Association of four genetic loci with uric acid levels and reduced renal function: the J-SHIPP Suita study.

BACKGROUND: Recent genome-wide association studies have identified several genetic variants as susceptibility loci for serum uric acid (UA) … ± 1.4, 5.1 ± 1.4 mg/dl; p = 2.0 × 10(-5)) genotypes. During 9.4 years of follow-up, 87 new cases of hyperuricemia were diagnosed. Multiple logistic regression analysis identified the accumulation of risk alleles as a significant determinant of future development of hyperuricemia (OR = 7.94; 95% CI: 1.97-53.6). In contrast, subjects with nonsense mutation predom …

Management of ulcerated finger pad tophi.

Finger pad tophi are an unusual but not very rare presentation of uncontrolled gout. Unlike tophaceous lesions of other locations, finger pad tophi are often difficult to diagnose at presentation due to resemblance to a variety of nodular lesions, coexistence with other connective tissue disorders and sometimes occurrence in asymptomatic hyperuricemia. They also pose a therapeutic dilemma due to a lack of clear treatment guidelines. Several case reports of finger pad tophi have been published before, focusing mainly on diagnosis and medical management. We discuss here a case of thumb pad tophi…

[Gouty arthritis].

Gouty arthritis is the most common form of arthritis in men. As a consequence of persisting hyperuricemia, uric acid crystals are deposited in the intra-articular and periarticular spaces and activate the innate immune system. The clinically impressive abrupt onset monoarticular arthritis in the lower extremities is highly suggestive of a gout attack. Arthrosonography can be used for early detection of crystal deposition on joint cartilage. In synovial fluid the detection of uric acid crystals in polarization microscopy is proof of gout even without the detection of intracellular uric acid cry…

A small disturbance, but a serious disease: the possible mechanism of D52H-mutant of human PRS1 that causes gout.

Phosphoribosyl pyrophosphate synthetase isoform 1 (PRS1) has an essential role in the de novo and salvage synthesis of human purine and pyrimidine nucleotides. The dysfunction of PRS1 will dramatically influence nucleotides' concentration in patient's body and lead to different kinds of disorders (such as hyperuricemia, gout and deafness). The D52H missense mutation of PRS1 will lead to a conspicuous phosphoribosyl pyrophosphate content elevation in the erythrocyte of patients and finally induce hyperuricemia and serious gout. In this study, the enzyme activity analysis indicated that D52H-mut…

Genetic disorders resulting in hyper- or hypouricemia.

Serum uric acid concentrations are governed by the balance of urate production and excretion. Besides well-known secondary causes of hyperuricemia, such as myeloproliferative diseases, decreased renal function, and excessive dietary purine intake, there are a number of genetic disorders that result in hyper- or hypouricemia. Renal impairment in these disorders may be associated with the development of chronic kidney disease, acute kidney injury, or urate nephrolithiasis. These conditions are frequently misdiagnosed, not because the diagnosis is complicated and difficult to ascertain, but rathe…

A novel heterozygous missense mutation in the UMOD gene responsible for Familial Juvenile Hyperuricemic Nephropathy.

BACKGROUND: Familial Juvenile Hyperuricemic Nephropathy is an autosomal dominant nephropathy, characterized by decreased urate excretion and progressive interstitial nephritis. Mutations in the uromodulin coding UMOD gene have been found responsible for the disease in some families. CASE PRESENTATION: We here describe a novel heterozygous p.K307T mutation in an affected female with hyperuricemia, renal cysts and renal failure. The proband's only son is also affected and the mutation was found to segregate with the disease. CONCLUSIONS: This mutation is the fourth reported in exon 5. Initial …

Renal urate handling: clinical relevance of recent advances.

Urate is the major inert end product of purine degradation in higher primates in contrast to most other mammals because of the genetic silencing of hepatic oxidative enzyme uricase. The kidney plays a dominant role in urate elimination. The kidney excretes 70% of the daily urate production. Therefore, it is important to understand renal urate handling mechanism because the under excretion of urate has been implicated in the development of hyperuricemia that leads to gout. The urate transport systems exist in the proximal tubule but they are complicated because of their bidirectional transport …

[Recurrent thromboses of the Björk-Shiley valve in a patient with gout].

A 56-year-old male, who had mitral stenosis and gout, underwent mitral valve replacement with the Björk-Shiley valve prosthesis. Four years after the surgery, the patient suddenly became dyspneic on exercise due to malfunctioning valve. Thrombosis was found on the prosthesis, which was replaced again by Björk-Shiley valve. Thrombotest value had been satisfactorily controlled at 12.8 +/- 4.2% with warfarin since the first surgery. Dilazep chlorate also had been prescribed. Hyperuricemia had been controlled with allopurinol. In spite of these combined medical therapy, thrombosed valve recurred. …

Expression, localization and metabolic function of "resurrected" human urate oxidase in human hepatocytes.

A high serum uric acid (SUA) concentration is associated with hyperuricemia (HUA) and gout. In order to obtain long-acting therapeutic effect, correction of purine metabolism at genetic level is advantageous. For this purpose, we expressed three "human-like" urate oxidases in human hepatocytes (HL-7702) by lentivirus-mediated transduction. Enzymatic assay revealed that the recombinant urate oxidases expressed in HL-7702 cells were functionally active. Electron microscopy study showed that the recombinant enzymes were localized to peroxisome and formed distinct crystalloid core structures as in…

Impact of GAstV-2 infection on intestinal microbiota and renal gene transcriptome in goslings.

Goose astrovirus-2 (GAstV-2) is the major etiological agent responsible for hyperuricemia and visceral gout in goslings, which causes substantial economic losses to the goose-raising industry. However, the complete pathogenic mechanisms underlying GAstV-2-induced hyperuricemia have not been fully elucidated. In this study, 1-day-old healthy Yangzhou goslings were orally inoculated with GAstV-2 to establish a viral infection model. The effects of GAstV-2 infection on growth performance, serum uric acid levels, cecal microbiota structure, and renal gene expression of goslings were systematically…

Continuous hemodiafiltration during chemotherapy for acute kidney injury in spontaneous tumor lysis syndrome due to high-grade B-cell lymphoma: a case report.

Tumor lysis syndrome (TLS) is a life-threatening oncological emergency, characterized by hyperuricemia, electrolyte abnormality, and acute kidney injury (AKI). TLS usually occurs during chemotherapy, but rarely occurs before chemotherapy as spontaneous TLS (STLS). We describe a 54-year-old man who showed general fatigue. Laboratory tests showed myeloblasts in peripheral blood, marked elevations of serum uric acid (67.9 mg/dL), hyperphosphatemia, and severe renal dysfunction. Bone marrow biopsy findings were compatible with high-grade B-cell lymphoma (HGBCL). The patient was diagnosed with AKI …

[Screening of novel uric acid-lowering probiotics and evaluation of their probiotic properties].

To address the growing health challenge of hyperuricemia while circumventing or compensating for the limitations of existing clinical treatment options, through a combination of large-scale screening and laboratory-based adaptive evolution, we identified a Lactobacillus acidophilus strain D801 exhibiting robust capacity for uric acid degradation. We then systematically evaluated the probiotic properties of the strain by assessing the in vitro uric acid degradation capacity, gastrointestinal tolerance, auto-aggregation ability, acid and bile salt tolerance, and antibiotic susceptibility. Strain…

Uric acid promotes aortic valve calcification via mediating valve interstitial cell osteogenic differentiation and endothelial dysfunction.

Aortic valve calcification is a lethal valvular heart disease lacking effective drug therapy. However, whether uric acid is involved in the development of aortic valve calcification is unclear. Two-sample Mendelian randomization (MR) analyses confirmed the causal relationship between uric acid and valvular heart disease. Uric acid levels were assessed in aortic valve tissue from patients with/without aortic valve calcification. To investigate the impact of hyperuricemia on aortic valve calcification, apolipoprotein E knockout (ApoE-/-) mice fed a high-fat diet (HFD) were also given an adenine …
