentation and being familiar with the MRI characteristics of a variety of elbow conditions will assist the physician in making an accurate diagnosis and help guide appropriate treatment.

Rehabilitation considerations in the management of terrible triad injury to the elbow.

An elbow dislocation associated with a radial head and coronoid fractures is termed a terrible triad. This injury almost always renders the elbow unstable requiring surgical intervention. The primary goal of surgery is to stabilize the elbow to permit early motion to prevent stiffness. Recent literature has improved our understanding of elbow anatomy and biomechanics as well as the pathoanatomy of this injury. This article reviews key concepts that will allow the surgeon and therapist to apply an systematic rehabilitation approach when managing such injuries.

Normal and variant anatomy of the elbow on magnetic resonance imaging.

Magnetic resonance imaging (MRI) provides excellent delineation of the bones of the elbow and the surrounding soft tissue structures. The components of the elbow can be divided into osseous structures, the joint capsule and ligaments, muscles and tendons, and nerves. In this article, the authors review the normal anatomy and appearance of these structures on MRI as well as the anatomic variants that should be recognized and distinguished from pathologic entities.
