Conservative treatment of 3- and 4-part proximal humeral fractures: Can poor outcomes be predicted?

PURPOSE: 3- and 4-part proximal humeral fractures are frequently treated conservatively. This study aims to combine radiographic, and patient reported outcome data to identify factors leading to poor outcomes following conservative treatment. METHODS: A retrospective local database analysis identified 3- and 4-part fractures. Radiographic and functional outcomes including Oxford Shoulder Score (OSS), QuickDash (QD), Subjective Shoulder Score (SSV) and VAS pain scores were collected. RESULTS: 104 patients were included at mean follow-up of 55 months. Analysis highlighted significant OSS diffe

[Hip prostheses as a treatment for acetabular fractures in the elderly].

INTRODUCTION: low-energy acetabular fractures in older adults have significantly increased, with an incidence up to 2.4 times higher over the last three decades. While osteosynthesis is the standard treatment in young patients, its suitability for older adults is debated due to higher risks of complications and reinterventions. These risks are exacerbated by osteoporosis, comminution, and articular surface damage, such as femoral head impaction and preexisting conditions like coxarthrosis. The literature suggests that total hip arthroplasty (THA) may be more appropriate in cases of severe disp

Iatrogenic Brachial Artery Injury During Long Proximal Humeral Interlocking System (PHILOS) Plating.

Iatrogenic vascular injuries are rare but potentially serious complications that can occur during orthopedic procedures involving the proximal humerus. We present a case report of a patient who sustained a brachial artery injury during a long Proximal Humeral Interlocking System (PHILOS) plating procedure for a proximal humeral fracture. A 62-year-old female patient with a left humerus proximal 1/3 shaft fracture underwent open reduction and internal fixation with a long PHILOS plate. During the procedure, difficulty was encountered in achieving adequate plate positioning due to osteoporotic b

Biomechanical analysis of a novel Y-plate designed for the treatment of extraarticular distal humerus fractures.

Adult distal humerus fractures are infrequent, yet they account for one-third of all humerus fractures. For the treatment of comminuted and osteoporotic fractures, locking plates are claimed to be biomechanically superior to alternative internal fixing techniques. Treatment remains difficult despite recent advancements and the use of locking plates due to frequent comminution, low bone quality, and limited healing ability in osteoporotic bone. An optimal design of the newly constructed plate and the control model were selected. The biomechanical characteristics of non-osteoporotic and osteopor

[Central-medullary nailing of proximal humerus fractures using T2 nails. A retrospective study of 38 cases].

The intramedullary nailing of humerus has emerged as one of the treatments of reference for proximal fractures. We have reviewed 38 patients aged 64.2 on average with a mean follow of 18 months. The functional scores used were the Constant and Murley's score as well as the DASH self-administered quality of life questionnaire. Radiological criteria have been analyzed, namely the cephalic angle noted alphaF and the presence of any osteolysis of the major tuberosity. Patients were grouped by type of fracture, according to Neer's classification, with nine cases in Neer 2 group, 19 in Neer 3 group,

Treatment of select open calcaneus fractures with reduction and internal fixation: an intermediate-term review.

BACKGROUND: Open calcaneal fractures are devastating, high-energy injuries frequently occurring in multiply-injured patients. Unsatisfactory outcomes with significant complications rates are frequently reported. Our purpose was to assess outcomes of delayed ORIF for open calcaneal fractures with medial hindfoot wounds and limited posterior facet comminution. MATERIALS AND METHODS: Following acute management of the open fractures, seven patients were determined to have stable medial hindfoot wounds and posterior facet comminution of three or fewer fracture fragments. The average age was 32.5 y

Internal fixation of supracondylar femoral fractures: comparative biomechanical performance of the 95-degree blade plate and two retrograde nails.

OBJECTIVE: The biomechanical stability of supracondylar femoral fractures fixed with a condylar blade plate (plate), a Green Seligson Henry nail (GSHN), or a new retrograde unreamed supracondylar femoral nail (new nail) based on the AO unreamed femoral nail were compared. DESIGN: A standardized simulated comminuted supracondylar femoral fracture (segmental defect) in fresh frozen paired cadaveric femora was stabilized with one of the implants. The interfragmentary fracture site stiffness in three directions and axial strength of the fixator-bone construct were compared (pairwise). RESULTS: T

Open or closed pinning for distal humerus fractures in children?

INTRODUCTION: In children, the choice between percutaneous pinning (PP) and open pinning fixation (OPF) for the surgical treatment of fractures of the distal humerus remains controversial, especially the PP method for internal humeral condylar (IHC) fractures. PATIENTS AND METHODS: Eighty fractures of the distal humerus in children were treated surgically in our hospital over a ten year period. 47% (n = 38) were supracondylar (SC), 20% (n = 16) comminuted (COM), 18% (n = 14) internal humeral condylar (IHC), and 15% (n = 12) lateral humeral condylar (LHC). We used PP, OPF and three times osteo

A minimally invasive technique for the reduction of calcaneal fractures using the Endobutton®.

Calcaneal fractures are very common injuries, and closed reduction-percutaneous pinning, open reduction-internal fixation, and primary arthrodesis are the procedures used in the surgical treatment of these injuries. The aim of surgical treatment is to restore the normal biomechanics of the Achilles tendon and rearfoot by anatomic reduction of the articular surfaces. Minimization of the soft tissue complications commonly associated with the open treatment of calcaneal fractures can be achieved using indirect closed reduction and Kirschner wire or screw-assisted minimally invasive percutaneous o

Volar plating for intra-articular fracture of the base of the proximal phalanx.

Comminuted fractures involving the articular surface of the base of the proximal phalanx are relatively rare and pose a challenging problem for hand surgeons because of the difficulty in achieving an accurate reduction and secure fixation of the articular surface. These fractures usually comprise a volar base fracture associated with a central depression of the articular surface. We describe a technique for open reduction and plate fixation of intra-articular fractures of the base of the proximal phalanx through a volar A1 pulley approach. Compared with the dorsal approach, this technique offe

Radial head arthroplasty.

Radial head fractures are a particularly common yet challenging problem facing orthopedic surgeons today. Over the years multiple treatment modalities have been used including conservative management, open reduction and internal fixation, head excision, and radial head arthroplasty. The clinical outcome studies of metallic radial head arthroplasty systems indicate that head replacement is a reasonable option to offer patients with comminuted radial head fractures and complex elbow trauma. Overall the results for this procedure are quite encouraging and appear durable given the severity of the

Intramedullary osteosynthesis. 1. Küntscher nailing in the femur.

Based on more than 20 years experience, the authors support the effectiveness of intramedullary nailing in the treatment of diaphyseal fractures of the femur. They review the history and advantages of this method and discuss the biomechanical principles on which it is based. Recent advances in technique have made it possible to secure proximal and distal anchorage in cancellous bone. This allows the method to be extended to more proximal and distal fractures than in the traditional method which is applicable only to the middle third. It also facilitates progressive axial compression, early loa

Treatment of femoral shaft fractures with the Brooker-Wills interlocking intramedullary nail.

Fifty femur fractures were treated with the Brooker-Wills intramedullary locking nail. The indications included subtrochanteric, subisthmal, segmental or comminuted acute fractures or nonunions, and intramedullary shortening procedures. Eighteen technical problems in 13 (26%) patients were encountered during insertion of the nail. The incidence of technical problems was high early in the series and in procedures done with the patient in the supine position. Adverse clinical results from technical errors occurred in two patients. Most technical errors seem avoidable with careful technique. The

Clamshell Corticotomy: A Technique to Address Challenges of Narrow Medullary Canal during Intramedullary Nailing of Tibial Shaft Fracture Fixation.

Intramedullary nailing for diaphyseal fractures is a gold standard procedure, that is, often associated with some intraoperative challenges. Fixing an intramedullary nail in a narrow canal in an unplanned surgery is a nightmare. Forceful implantation leads to iatrogenic fracture or comminution at the fracture site. Forceful reaming in such scenarios is also associated with thermal osteonecrosis and at times incarceration of flexible reamer head. It is not uncommon to face a challenge intraoperatively with regards to a narrow medullary canal, however, well we plan preoperatively. Clamshell oste

Hemiarthroplasty in elderly, debilitated patients with an unstable femoral fracture in the trochanteric region.

The purpose of this study was to evaluate the results of patients with an unstable fracture in the trochanteric region who were treated by hemiarthroplasty. During a 10-year period, 154 of the 308 patients with a trochanteric fracture treated in our hospital had an unstable fracture. In patients with severe comminution and osteoporosis an endoprosthesis was inserted: 5 patients with a subtrochanteric and 17 with a pertrochanteric fracture. Ten patients suffered from central nervous system diseases, and in 10 patients cardiovascular or pulmonary disorders were diagnosed. Pre- and postoperative

Open Reduction and Internal Fixation of a Partial Articular (OTA-43B) Pilon Fracture Through a Direct Anterior Approach.

We present the management of an OTA-43B Pilon fracture with a depressed plafond fragment, medial comminution, and displacement of the Chaput fragment. The patient was initially managed with external fixation to gain initial reduction of the pilon fracture and to allow the soft tissues to heal. As demonstrated, the extensile anterior approach can be modified and used to see the anteromedial and anterolateral tibial plafond so an anatomic reduction could be obtained. Also detailed is the reduction and fixation strategy and soft tissue closure, all of which plays a critical role in the outcome.

Technical Tips for Reduction and Stable Fixation of Proximal Humerus Fractures.

The surgical treatment of proximal humerus fractures, though common, is associated with surprisingly high complication rates upward of 34%. Obtaining a reduction and placing a stable fixation can be challenging because many fractures treated surgically are comminuted and occur in osteoporotic bone. Nevertheless, innovations in technique and implant design are mitigating some failures. These innovations include the use of fibular strut allograft and other fixation adjuncts, the correct placement of calcar screws and other locking fixation, and having a systematic approach to reduction and intra

[Spiral computed tomography in the diagnosis of ankle and foot injuries].

The data on 187 patients with traumatic lesions of the ankle joint and foot were used to analyze the capacities of computed tomography (CT) in the diagnosis of this pathology. The efficiency of CT versus X-ray study was evaluated. Specific guidelines are given to make spital CT. CT is shown to be of the most informative value in the diagnosis of lesions of the tibial plateau, undisplaced fractures of the internal malleolus, small marginal comminuted fractures, and lesions of the distal tibiofibular syndesmosis. There is evidence for that CT plays a dominant role in the diagnosis of lesions of

Fractures of the distal radius treated by internal fixation and early function. A prospective study of 73 consecutive patients.

Stable fixation of fractures of the distal radius can be achieved by using two 2.0 mm titanium plates placed on the radial and intermediate columns angled 50 degrees to 70 degrees apart. We describe our results with this method in a prospective series of 74 fractures (58 severely comminuted) in 73 consecutive patients. Early postoperative mobilisation was possible in all except four wrists. All of the 73 patients, except two with other injuries, returned to work and daily activities with no limitations. The anatomical results were excellent or good in 72 patients and fair in one. Our discussio
