Railway train versus motor vehicle collisions: a comparative study of injury severity and patterns.

OBJECTIVE: This study compares the demographics, injury severity, resource use, and injury patterns of patients involved in railway train-motor vehicle (RT-MV) to motor vehicle-motor vehicle (MV-MV) collisions. METHODS: Retrospective trauma registry review of 74 RT-MV and 1,931 MV-MV consecutive patients, age more than 14 years, presenting to two Level I trauma centers, January of 1991 to May of 1998. RESULTS: Compared with MV-MV, RT-MV had significantly more males (72% vs. 54%), higher mortality (15% vs. 7%), higher Injury Severity Score (median, 20 vs. 9), longer intensive care unit length

Routine post-traumatic radiographic screening of midfacial injuries: is one view sufficient?

Midfacial injuries are a common reason for attendance at Accident and Emergency (A&E) departments. A&E staff find these injuries difficult to assess due to limited undergraduate training in maxillofacial examination and early tissue oedema which may mask asymmetry. Patients are therefore referred for 'facial views' radiographs and commonly three views (OM15, OM30, lateral face) are taken for screening. We reviewed prospectively 137 consecutive patients over a 5-month period to determine whether one view (OM15) was sufficient to make a diagnosis in addition to clinical examination. Eighty-three…

Jumping Trends In Trampoline-Related Craniofacial Injuries.

INTRODUCTION: In 1990, the American Academy of Pediatrics recommended the sale of trampolines cease in the United States. The risk of traumatic injury is well-documented, yet trampolines remain a growing source of recreation. Trampoline parks, in particular, are becoming a common entertainment attraction that can result in serious injury. There is currently a paucity of literature characterizing craniofacial injuries from trampoline use and plans to prevent these injuries. METHODS AND MATERIALS: A retrospective review of the National Electronic Injury Surveillance System was conducted to anal…

E-Scooters and Craniofacial Trauma: A Systematic Review.

STUDY DESIGN: Systematic review. OBJECTIVE: Standing electric scooters (e-scooters) provide a cheap and environmentally friendly transport alternative, but also elicit substantial concern regarding their volume of associated injuries especially in the craniofacial region. This review aims to explore the demographics, risk factors, types of injury and surgical management of craniofacial trauma associated with e-scooters. METHODS: PubMed and Scopus databases were systematically searched. Inclusion criteria were clinical studies investigating craniofacial trauma associated with e-scooters. Excl

Extensive neuronal cell death following intracranial transection of the facial nerve in the adult rat.

The aim of the present study is to examine the neuronal degeneration and the glial response following intracranial transection of the facial nerve close to the brainstem and furthermore to compare the results with a distal nerve injury. The facial nerve was cut either intracranially in the posterior cranial fossa or further distally, where it passes the parotid gland, in adult rats. Intracranial axotomy caused a massive loss of neuronal profiles. Only 26.8+/-11.3% of facial motor neuronal profiles were found ipsilateral to the nerve injury when compared to the contralateral side, following int

Immediate compared with late repair of extracranial branches of the facial nerve: a comparative study.

The best outcomes after injury to the facial nerve are seen after immediate direct coaptation, but in practice, this happens infrequently. We ask whether late repair (between 3 weeks and 18 months) is comparable to immediate repair. In this prospective observational study over a two-year period (2016-18), we identified 18 patients (11 male and 7 female, mean (range) age 58 (23-94) years), who had sustained extracranial injuries to the facial nerve. Eight were identified in the acute phase (within 72hours of injury) and repaired (immediate repair group). Ten presented in the late phase beyond s

Rate and extent of early axonal degeneration of the human facial nerve.

Electrophysiologic and histologic studies on a severed human facial nerve have shown that 1) response to stimulation by electroneuronography was lost on the sixth day following injury, 2) wallerian degeneration progressed in a nonuniform "front," leaving some of the distal stump axons still viable and responsive to intraoperative stimulation 2 days following injury, and 3) the extent of proximal retrograde degeneration was far greater than to the next node of Ranvier. These findings are discussed in light of the current knowledge regarding peripheral nerve degeneration, and explanations are su

Communication of infraorbital nerve and facial nerve: anatomic and histologic study.

The maxillary nerve, second division of the trigeminal nerve, is entirely sensory. It has been reported that drooling may occur later in the event of fracture of the zygoma in which hypesthesia prevails. The aim of the study is to elucidate additional detailed anatomy of the infraorbital plexus, consisting of the superior labial branch of the infraorbital nerve and facial nerve in the cheek. The authors dissected infraorbital nerves and facial nerves in 16 cadavers. Most terminals of the zygomatic branch of the facial nerve emerged from under the levator labii superiors and zygomatic muscle an

Toward CT-based facial fracture treatment.

Facial fractures have formerly been classified solely by anatomic location. CT scans now identify the exact fracture pattern in a specific area. Fracture patterns are classified as low, middle, or high energy, defined solely by the pattern of segmentation and displacement in the CT scan. Exposure and fixation relate directly to the fracture pattern for each anatomic area of the face, including frontal bone, frontal sinus, zygoma, nose, nasoethmoidal-orbital region, midface, and mandible. Fractures with little comminution and displacement were accompanied by subtle symptoms and required simple

Transdural Stimulation for the Identification of a Dehiscent Geniculate Ganglion in Intradural Middle Cranial Fossa Surgery: A Proof-of-Principle Study.

PURPOSE: Facial nerve injury is a potential complication in intradural middle cranial fossa surgery, particularly because of dehiscence of the geniculate ganglion, exposing the nerve to thermal injury. Identifying the geniculate ganglion during surgical approaches is crucial for preserving facial nerve function. This study aimed to evaluate the feasibility and efficacy of transdural stimulation in identifying the geniculate ganglion during intradural approaches to improve the preservation of facial nerve function. METHODS: We conducted a retrospective analysis on patients who underwent surger

Defining a Preauricular Safe Zone: A Cadaveric Study of the Frontotemporal Branch of the Facial Nerve.

BACKGROUND: In the preauricular region, the frontotemporal branch of the facial nerve is vulnerable to injury, which can result in facial palsy and poor cosmesis after surgical interventions. OBJECTIVES: The purpose of this study was to describe variations in the branching patterns of the frontotemporal branch of the facial nerve and the relation between this branch and the surrounding anatomic landmarks. Based on our findings, we propose a Danger Zone and Safe Zones for preauricular interventions to avoid frontal branch injury. METHODS: Twenty cadaveric half-heads, 10 freshly frozen and 10

Surgical outcomes of transmastoid facial nerve decompression: Preliminary data from a Malaysian tertiary hospital from 2013-2018.

INTRODUCTIONS: Facial nerve palsy (FNP) occurs in 7-10% of temporal bone fractures. The aim of this study was to review the surgical outcome of nine patients with severe to complete traumatic facial nerve (FN) injury. METHODS: The patients were evaluated clinically and FNP was graded using the House Brackmann (HB) scale. High resolution computerized tomography (HRCT) of the temporal bone was used to evaluate temporal bone fractures. Transmastoid facial nerve decompression was performed and the facial nerve function was re-evaluated in subsequent follow ups. RESULTS: There were five cases wit

Inducible galanin and GalR2 receptor system in motor neuron injury and regeneration.

Galanin has been ascribed several physiological roles that are thought to be mediated via multiple galanin receptors. Recently, two galanin receptors--galanin receptor-1 (GalR1) and galanin receptor-2 (GalR2)--have been cloned and characterized and shown to have differences in amino acid sequence, pharmacology, and second messenger signaling systems. Previous studies have demonstrated an up-regulation of galanin expression in damaged neurons of several different types. Using in situ hybridization histochemistry this study investigated whether adult cranial motor neurons express mRNAs encoding
