Chromosomal Abnormalities Affect the Surgical Outcome in Infants with Hypoplastic Left Heart Syndrome: A Large Cohort Analysis.

Patients with hypoplastic left heart syndrome (HLHS) can have associated genetic abnormalities. This study evaluated the incidence of genetic abnormalities among infants with HLHS and the short-term outcomes of this population during the first hospitalization. This is a retrospective analysis of the multi-center Pediatric Heath Information System database of infants with HLHS who underwent Stage I Norwood, Hybrid, or heart transplant during their first hospitalization from 2004 through 2013. We compared clinical data between infants with and without genetic abnormality, among the three most co

[No correlation between rates of caesarean section and perinatal mortality in Iceland].

INTRODUCTION: Caesarean section rates have increased over the past decades without a concomitant decrease in perinatal mortality. In Iceland the same trend has been seen while at the same time perinatal mortality rate has remained low. Most caesarean sections are done at term. Crude perinatal mortality rates give limited information about whether the increase in section rates leads to a lower perinatal death rate among term non-malformed singleton infants. The relation between caesarean section and perinatal mortality rates in singleton, non-malformed infants of birthweight > or =2500 g in Ice

Historical Perspectives: Low Birthweight and Preterm Infants in Indonesia.

The incidence of infants with low birthweight (LBW), prematurity, and small for gestational age (SGA) in Indonesia remains high, and the morbidity and mortality in these infants are significant. To study these groups of infants in Indonesia, the author searched PubMed/Medline, the Cochrane Library, Google, Indonesian reports, local publications, and doctoral and master theses in English and Indonesian from 1987 to February 2019. In this review, the development of local reference growth charts of infants born in Indonesia is described, as well as the importance of these curves in defining the p

Frequency of anomalies and hospital outcomes in infants with gastroschisis and omphalocele.

BACKGROUND: Gastroschisis and omphalocele are the most common anterior abdominal wall defects affecting infants. There are few large cohort studies describing the frequency of associated anomalies in infants with these 2 conditions. We describe associated anomalies and outcomes in infants with these defects using a large, multi-center clinical database. METHODS: We identified all infants with gastroschisis or omphalocele from a prospectively collected database of infants discharged from 348 neonatal intensive care units in North America from 1997 to 2012. Maternal and patient demographic data

A 24-Year-Old Woman Presenting in the Third Trimester of Pregnancy with Nausea, Vomiting, and Abdominal Pain and Diagnosed with Acute Fatty Liver of Pregnancy.

BACKGROUND Acute fatty liver of pregnancy (AFLP) is a rare obstetric emergency that most commonly occurs in the third trimester and has high mortality rates for the mother and fetus. AFLP is a diagnosis of exclusion supported by identifying 6 or more of the 15 Swansea criteria. This report is of a 24-year-old woman presenting in the third trimester of pregnancy with nausea, vomiting, and abdominal pain and diagnosed with AFLP. CASE REPORT A 24-year-old woman presented at 36 weeks of gestation with nausea, vomiting, and abdominal pain. Investigations showed leukocytosis, hyperbilirubinemia, inc

Early use of corticosteroids in infants with a clinical diagnosis of Pneumocystis jiroveci pneumonia in Malawi: a double-blind, randomised clinical trial.

BACKGROUND: Pneumocystis jiroveci pneumonia (PJP) is the most common opportunistic infection in infants with vertically acquired HIV infection and the most common cause of death in HIV-infected infants. OBJECTIVES: To determine whether early administration of adjuvant corticosteroids in addition to standard treatment reduces mortality in infants with vertically acquired HIV and clinically diagnosed PJP when co-infection with cytomegalovirus and other pathogens cannot be excluded. METHODS: A double-blind placebo-controlled trial of adjuvant prednisolone treatment in HIV-exposed infants aged 2

Organ dysfunction and mortality in preterm neonates with late-onset bloodstream infection.

BACKGROUND: Organ dysfunction (ODF) in late-onset bloodstream infection (LBSI) is associated with increased risk of adverse outcomes. However, no established definition of ODF exists among preterm neonates. Our objective was to describe an outcome-based ODF definition for preterm infants, and assess factors associated with mortality. METHODS: This is a six-year retrospective study of neonates <35 weeks gestational age, >72 h of age, with non-CONS bacterial/fungal LBSI. Discriminatory ability of each parameter for mortality was evaluated: base deficit ≤-8 mmol/L (BD8), renal dysfunction (urine

Predictive ability of neonatal illness severity scores for early death in extremely premature infants.

BACKGROUND: The predictive ability of neonatal illness severity scores for mortality or morbidity in extremely premature infants has not been extensively studied. We aimed to evaluate the ability of neonatal illness severity scores [Clinical Risk Index for Babies II (CRIB II), Score for Neonatal Acute Physiology II (SNAP-II), and SNAP-Perinatal Extension II (SNAPPE-II)] in predicting mortality and short-term morbidity of extremely premature infants. METHODS: This retrospective study involved 171 infants with gestational age (GA) between 22 and 27 weeks who were admitted to the NICU during 201

A Review on Umbilical Cord Milking and Its Implications in Neonatal Health.

In India, there is an extreme lack of advancement in techniques concerning the care of infants during labor and post-pregnancy, i.e., the postnatal period. India projected an Infant Mortality Rate of 28.771 deaths per 1000 live births for the year 2021. Such a high death rate in infants arises a dire need to discover and launch new techniques and increase the application of the existing lesser-known techniques. One less well-known technique is the milking of the infant's umbilical cord. Transfusion of placental blood has recently been considered definitive in the care of newborns with the boon…

Predisposition of infants with chronic lung disease to respiratory syncytial virus-induced respiratory failure: a vascular hypothesis.

BACKGROUND: Respiratory syncytial virus (RSV) causes the highest rate of severe respiratory infections and mortality in infants and children worldwide. Preterm infants with underlying chronic lung disease (CLD), including bronchopulmonary dysplasia (BPD), are among those at high risk for severe morbidity, long term sequelae and mortality postinfection. The definition of CLD/BPD has evolved and is currently described as a disease of restricted lung development (i.e. impaired alveolar and pulmonary vascular development). This article describes potential mechanisms by which RSV infection causes r

Localization of the T-cell response to RSV infection is altered in infant mice.

OBJECTIVES: Respiratory syncytial virus (RSV) is a leading cause of lower respiratory tract infections worldwide, causing disproportionate morbidity and mortality in infants and children. Infants with stronger Th1 responses have less severe disease, yet little is known about the infant T-cell response within the air space. Thus, we tested the hypothesis that RSV infected infant mice would have quantitative and qualitative deficiencies in CD4+ and CD8+ T-cell populations isolated from the bronchoalveolar lavage when compared to adults and that local delivery of IFN-γ would increase airway CD4+

Congenital anomalies: an increasingly important cause of mortality and workload in a neonatal intensive care unit.

With improvement in perinatal management and advances in neonatal intensive care, congenital anomalies have emerged as a major cause of neonatal mortality and morbidity. A 1 year review (1985) of all the admissions to our neonatal intensive care unit (NICU) showed that 78 of 790 (10%) had one or more congenital anomalies. Of these, 26% died in the NICU; another 10% died during the first year; therefore the total mortality of infants with congenital anomalies in the first year of life was 36%. Even though they accounted for only 10% of the total admissions, the infants with congenital anomalies

Machakos Project Studies: agents affecting health of mother and child in a rural area of Kenya. XVI. The outcome of pregnancy.

The outcome of pregnancy of all women belonging to the study population of the Machakos project studies who delivered during 1975 and 1976 is presented. The birth-rate was 43.0 per 1,000 population per year, the fertility rate was 235 per 1,000 women 15--44 years of age per year. Unexpected low mortality rates were found; stillbirths, neonatal and infant death rates were respectively 29.8 per 1,000 total births and 22.5 and 50.0 per 1,000 live births. One maternal death occurred among the 2,223 deliveries. Maternal age of less than 25 and over 34 years, a history of previous perinatal death an

Breastfeeding.

The benefits of breastfeeding for the infant and nursing mother are well established. The World Health Organisation (WHO) recommends exclusive breastfeeding up to 6 mo and continued breastfeeding up to 2 y. Exclusive breastfeeding is the most effective intervention to reduce infant mortality and is estimated to prevent 13 % of under 5 child mortality in low income countries. However, rate of exclusive breastfeeding is alarmingly low in our country. Mothers who face problems in breastfeeding immediately resort to top feeds. It is the responsibility of the health professionals to identify and ma…

A proportional hazards model with time-dependent covariates and time-varying effects for analysis of fetal and infant death.

Birth-weight- and gestational-age-specific perinatal mortality curves intersect when compared by race and maternal smoking. The authors propose a new measure to replace fetal and infant mortality and an analytic strategy to assess the effects of risk factors on this outcome. They used 1998 data for US Blacks and Whites. Age-specific post-last menstrual period (LMP) mortality rate was defined as the proportion of deaths (stillbirth, perinatal death, or infant death) at a given age post-LMP. The authors used extended Cox regression with time-varying covariates and hazard ratios to model the effe…

Adverse perinatal outcomes associated with respiratory distress syndrome in preterm infants: a retrospective analysis.

BACKGROUND: With the development of perinatal medicine and the continuous improvement of neonatal treatment technology, the birth rate of preterm infants is increasing. Respiratory distress syndrome (RDS) is one of the most prevalent complications with a high mortality rate among preterm infants. It is associated with short and long-term adverse outcomes for newborns, and seriously affects their survival rate and long-term quality of life. The aim of the present study was to investigate the perinatal risk factors for RDS, and the major complications and mortality rate associated with RDS in pr

Disseminated Tuberculosis With Cavitary Lung Disease in a Very Preterm Infant.

Tuberculosis (TB) is an important cause of poor health outcomes and mortality for pregnant women and their infants globally. Screening during pregnancy helps diagnose TB at a critical period to avert morbidity and mortality but may be overlooked. TB incidence in the United States is low but has been increasing since 2020, alongside a global resurgence. The Centers for Disease Control and Prevention (CDC) recommends TB screening for all individuals with epidemiologic risk factors, including birth, residence, or prolonged travel in medium- or high-prevalence settings; immunosuppression; and clos

Comprehensive Assessment of Risk Factors of Cause-Specific Infant Deaths in Japan.

BACKGROUND: Public attention is given to infants with socially high risks of child abuse and neglect, while clinical attention is provided to infants with a biologically high risk of diseases. However, few studies have systematically evaluated how biological or social factors cross over and affect cause-specific infant mortality. METHODS: We linked birth data with death data from the Japanese national vital statistics database for all infants born from 2003-2010. Using multivariate logistic regression, we examined the association between biological and social factors and infant mortality due …
