Atypical presentation of infiltrative thyroid dermopathy.

A 46-year-old woman with Graves' disease developed infiltrative dermopathy of the thenar eminences. We believe this to be the first reported case of infiltrative dermopathy affecting the thenar eminences, and question whether repetitive occupational injury may have been a contributing factor. There is little published evidence to guide the treatment of infiltrative dermopathy.

Iontophoretic delivery of terbinafine in onychomycosis: a preliminary study.

Background Onychomycosis is a common disease; topical treatment is usually poorly effective, while systemic treatment is more effective but may be associated with side-effects. Iontophoretic drug delivery may improve drug penetration through the nail and lead to better therapeutic results. Objectives To evaluate the efficacy, safety and tolerability of topical treatments with terbinafine HCl delivered with or without an iontophoretic patch in patients with onychomycosis of the toenails. Methods Patients enrolled into the study were divided randomly into two groups. Group A was treated with ter

Diagnosis of Trichophyton rubrum from onychomycotic nail samples using polymerase chain reaction and calcofluor white microscopy.

BACKGROUND: A high rate of false-negative dermatophyte detection is observed when the most common laboratory methods are used. These methods include microscopic observation of potassium hydroxide-digested nail clippings and culture methods using agar-based media supplemented with cycloheximide, chloramphenicol, and gentamicin to isolate dermatophytes. Microscopic detection methods that use calcofluor white staining or periodic acid-Schiff staining may also be substituted for and have previously been reported to be more sensitive than potassium hydroxide-digested nail clippings. METHODS: Trich

Fatal hepatitis after long-term pulse itraconazole treatment for onychomycosis.

OBJECTIVE: To report the occurrence of acute cytolytic hepatitis in a patient exposed to pulse itraconazole therapy for 24 weeks and provide a concise review of the literature on cases of itraconazole-induced hepatitis. CASE SUMMARY: A 61-year-old woman with no apparent risk factors for liver injury developed acute hepatitis one week after the final dose of a long-term course of pulse itraconazole therapy (200 mg orally twice daily, 1 wk on, 3 wk off, for 24 wk) for onychomycosis. Monitoring of liver enzymes was not performed during the treatment period. Serologic evaluations on presentation

Venous insufficiency in patients with toenail onychomycosis.

OBJECTIVE: Onychomycosis is a common fungal infection of the toenails and can originate secondary to vascular abnormalities. The aim of this study was to evaluate the relationship between onychomycosis and venous insufficiency. METHODS: Forty-two patients with onychomycosis and 39 healthy control subjects who had normal toenails were enrolled in the study. Doppler examinations were performed with a commercially available scanner and a 7.5-MHz linear probe. Major superficial and deep veins of the lower limb, including long and short saphenous, femoral, and popliteal veins, were examined. Venou

Onychomycosis--treatment, relapse and re-infection.

The treatment of onychomycosis has improved considerably following the introduction of the oral antifungals terbinafine and itraconazole. The pharmacokinetic characteristics of both these drugs has made short treatment times effective. Patients with a culture-proven dermatophyte infection of the toe-nails, receiving terbinafine 250 mg daily for 12 weeks, have a 70% chance of cure. The risk of relapse is probably less than 10%, and this may be prevented by a further course of terbinafine should the expected increasing length of unaffected nail growth stall or cultures become positive. Once cure

[The Calmette-Guérin bacillus (BCG). An agent to consider in pediatric infection].

Complications occurring a long time after BCG vaccination in healthy children have been occasionally referred, despite being rare. Osteitis seems to be the most frequent of those complications; therefore it must be considered in the differential diagnosis of that sort of lesions. The authors report a case of a 15 month old boy, previously healthy, who suffered an enlargement of the right foot for 3 weeks, unresponsive to antibiotics. The lesion revealed to be osteitis due to bacillus Calmette-Guérin, and abated after specific treatment.

A 12-week treatment for dermatophyte toe onychomycosis: terbinafine 250 mg/day vs. itraconazole 200 mg/day--a double-blind comparative trial.

Lamisil (terbinafine) 250 mg daily and itraconazole 200 mg daily were compared in the treatment of dermatophyte toe onychomycosis over 12 weeks in a double-blind randomized clinical trial. At the end of follow-up (week 48) treatment with Lamisil led to negative mycology in 73% of patients compared with 45.8% in the itraconazole group (P < 0.0001). Globally the clinical symptoms of the target nail improved, a response which was in favour of Lamisil (P = 0.001). The percentages of patients who were clinically totally cured or who presented with only minimal symptoms were 76.3% for the Lamisil-tr

Importance of Preexisting Physical Factors in the Development of Dermatological and Muscular Lesions During Hiking.

Hiking has become very popular due to the physical and mental health benefits it provides. Skin lesions may occur with hiking and various factors contribute to their development. Although some lesions may appear to have little importance, something as simple as chaffing can cause serious ulcers in people with at-risk feet. Few studies have analyzed preexisting physical characteristics in hiking and addressed the development of lesions in hikers. This observational and longitudinal study examines the development of foot lesions during hiking, taking into account the influence of existing skin d

Scopulariopsis brevicaulis as the cause of dermatomycosis.

Scopulariopsis (S.) brevicaulis (Saccardo) Bainier 1907 is a ubiquitous fungus frequently isolated as a saprophyte from various layers of the soil, wood, straw, paper, food, and occasionally in animals and humans. This nondermatophyte filamentous fungus is multiresistant and is frequently associated with onychomycosis in humans. In the last two decades, the number of reports on its pathogenic role in different localized and disseminated infections has been on an increase. Identification was done by native KOH microscopy and culture on the Mycobios selective agar (Biolife). From January 1, 2002

Subcutaneous granuloma annulare: radiologic appearance.

OBJECTIVE: Granuloma annulare is an uncommon benign inflammatory dermatosis characterized by the formation of dermal papules with a tendency to form rings. There are several clinically distinct forms. The subcutaneous form is the most frequently encountered by radiologists, with the lesion presenting as a superficial mass. There are only a few scattered reports of the imaging appearance of this entity in the literature. We report the radiologic appearance of five cases of subcutaneous granuloma annulare. DESIGN AND PATIENTS: The radiologic images of five patients (three male, two female) with

Dermoscopic features in fungal melanonychia.

BACKGROUND: Data on the dermoscopic features of fungal melanonychia are limited. AIM: To identify the dermoscopic features of fungal melanonychia. METHODS: We reviewed patient files, clinical history and dermoscopic images of all cases with a diagnosis of fungal melanonychia seen at our dermoscopy unit within the past year. RESULTS: In total, 14 cases with 20 involved nails were reviewed. The most common type of melanonychia was melanonychia striata (7/20). Multicoloured pigmentation was observed in 19 of the nails. The main dermoscopic pattern was homogeneous pigmentation; however, black p
