Selecting a spectrometric detector for drinking water monitoring in the case of nuclear and radiological emergencies.

Rapid and reliable radionuclide-specific monitoring of drinking water may be required following a radiological or nuclear (RN) emergency. In this study, ten gamma-detector configurations were evaluated for continuous drinking-water monitoring using Monte Carlo simulations combined with measured detector backgrounds. Two source terms were investigated, representing fallout from a nuclear weapon detonation and a nuclear power plant accident, each at 48 h and 7 d after release. Two measurement geometries were simulated: a 4 L Marinelli beaker and a shielded detector positioned directly against a

A Critical Care Cardiology Perspective on Managing Acute Emergencies in Patients With Durable Ventricular Assist Devices.

Durable left ventricular assist devices (dLVAD) remain a lifesaving therapy in patients with stage D heart failure that is refractory to conventional medical therapies. Owing to improvements in technology and patient outcomes, the number of patients supported with dLVADs has increased over the past decade. Despite this growing population, there are few resources for cardiovascular intensivists that integrate epidemiology, diagnostic workup and multidisciplinary medical management of acute emergencies in patients supported with dLVADs.

Ketamine Versus Propofol for Sedation in Acute Psychiatric Emergencies During Aeromedical Retrieval: A Randomized Clinical Trial.

OBJECTIVES: The prevalence of psychiatric disease is increasing globally. Regional Australian communities are disproportionately affected and uniquely lacking mental health resources. Those needing inpatient psychiatric care often require aeromedical retrieval, creating safety and operational challenges. The optimal medication regimen for sedation of non-intubated patients experiencing psychiatric agitation is unclear. METHODS: We performed a prospective, single-center, open-label, phase IV randomized clinical trial (RCT) of ketamine versus propofol for the sedation of acutely agitated patien

Prompt-to-policy: Leveraging large language models to guide deep reinforcement learning in public health emergencies.

Rapid and effective decision-making is critical in public health emergencies, where resource allocation must balance multiple objectives under uncertain conditions. Traditional optimization methods often struggle with computational complexity and real-time, heterogeneous data. To address these challenges, we propose a hybrid intelligent agent combining an enhanced Double Deep Q-Network (D2QN-JDA) with large language models (LLMs). The D2QN-JDA improves learning stability and adaptability through joint state-action inputs, dynamic exploration rate, and adaptive reward normalization. The LLM com…

Patient-centered care for patients requiring dialysis during disasters and public health emergencies: a scoping review.

BACKGROUND: Disasters and public health emergencies disrupt health systems, threaten continuity of care, and disproportionately affect medically vulnerable populations. Patients requiring dialysis are especially at risk because their survival depends on regular access to complex, resource-intensive treatment. While disaster nephrology literature increasingly addresses preparedness and resilience, it remains unclear how explicitly this literature addresses patient-centered care. OBJECTIVE: To map the literature on patient-centered care for patients requiring dialysis during disasters and publi…

Death Anxiety Status and Coping Strategies of Front Line Medical Staff During the Public Health Emergencies.

BACKGROUND AND AIM: During the process of responding to epidemics stemming from sudden public health emergencies, front line medical staff often experience death anxiety to varying degrees; however, this phenomenon has not been the focus of extensive attention. In this study, using the outbreak of the COVID-19 pandemic as a case example, we aimed to investigate the status of death anxiety and corresponding coping strategies among front line medical staff within the context of public health emergencies. METHODS: By convenient sampling, 170 front line medical staff were recruited into this stud…

Emergency Physicians' Confidence in Managing Obstetric and Gynecologic Emergencies in Saudi Arabia: A Cross-Sectional Study.

BACKGROUND: Obstetric and gynecologic emergencies are time-sensitive conditions encountered in emergency departments and may result in substantial maternal morbidity and mortality if not promptly recognized and managed. Emergency physicians are frequently the first clinicians to evaluate pregnant patients presenting with acute complications, yet exposure to obstetric emergencies and related procedural training may be limited. This study assessed the confidence of emergency physicians in Saudi Arabia in managing obstetric and gynecologic emergencies and identified factors associated with higher…

Characteristics and outcomes of diabetes emergencies in nonagenarians admitted to ICU: a binational retrospective cohort study.

INTRODUCTION: Nonagenarian patients admitted to Intensive Care Units (ICU) are expected to rise with an ageing population. However, diagnosis-specific data is lacking in this cohort to guide clinical decisions. This includes diabetic emergencies, namely hyperosmolar hyperglycemic state (HHS) and diabetic ketoacidosis (DKA), which carry substantial morbidity and mortality. We sought to determine the incidence, clinical characteristics, and outcomes of nonagenarians admitted to ICUs across Australia and New Zealand (ANZ) with DKA and HHS to provide foundations for evidence-based prognostication …

Interhospital transport of patients with time-critical cardiac emergencies by an adult critical care transfer team.

INTRODUCTION: Due to centralisation of cardiac care, patients with acute cardiac conditions commonly require interhospital transfer. We aimed to describe the epidemiology and management of patients with time-critical cardiac emergencies by one UK Adult Critical Care Transfer Service (ACCTS) and identify potentially modifiable factors associated with prolonged packaging and transfer time. METHODS: We conducted an observational cohort study of all patients referred to the ACCESS London ACCTS between 1 April 2023 and 1 April 2025 where the reason for transfer was escalation of care for specialis…

Preventing and Managing Tracheostomy-Related Emergencies in the Radiology Suite: Best Practices for Safety and Preparedness.

BACKGROUND: Airway emergencies in patients with tracheostomies are life-threatening and may arise from tube dislodgment, obstruction, bleeding, or improper handling during patient transfers. While these incidents are rare in radiology suites, the limited experience of staff with tracheostomy care heightens the risk of adverse outcomes. Prompt and well-coordinated responses are essential to ensure patient survival. OBJECTIVE: This review identifies risk factors for tracheostomy-related emergencies in radiology and offers evidence-based recommendations to improve patient safety, staff preparedn…

Management of Medical Emergencies on Psychiatry Inpatient Floors: A Novel Simulation-Based Curriculum for Psychiatry Residents.

Introduction For most psychiatry residents, training in managing the early phases of acute medical emergencies is limited to exposure during medical school and junior off-service rotations. Patients with psychiatric illnesses, however, often have higher rates of medical comorbidities than their age-matched controls. We developed a half-day medical refresher and simulation curriculum to improve the confidence and efficacy with which psychiatry residents can manage medical emergencies on inpatient floors. Methods Based upon a detailed needs assessment conducted with psychiatry residents at vario…
