Preoperative BMD does not influence femoral stem subsidence of uncemented THA when the femoral T-score is > -2.5.

Background and purpose - It is believed that in uncemented primary total hip arthroplasty (THA) the anchorage of the stem is dependent on the level of bone mineral density (BMD) of the femoral bone. This is one of the reasons for the widely accepted agreement that a cemented solution should be selected for people with osteoporosis or age > 75 years. We evaluated whether preoperative BMD of the femur bone is related to femoral stem migration in uncemented THA.Patients and methods - We enrolled 62 patients (mean age 64 years (range 49-74), 34 males) scheduled for an uncemented THA. Before surger

Management of Metastatic Spine and Hip Alveolar Soft Part Sarcoma: Case Report and Review of Literature.

Alveolar soft part sarcoma (ASPS) is a rare soft tissue tumor. Primary or metastatic involvement of the spine is unusual in ASPS. In most case, it is refractory to chemotherapy and radiation. Surgical resection is the most effective intervention. We report the case of a 38-year-old female having ASPS along with metastatic spine and hip involvement treated surgically as a single-stage operation, which is the first of its kind approach to our knowledge. We present the case of a 38-year-old female with simultaneous L4 pathological fracture with symptomatic lumbar canal stenosis without focal neur

Low Bone Mineral Density of the Forearm and Femur among Postmenopausal Women with Metaphyseal Comminuted Fracture of the Distal Radius.

Osteoporosis is characterized by bone loss and skeletal fragility and is likely to occur in postmenopausal women. Distal radius fracture is a type of fragility fractures associated with osteoporosis. Bone mineral density (BMD) refers to the amount of mineral in bone tissue and is an indicator of osteoporosis. This study aimed to investigate the relationship between the severity of distal radius comminution and the BMD of the healthy contralateral forearm and femur in postmenopausal women. Of 165 women who sustained low-energy trauma from falls on flat ground, forearm and femoral neck BMDs were

Ipsilateral femoral neck, intertrochanteric and acetabular fractures with posterior dislocation of the hip: A case report and literature review.

INTRODUCTION: Traumatic hip dislocation with ipsilateral femoral neck fracture, intertrochanteric fracture and acetabular fracture is a rare and serious injury that occurs mostly in young patients and after high-energy trauma. The treatment of these injuries is a major challenge for orthopedic surgeons; it can have devastating consequences if not treated properly, including avascular necrosis of the femoral head and traumatic osteoarthritis of the hip. In previous case reports only internal fixation of femoral neck fracture was performed without revascularisation and there was a lack of long-t

Can one really measure magnesium deficiency using the short-term magnesium loading test?

OBJECTIVE: To compare a 1-h-version of a magnesium-loading-test (MLT) designed for outpatients in healthy controls with the 8-h standard; to … the formula: 1 - [Mg 24 h-urine/Mg test dose] x 100. Femur Mg was measured by AAS in a peace of the femur neck. Patients after renal transplantation were randomized after the first Mg load to either obtain daily 5 mmol Mg-aspartate hydrochloride per kilogram bodyweight, or placebo. Four months later a second loading-procedure was performed. MAIN OUTCOME MEA …

Medial migration of lag screw with intrapelvic dislocation in gamma nailing--a unique problem? A report of 2 cases.

Within the last decade, intramedullary nailing systems have gained popularity in the treatment of unstable fractures of the trochanteric region with good clinical results. However, these implants are not free of complications. Commonly reported treatment failures include implant cutout through the femoral neck, stress-derived femur fractures at the distal end of the nail, and secondary rotational displacement and varus deformity of the femoral neck and head. Only few reports exist on primary medial migration of the lag screw after treatment with intramedullary implants. We report on a rarely d

Combined procedure of open reduction and shortening of the femur in treatment of congenital dislocation of the hips in older children.

A one-stage, combined operative procedure for reduction of congenitally dislocated hips in older children consists of shortening of the femur; open reduction by an inferior approach to the joint; reconstruction of the acetabular roof; correction of anteversion of both the femoral neck and the neck-shaft angle; anterior transposition of the iliopsoas muscle. On 60 hips operated in children, ages 5 to 15 with a follow-up period, ranging from 5 to 9 years, the results were found to be excellent in 3 per cent, good in 60 per cent, fair in 30 per cent, and poor in 7 per cent. The procedure is indic

Predicting femoral neck screw pullout strength using DXA and novel assessments of bone microarchitecture.

UNLABELLED: This study determined how well DXA and other novel assessments of bone microarchitecture predicted screw pullout strength in the femoral neck. Hip-specific DXA performed as well as the novel approaches at predicting screw pullout. These findings support the use of DXA to evaluate bone quality and aid in surgical decision-making. PURPOSE: The primary purpose of our study was to determine how well dual-energy x-ray absorptiometry (DXA), trabecular bone score (TBS Ortho), 3D-Shaper, and quantitative computed tomography (qCT) predicted screw pullout of cannulated screws placed in the

[Finite element analysis of adding one transverse screw for Pauwels type Ⅲ femoral neck fractures].

OBJECTIVE: To investigate whether adding 1 transverse screw (TS) to the triangular parallel cannulated screw (TPCS) fixation has a mechanical stability advantage for Pauwels type Ⅲ femoral neck fractures by conducting finite element analysis on four internal fixation methods. METHODS: Based on CT data of a healthy adult male volunteer's femur, three Pauwels type Ⅲ femoral neck fracture models (Pauwels angle 70°, Pauwels angle 80°, and Pauwels angle 70° combined with bone defect) were constructed using Mimics 21.0 software and SolidWorks 2017 software. Four different internal fixation models w

Spatial distribution of hip cortical thickness in postmenopausal women with different osteoporotic fractures.

UNLABELLED: Few studies h ave discussed the association between cortical bone outside the fracture site and the fracture itself. Focusing on hip cortical thickness, this study revealed distinct distributions of the parameters for hip (trochanteric or femoral neck), vertebral, and peripheral osteoporotic fractures and suggested that the spatial distribution of hip cortical thickness was fracture-specific. PURPOSE: Cortical bone is critical for bone strength. Hip cortical thickness is reported to be closely associated with the incidence of hip fractures, but its relationship with nonhip fractur

Slipped Capital Femoral Epiphysis in an Adult with Panhypopituitarism: A Case Report.

CASE: Our patient was a 31-year-old woman with previously undiagnosed panhypopituitarism who presented with bilateral slipped capital femoral epiphysis (SCFE). Together with endocrinology, her treatment plan involved medical management of her panhypopituitarism and surgical correction with femoral neck osteotomy. Hormone deficiencies were corrected 6 weeks after replacement therapy began, and the patient was cleared for surgery. By 5 months after osteotomy, examination revealed delayed union and prominent screws bilaterally. Hardware exchange and repeat internal fixation were then performed. A

Loading modality and age influence teriparatide-induced bone formation in the human femoral neck.

Teriparatide (TPTD) reduces risk of both vertebral and nonvertebral fracture, but increases bone mineral density (BMD) much more at the spine than the hip. TPTD and mechanical loading may have a synergistic anabolic effect on BMD, which may help explain these site-specific differences. Under normal daily activity, the femoral neck (FN) is under bending, placing one side under tension and the other under compression. We sought to further understand the relationship between mechanical loading and TPTD at the hip by investigating the effect of tensile versus compressive loading on TPTD stimulated

Fragility fractures of the proximal femur: review and update for radiologists.

Proximal femoral fragility fractures are common and result in significant morbidity and mortality along with a considerable socioeconomic burden. The goals of this article are to review relevant proximal femoral anatomy together with imaging, classification, and management of proximal femoral fragility fractures, and their most common complications. Imaging plays an integral role in classification, management and follow-up of proximal femoral fragility fractures. Classification of proximal femoral fragility fractures is primarily based on anteroposterior hip radiographs. Pertinent imaging feat

[Recurrent posterior traumatic luxation of the hip and anteversion defect of the femoral neck in adults. Apropos of a case, general review of the literature].

The recurrent traumatic posterior dislocation of the hip is exceptional. On the reported observation, an anteversion fault of the femoral neck was demonstrated. This deformity had already been pointed out in cases of simple traumatic dislocation. It had never been noted in case of recurrent dislocation. It appears logical in case of important fault, not to be satisfied with a posterior capsulorrhaphy, but to combine it with a femoral rotation osteotomy.

Histomorphometric study of the dynamics of mineralization in a case of osteomalacia caused by anticonvulsants treated with 25(OH)d3.

The authors describe the experimental method used to make histomorphometric determinations in a case of osteomalacia caused by anticonvulsant drugs treated with 25(OH)D3. This comprised double fluorescent markings repeated at set time intervals after taking biopsy samples from the patient. This combined method of study enabled the effect of the treatment to be monitored throughout its course.
