Esophagoaortic perforation by foreign body (coin) causing sudden death in a 3-year-old child.

We report an extremely unusual consequence to foreign body ingestion in a case of a 3-year-old boy who died suddenly and at autopsy was found to have an esophagoaortic fistula. This fistula was caused by a coin which lodged posteriorly and eroded through the esophagus into the aorta. Serious complications following foreign body ingestion are rare and include stricture formation, intramural abscess, and the formation of fistula tracts. This case illustrates the potentially unpredictable behavior of impacted foreign bodies. The child's parents were initially suspected of child abuse based on the

An unusual complication of T-tubes.

The morbidity of long-term ventilation tubes (Goode T-tubes) is often discussed with reference to otorrhoea, tympanosclerosis and long-term perforation. We report three cases of the T-tube slipping into the middle-ear cleft. In two of the three patients this was asymptomatic and the T-tubes were left in situ. In the third patient this complication was documented on three occasions when the T-tube was removed and reinserted for recurrent effusions. We are unaware of this complication being previously reported in the literature.

Femoral artery and nerve compression by bulk allograft used for acetabular reconstruction. An unreported complication.

Absent pulses in the ipsilateral limb and a femoral nerve palsy occurred after revision total hip arthroplasty. Emergent arteriogram revealed an occlusion of the femoral artery inferior to the inguinal ligament. The femoral artery was found to be compressed by the bulk allograft bone used for reconstruction of an anterior segmental acetabular defect. No laceration or thrombosis of the artery was present. A high-speed burr was used to remove the anterior portion of the offending allograft. Pulses were immediately restored and the patient had no long-term vascular sequelae. The femoral nerve pal

Paradoxical bullet embolism: case report and literature review.

Bullet embolization through the arterial or venous systems, although unusual, often causes diagnostic confusion. Paradoxical emboli are even more unusual. We present a case of a paradoxical bullet embolism from the left external iliac vein to the left common iliac artery via a patent foramen ovale.

Secure placement of peritoneal dialysis catheters using a laparoscopic technique.

Nineteen patients underwent laparoscopic placement of a peritoneal dialysis catheter with suture fixation into the pelvis. Operating time ranged from 20 to 60 min (median, 33), and the median postoperative stay for patients admitted solely for catheter placement was 1 day. No significant morbidity was associated with this procedure, and all but one catheter functioned well over the long term. Suture fixation of laparoscopically placed peritoneal dialysis catheters is straightforward, and may achieve better catheter function.

Retroperitoneal caval filter as a source of abdominal pain.

A patient with a Mobin-Uddin vena caval filter placed 10 years earlier was referred with abdominal pain, nausea, anorexia and weight loss. Diagnostic studies confirmed that the filter was in an extracaval position in the retroperitoneum, with the filter struts impinging on the right kidney, duodenum and colon. Following surgical removal of the filter the patient is now symptom-free with good appetite and weight gain.

Failure of the intracaval filter of Günther to prevent recurrence of pulmonary embolism--report of two cases.

Two out of nine patients in which inferior vena cava interruption was performed with a Günther filter developed a recurrent pulmonary embolism. In both cases, the filter had moved down and the anchoring legs had perforated the wall of the vena cava. The source of the pulmonary embolism was a clotted basket filter. Anticoagulation was given for two weeks in one patient and six months in the other after insertion of the filter, but it had been stopped before the thrombotic event leading to the recurrent pulmonary embolism. The formation of the thrombi had probably been caused by the migration of

[Lung embolism caused by vaseline following insertion of a venous catheter].

Pulmonary embolism usually is due to natural causes, most often to obstruction of pulmonary arteries by blood clots breaking loose from venous thrombi forming in the leg or pelvis. Clots of other cellular material such as tumor fragments or fatty tissue (this especially in case of severe soft tissue and skeletal trauma), but also foreign material can give rise to this- often times lethal-condition. This is a report of foreign-body pulmonary embolism with obstruction of mainly the right lower lobe pulmonary arteries due to white vaseline embolism, probably responsible for the patient's prematur

Prolonged middle ear ventilation: the right tube in the right place.

Premature extrusion of ventilating tubes continues to plague both doctor and patient. Migrating epithelium on the lateral surface of the drum has been identified as the principal cause of tube migration and extrusion. This information dictates insertion of a tube in an area where the migrating forces tend to keep the tube in place instead of encouraging rejection. When optimally placed, tubes specifically designed for long-term ventilation will function four times longer than randomly placed conventional tubes. The long-term ventilating tube in use since 1965 has been further refined. A notch

Deflated intragastric balloon-induced small bowel obstruction.

We describe the rare case of a 49-year-old man who had an air-filled intragastric balloon placed for weight control 11 months ago which had been lost of sight for follow-up and presented six months after the last radiologic control with acute small bowel obstruction. Computed tomography confirmed the migration of the deflated balloon in the small bowel. It was impacted in the ileum so an enterostomy-suture was performed for removal. Intragastric balloons are temporarily used to obtain weight reduction in the super-obese prior to bariatric surgery and in patients who are not fit for surgery. Th

Thoracotomy for Extracting Grass Inflorescence Aspiration: a Case Report.

Following aspiration of grass inflorescence, it often migrates to the peripheral part of the airway due to its unique shape. It often causes complications like chronic pneumonia, recurrent asthmatic attacks, lung abscess, recurrent hemoptysis, and bronchiectasis that may necessitate surgical intervention. In this case, we have reported a 13-year-old boy with grass inflorescence aspiration. The bronchoscopic procedure failed to remove the foreign body, so he underwent thoracotomy.

From contraception to calculus: copper-T migration resulting in vesical calculus formation.

A woman in her 40s presented with persistent lower urinary tract symptoms (LUTS) over 6 months, including suprapubic pain and dysuria. Initial investigations were inconclusive until non-contrast CT revealed a large bladder stone encapsulating an intrauterine contraceptive device (Copper-T). She underwent successful perurethral cystolithotripsy using Holmium:YAG laser, resulting in complete stone and device removal. This case underlines the importance of suspecting migrated intrauterine devices in unexplained LUTS and highlights minimally invasive endourological techniques as effective manageme

[Migration of an intrauterine contraceptive device outside of the uterine cavity - a case report].

Intrauterine devices (IUDs) are a safe and common form of contraception. Uterine rupture and migration of the IUD is a rare complication of insertion but can be serious and cause damage to adjacent organs. We present a case report of a 43 year old woman with chronic abdominal pain who was diagnosed with an IUD in the wall of the sigmoid colon. That IUD had been inserted in the uterine cavity 22 years earlier.

The wandering Mirena: laparoscopic retrieval.

Levonorgestrel-containing intrauterine contraceptive devices, marketed as Mirena (Bayer HealthCare Pharmaceuticals, Inc. Australia) are widely used in contemporary gynecology, primarily as an effective method for contraception and for control of menstrual disorders like menorrhagia and dysmenorrhea. In this article, the authors report 2 cases of Mirena migration following intrauterine insertion by general practitioners (family physicians). In the first case, the contraceptive device had moved to the patient's right iliac fossa just anterior to the cecum and, in the second, within the peritonea

The Removal of Intravascular Foreign Bodies by Intervention in Pediatrics.

INTRODUCTION: The use of intravascular catheters has become increasingly widespread in children, due to their use in diagnostic procedures (such as coronary, intracardiac, cerebral, and renal angiography, as well as pressure monitoring) and for therapeutic purposes (including angioplasties, valvuloplasties, congenital defect closure, chemotherapy, among other uses). However, they are not free from complications, which may include catheter fracture and migration within the cardiovascular system, potentially leading to vascular or cavity perforation, arrhythmias, and even death. OBJECTIVE: To d
