Serial endomyocardial biopsies (EMBs) are scheduled even several years after heart transplantation (HTx) to monitor for late rejection (LR). Starting in January 1995, we performed heart transplantation, randomly using standard and bicaval techniques. In the latter technique, the anatomy of the right atrium is maintained, since the venae cavae are anastomosed. Over 200 endomyocardial biopsies in pediatric patients have been performed since 1962, and an appropriate interpretation of myocardial hypertrophy in biopsy specimens at different ages is now necessary. The diameter of myocytes was measured in 108 biopsy specimens and 209 autopsy specimens from pediatric cases. Thirty-six hearts of patients who died with indwelling Swan-Ganz catheters in place were prospectively examined to assess the incidence and extent of localized lesions in the right side of the heart. Bland mural thrombosis in the superior vena cava, the right atrium and the pulmonary artery was found in 22 of 36 cases (61%). A technique for simplified heterotopic rat heart transplantation having only aorto-aortic anastomosis is presented.
