We performed a fresh cadaver dissection study of the superficial venous system (cephalic vein and its branches) and the deep venous system (venae comitantes) of the radial forearm to assess the suitability of each system for venous anastomosis during free tissue transfer. The advantages of the ipsilateral forearm flap in emergency post-traumatic hand reconstruction are now well-established. A prerequisite for such a transfer has always been the presence of at least one intact palmar arch. We report the successful transfer of two reverse forearm flaps despite disruption of both palmar arches. In both cases the flap was raised on the vascular axis which had ceased to contribute significantly to the hand circulation. The anatomical basis of this transfer is discussed.

During 1980 and 1981, 1914 fractures of the distal forearm occurred in Malmö, Sweden. The age-specific incidence peaked in childhood and there was a large increase in women after the age of 50; in men there was a smaller increase after the age of 70. In comparison with the Alffram & Bauer (1962) study of the same population in 1953-57, the age-specific incidence in elderly women and men has almost doubled. There was almost the same incidence of shaft fracture of the ulna or radius as 25 years ago.

Two unrelated infants with delayed development and suspected abnormalities of the upper limbs were found to have the Coffin-Lowry syndrome. Both had marked fullness of the forearms in the presence of normal skeletal structures which resulted from increased subcutaneous fat. Although initially misleading, the forearm changes may serve as an early diagnostic clue in this disorder.

We have measured forearm oxygen consumption and blood flow changes during two wakeful rest behaviors. We have observed acute reduction of forearm respiration (28%) during an acute stylized rest state (TM) and a nonsignificant small decline (11%) during unstylized ordinary eyes-closed rest. These changes were not associated with significant change of forearm blood flow or glycolytic metabolism.
