Consensus Decision-Making for the Management of Antiplatelet Therapy before Non-Cardiac Surgery in Patients Who Underwent Percutaneous Coronary Intervention With Second-Generation Drug-Eluting Stents: A Cohort Study.

Background Although antiplatelet therapy (APT) has been recommended to balance ischemic-bleeding risks, it has been left to an individualized decision-making based on physicians' perspectives before non-cardiac surgery. The study aimed to assess the advantages of a consensus among physicians, surgeons, and anesthesiologists on continuation and regimen of preoperative APT in patients with coronary drug-eluting stents. Methods and Results A total of 3582 adult patients undergoing non-cardiac surgery after percutaneous coronary intervention with second-generation stents was retrospectively includ

Bilateral Superior Laryngeal Nerve Block to Attenuate the Pressor Response of Laryngoscopy and Intubation in Adult Patients Undergoing Elective Surgeries Under General Endotracheal Anesthesia: A Randomized Controlled Double-blind Study.

BACKGROUND: Laryngoscopy and endotracheal intubation are essential for airway management but provoke a transient pressor response characterized by hypertension and tachycardia. This reflex sympathetic stimulation can pose significant risks to patients with cardiovascular comorbidities. OBJECTIVES: To evaluate the efficacy of ultrasound-guided bilateral superior laryngeal nerve block (SLNB) using 2% lidocaine in attenuating hemodynamic responses and reducing postoperative airway complications during elective surgeries under general anesthesia. METHODOLOGY: This randomized double-blind control

[Development process, current status, and future trends of acute care surgery].

Emergency surgery has always been an important component of general surgery, with major diseases including acute abdomen and trauma. With the differentiation and development of general surgery sub specialties, department of emergency surgery has begun to emerge, but its development faces various challenges, such as insufficient medical staff, inadequate treatment capabilities, and poor treatment outcomes. To change this situation, about 20 years ago, a new treatment model began to emerge in the United States, establishing a department of acute care surgery that integrates trauma, emergency gen…

Shaping anesthetic techniques to reduce post-operative delirium (SHARP) study: a protocol for a prospective pragmatic randomized controlled trial to evaluate spinal anesthesia with targeted sedation compared with general anesthesia in older adults undergoing lumbar spine fusion surgery.

BACKGROUND: Postoperative delirium is common in older adults, especially in those patients undergoing spine surgery, in whom it is estimated to occur in > 30% of patients. Although previously thought to be transient, it is now recognized that delirium is associated with both short- and long-term complications. Optimizing the depth of anesthesia may represent a modifiable strategy for delirium prevention. However, previous studies have generally not focused on reducing the depth of anesthesia beyond levels consistent with general anesthesia. Additionally, the results of prior studies have been

[Outcomes of complex therapy in female patients after breast-saving surgery for breast carcinoma at the 1st Surgical Department 1st Faculty of Medicine, Charles University and General University Hospital in Prague during a ten-year follow up period].

INTRODUCTION: Good longterm outcomes of complex therapy in operable breast cancer can be achieved mainly due to early diagnosis of the tumor, adequate radicality of surgery and adequate oncotherapy. The following outcome criteria are considered significant: long-term survival rate in complete remission, a number of locoregional recurrences and a number of reoperations or mastectomies required by results of final histological examination, patient satisfaction with a cosmetic result of their breast- saving surgery. Comparison of complex treatment results collected from patients who underwent bre
