Anatomical landmarks to the superficial and deep palmar arches.

BACKGROUND: Knowledge of the relationship of the palmar arches to anatomical landmarks would decrease iatrogenic injuries, facilitate treatment of vascular occlusive disease, and ease interpretation of abnormal arteriograms. The purpose of this study was to identify the location of the palmar arches in relation to surface and bony landmarks. METHODS: The palmar arches in 48 cadavers were identified through dissection. The most distal points of the palmar arches were measured in relation to Kaplan's cardinal line, the distal wrist crease, and the carpometacarpal joint of the ring finger. The d

The clinical implications of the oblique retinacular ligament.

The oblique retinacular ligament originates from the flexor tendon sheath, courses past the proximal interphalangeal joint, and merges with the lateral extensor tendon. There has been disagreement regarding the contribution of the oblique retinacular ligament to coordinated movements between the proximal and distal interphalangeal joints. Landsmeer postulated that it acts as a dynamic tenodesis that tightens with proximal interphalangeal joint extension, causing obligatory distal interphalangeal joint extension. However, studies have shown that the oblique retinacular ligament is variably pres

The concentric circles method to define and assess anterior subluxation in bony mallet finger.

We have described and validated the concentric circles method for assessing anterior subluxation in the bony mallet finger. We retrospectively analysed 114 radiographs from cases of bony mallet finger assessed by four hand surgeons and one radiologist using three approaches: subjective visual inspection, the concentric circles method and the malalignment method. The concentric circles method defines subluxation as a disruption of the normal concentric annulus between the middle phalangeal head and distal phalangeal articular surface on true lateral radiographs. It showed improved accuracy (94%

Management of proximal interphalangeal joint fractures and dislocations.

Proximal interphalangeal joint (PIP) injuries are among the most common in the hand and their severity is often underestimated. These injuries often lead to prolonged disability, pain, and stiffness. Appropriate treatment includes a thorough assessment, physical examination, and directed imaging. Such an approach should lead to a rational treatment plan that focuses on the rehabilitation of all damaged components, including osseous, articular, and soft tissue structures. This article reviews all elements in the management of PIP injuries and introduces an assessment method for PIP injuries bas

The digital extensor mechanism of the hand.

In summary, there is a complex interplay between the extrinsic and intrinsic components of the digital extensor mechanism. Numerous examples of dynamic anatomy and pathology of the extensor mechanism in common clinical settings have been described. Treatment principles have been outlined. This review should provide the orthopaedic surgeon with a framework for management of common hand problems.

Proximal toe phalanx transplantation for bony stabilization and lengthening of partially aplastic digits.

In congenital malformations the lengthening of partially aplastic digits, by on-top plasty of a bone graft or the bony stabilization of finger stumps consisting of soft tissues without skeletal elements, is extremely difficult. The usual bone graft, taken from the iliac crest or the fibula, will undergo resorption and will have disappeared a few months postoperatively. Experience has shown that the proximal toe phalanx will with-stand any resorption if it is taken whole, covered by its periosteum. Another advantage is the possibility of the construction of a new joint between the metatarsophal

The balanced finger: biomechanics of intrinsic and extrinsic systems and principles of reconstruction.

BACKGROUND: The balance of the finger is maintained through a complex interaction of intrinsic and extrinsic mechanisms that coordinate motion, stability and force transmission across the metacarpophalangeal and interphalangeal joints. Although flexor muscle mass exceeds that of the extensors, equilibrium is achieved through precisely calibrated tendon positioning, lever arms and passive restraints. This balance permits efficient motion with minimal energy expenditure and allows rapid transition between power and precision function.Physiological balance:Active balance is generated by coordinat

Clinical and radiological midterm outcome after treatment of pilonoidal fracture dislocations of the proximal interphalangeal joint with a parabolic dynamic external fixator.

INTRODUCTION: Several methods treating proximal interphalangeal joint (PIP) fracture dislocations have been established providing early joint mobilization. The aim of this study was to evaluate the clinical and radiological outcome of unstable fracture dislocations of the PIP treated with a parabolic dynamic external fixator consisting of two Kirschner (K)-wires. MATERIALS AND METHODS: Twenty-one patients who sustained a pilonoidal fracture of the PIP joint and were treated with a dynamic external fixator were evaluated retrospectively. The active range of motion, pain level, DASH score, Buck

Successful replantation of an avulsed middle finger.

In avulsion amputations of the digits, soft-tissue injuries are extensive and often require tendon, nerve, and vessel transfers or grafts. The functional results of such digital replantations are frequently less than ideal. Therefore, avulsion amputation of a single digit proximal to the insertion of the flexor digitorum superficialis has been a contraindication to replantation, because the anticipated poor functional result may interfere with overall hand function, and is not worth the sacrifice of a tendon, nerve, or vessel from another digit or transfer. The authors report a patient with av

[Injuries of the trapezo-metacarpal joint].

The trapezo-metacarpal joint has a complex structure with specific anatomical, physiological, radiological and clinical characteristics. Several anatomical descriptions of this joint are available, and we believe one should know it well in order to understand its radiological problems. For instance a wrong projection may lead to overlook a fracture, especially in the trapezium. Several mechanisms have been suggested to explain the lesions of the base of the first metacarpal and of the trapezium. The therapeutic consequences of these considerations are not very important. Regardless of the lesi

[Surgical treatment of malformations of the hand (author's transl)].

The frequency of malformations of the hand is estimated at 0.5 percent of live births. The course of early developmental processes makes early correction advisable. Knowledge of the special anatomy and mastery of the microsurgical operation techniques makes this possible. The main indications and methods that hve been tested are discussed.

Surgical treatment of postburn boutonniere deformity.

A new method of surgical treatment of postburn boutonniere deformity was used in 124 fingers of 66 patients. This method includes reconstruction of the lateral band's position of an extensor apparatus and anatomic continuity of a central band. Before tendon plasty, passive movements in a proximal interphalangeal joint are restored and contracture of others joints is released. If the soft tissue of the digital back is affected, tendon grafts are covered with a groin pedicled flap. With this method we could restore active movements in the proximal interphalangeal joint in 89.2 percent of the pat

Reconstruction of a Pediatric Distal Phalanx Amputation With Stacked Integra Dermal Substitute: A Case Report.

Finger amputations in children present unique challenges and require special considerations compared to their adult counterparts. Maximizing length and preserving fingertip bulk and sensation is essential for maintaining a functional digit. Synthetic dermal substitutes have been recently used for soft tissue coverage for pediatric syndactyly as well as burn injuries; however, the literature discussing pediatric amputation cases with soft tissue damage proximal to the bony level is limited. In this case, we report a two-year-old patient who developed dry gangrene of her right index finger after

Review of Acute Traumatic Closed Mallet Finger Injuries in Adults.

In adults, mallet finger is a traumatic zone I lesion of the extensor tendon with either tendon rupture or bony avulsion at the base of the distal phalanx. High-energy mechanisms of injury generally occur in young men, whereas lower energy mechanisms are observed in elderly women. The mechanism of injury is an axial load applied to a straight digit tip, which is then followed by passive extreme distal interphalangeal joint (DIPJ) hyperextension or hyperflexion. Mallet finger is diagnosed clinically, but an X-ray should always be performed. Tubiana's classification takes into account the size o

[Arthroscopy of the metacarpophalangeal joints].

With the advancements in arthroscopic technique, arthroscopy has become feasible in most human joints, even those as small as the finger joints. The metacarpophalangeal joints are very well suited for arthroscopy and arthroscopic therapy. Good results have been reported on arthroscopic synovectomy of the metacarpophalangeal joints in rheumatoid arthritis. Osteochondral lesions in degenerative arthritis, loose bodies and foreign bodies can well be treated. Arthroscopic arthrolysis for capsular contracture and treatment of post-traumatic lesions have been performed successfully. The arthroscopic…

[Camptodactyly].

Camptodactyly is a permanent, nontraumatic flexion of the proximal interphalangeal joint. The prevalence is around 1% and the little finger is most commonly affected. Two groups may be identified, depending on the age of onset: camptodactyly severe within 2-3 years or beginning in the early teens. Many anatomical abnormalities have been incriminated as the cause of camptodactyly: anomalous lumbrical muscle, short flexor digitorum superficialis, retractile subcutaneous tissue, anomalous extensor muscle. Splinting is always required, with dynamic and/or static splinting of the proximal interphal
