[Stress reactions and stress fractures in the high performance athlete. Causes, diagnosis and therapy].

From 1987 until July 1991 70 athletes with stress reactions or stress fractures were treated in the orthopaedic department of the Hannover Medical School. The average age of the 42 male and 28 female athletes was 22.6 years. The number of athletes involved in track and field sports was 29 (41.4%), in gymnastics 9 (12.9%) and in soccer 5 (7.1%). The most common bone injured was the tibia in 29 (41.4%), followed by the tarsal navicular in 21 (30.0%), the midfoot in 17 (24.3%) and the fibula in 4 (5.7%) athletes. In three cases double stress fractures were found in adjacent locations; in one case

Presentation of cervical metastases and pathological mandibular fracture due to pulmonal adenocarcinoma: A case report.

INTRODUCTION: Oral metastatic tumors due to malignancies at distant locations are very rare. There are only few cases in the literature reporting about mandible/cervical metastases. Sometimes the oral finding is the manifestation of the disease. PRESENTATION OF CASE: We report on a 50-year-old patient who was diagnosed with a pathological mandibular fracture and cervical lymph node metastases due to an adenocarcinoma of the lung without other peripheral metastases. Following the interdisciplinary tumor board decision, resection and ipsilateral neck dissection including a mandible reconstructi

Anterolateral congenital tibial bowing: case report.

BACKGROUND: The treatment of congenital curvatures (bowing) of the tibia still represents a challenging problem for all pediatric orthopedic surgeons because of its unpredictable course, especially if pseudoarthrosis occurs after a pathologic fracture of the tibia. CASE PRESENTATION: We describe the case of a child affected by an isolated curvature of his left leg. The congenital malformation was discovered at birth and no other pathological clinical finding was present. The first x-ray showed the presence of a congenital curvature of the tibia of the antero-lateral type. He was born in anoth

Biomechanical Analysis of Stability of Posterior Antiglide Plating in Osteoporotic Pronation Abduction Ankle Fracture Model With Posterior Tibial Fragment.

BACKGROUND: We performed a biomechanical comparison of 2 methods for operative stabilization of pronation-abduction stage III ankle fractures; group 1: Anterior-posterior lag screws fixing the posterior tibial fragment and lateral fibula plating (LSLFP) versus group 2: locked plate fixation of the posterior tibial fragment and posterior antiglide plate fixation of the fibula (LPFP). METHODS: Seven pairs of fresh-frozen osteoligamentous lower leg specimens (2 male, and 5 female donors) were used for the biomechanical testing. Bone mineral density (BMD) of each specimen was assessed by means of

Autogenous tibial strut grafts used in anterior spinal fusion for severe kyphosis and kyphoscoliosis.

STUDY DESIGN: A retrospective review was performed. OBJECTIVE: To evaluate the results of autogenous tibial strut grafts for anterior fusions in children with severe kyphosis and kyphoscoliosis regarding maintenance of correction, clinical outcome, graft fractures, and donor-site morbidity. SUMMARY AND BACKGROUND DATA: Anterior strut grafts harvested from the iliac crest, rib, and fibula often are used to treat severe kyphosis and kyphoscoliosis. Several studies in the literature have observed graft failures, loss of correction, or donor-site morbidity when these grafts have been used. Autog

Comparison of pediatric sports fracture outcomes based on provider type.

Objective: Pediatric primary care sports medicine physicians and pediatric sports medicine orthopedic surgeons, in conjunction with physician assistants (PAs), often manage patients with fractures. We sought to determine if pediatric patients with fractures seen by primary care sports medicine physicians had similar outcomes and satisfaction as those seen by orthopedic surgeons. Methods: We performed a retrospective chart review of four to 18-year-old patients who were treated by a sports medicine provider (primary care or orthopedic surgeon/PA) for a fracture of the radius, ulna, tibia, or fi

Biomechanical investigation of different internal fixations in medial opening-wedge high tibial osteotomy.

BACKGROUND: The medial opening-wedge osteotomy for treatment of varus gonarthritis has become very popular in recent years. Different implants for internal fixation after the osteotomy have been created. It is still unclear, which principle of internal fixation (angle stable or non-angle-stable plate with or without spacer) has the best biomechanical properties. The aim of this study was to determine the biomechanical properties of different fixation techniques in medial opening-wedge high tibial osteotomy in a porcine bone model. METHODS: A 10-mm high tibial opening-wedge osteotomy was perfo

Marin-Amat Syndrome: A Case Report of a Rare Facial Synkinesis Following Traumatic Facial Nerve Injury.

Marin-Amat syndrome is a rare form of facial synkinesis resulting from aberrant connections between the trigeminal and facial nerves. This condition manifests as involuntary eyelid closure upon voluntary jaw movement. It is distinct from the more common Marcus Gunn jaw-winking syndrome (MGJWS), which involves upper eyelid elevation during mastication. Here, we reported a case of a 64-year-old woman who developed this syndrome following combat-related maxillofacial trauma to the lateral mandible, which resulted in a left facial nerve injury. After her mandibular fractures were stabilized using

[Lengthening, axial correction and segmental reconstruction of the extremities using the distraction method].

Between 1988 and 1992 lengthenings axial, corrections and reconstructions have been performed by distraction on lower arms, upper and lower legs in 25 patients because of deviations in length due to either congenital or acquired (multiple cartilaginous exostoses, posttraumatic conditions, congenital pseudarthrosis deformations or defects (fibula-defect, amnion-band-complex, amputation). On the average elongation amounted to 4 cm for an average installation time of the fixator system of 22 weeks and a mean duration of 25 weeks for the adaptation to full strength. Problems arose in the form of s

Unusual Occurrence of Incarceration and Breakage of the Coiled Shaft of the Intramedullary Reamer during Closed Intramedullary Nailing of Tibia.

INTRODUCTION: Intramedullary (IM) nailing is a well-established method of treating closed tibia shaft fracture. We hereby describe an unusual incidence of the incarceration of the flexible IM reamer in the distal fracture fragment with breakage of the coiled shaft of the reamer in the proximal third of tibia during closed IM nailing. This case report highlights the usefulness of making a small window in the tibia to aid retrograde removal of incarcerated reamer. CASE PRESENTATION: A 26-year-old male patient had road traffic accident and sustained a close fracture of the right distal 3rd tibia

Axial load weightbearing radiography in determining lateral malleolus fracture stability: a cadaveric study.

BACKGROUND: Ankle fractures are the most common fracture treated by orthopaedic surgeons in the United States. Isolated lateral malleolus fractures are treated nonoperatively, while associated deltoid disruption injuries are unstable and usually treated by fixation of the fracture. Various stress radiographs and MRI have been used to determine deltoid competency and the subsequent need for operative stabilization of lateral malleolus fractures. To date, no standardization of stress radiography has been performed. The aim of this study was to evaluate the ability of weightbearing ankle X-rays t

Ankle fractures involving the posterior malleolus: patient characteristics and 7-year results in 100 cases.

INTRODUCTION: The presence of a posterior malleolar (PM) fragment has a negative prognostic impact in ankle fractures. The best treatment is still subject to debate. The aim of this study was to assess the medium-to-long-term clinical and functional outcome of ankle fractures with a PM fragment in a larger patient population. MATERIALS AND METHODS: One hundred patients (69 women, 31 men, average age 60 years) with ankle fractures including the PM were evaluated clinically and radiographically. Patients with Bartoníček-Rammelt type 3 and 4 fracture displayed a significant female preponderance.

Association of Specific Lower Extremity Injuries With Delayed Amputation.

INTRODUCTION: Despite medical interventions to preserve viability and functionality of injured limb(s) among combat-injured service members, delayed amputations may occur. The goal of this study was to determine whether specific lower extremity (LE) injuries were associated with delayed amputations. METHODS: The Expeditionary Medical Encounter Database was queried for combat-related LE injuries between 2003 and 2015. The Abbreviated Injury Scale (AIS) was used to categorize LE injuries by severity. Injury episodes with a maximum LE AIS of 1 or amputation on the day of injury were excluded. Th

Acute presentation of a proximal fibular stress fracture after a total knee arthroplasty.

A 67-year-old woman underwent a routine and uneventful elective total knee arthroplasty for osteoarthritis at our centre. No intraoperative nor immediate postoperative complications were noted clinically nor radiologically. At 5 weeks postoperative, she began to notice some new discomfort in her upper calf area, with no preceding history of trauma. A Doppler ultrasound scan ruled out a deep vein thrombus. Only on further re-imaging of her knee with X-rays and CT was there shown to be a fibular fracture of the proximal third with evidence of callus formation. The pain arising from her stress fr

Comparison of Open Anterior Syndesmotic Repair Augmented With Suture-Tape and Trans-syndesmotic Screw Fixation: A Biomechanical Study.

The treatment of syndesmotic injuries with ankle fractures is controversial. The purpose of this study was to compare the biomechanical properties of open anterior syndesmotic repair with those of screw fixation. Ten matched pairs of human cadaver specimens were subjected to open syndesmotic repair or screw fixation. Each specimen underwent initial intact physiologic loading, consisting of 10 cycles of external torsional loading with a peak torque of 7.5 Nm at 0.05 Hz. Injuries of the anterior inferior tibiofibular ligament, tibiofibular interosseous membrane, and deltoid ligament were applied
