A case of late-diagnosed colovesical fistula leading to complete colonic obstruction and urethral fecal diversion.

BACKGROUND: Colovesical fistula (CVF) typically manifests with pneumaturia or fecaluria and is diagnosed promptly. We present an exceptionally rare and severe manifestation of CVF resulting from a 10-year diagnostic delay, which culminated in extraordinary complications, scarcely documented in contemporary literature. CASE PRESENTATION: A 42-year-old Arab male from Syria was diagnosed with a colovesical fistula (CVF) after 10-year diagnostic delay. This protracted interval resulted in the development of a fistula measuring approximately 5 cm, herniation of the affected sigmoid colon segment i

Enterosalpingeal fistula complicating Crohn's disease: Report of two cases and review of the literature.

INTRODUCTION AND IMPORTANCE: Reports of enterosalpingeal fistulae complicating Crohn's disease are scarce. They involve the last ileal loop and lead to a progressive destruction of the salpinx. Usually, no genital symptoms are found. In all the cases reported in the literature, the fistula was diagnosed intra-operatively and resection of the right salpinx was performed without the patient's pre-operative consent. CASE PRESENTATION: We describe 2 cases of women presenting with an Enterosalpingeal fistulae complicating Crohn's disease. Radiological findings allowed a pre-operative diagnosis. Th

Enterovesical fistula complicating Crohn's ileocolitis.

An enterovesical fistula, an infrequent complication of Crohn's ileocolitis which is considered to constitute a difficult surgical problem, was encountered in 29 patients. These patients had been treated for Crohn's disease by medical means for years and several had previously undergone bowel resections. An exacerbation of the Crohn's disease occurred in all patients coincident with the appearance of dysuria or the obvious clinical presentation (fecaluria) of the fistula. Other manifestations of active Crohn's disease frequently coexisted. Radiographic evaluation was of limited help in establi

[Gallstone ileus in underlying stenosis of the sigmoid due to recurrent diverticulitis--a rare complication of cholelithiasis].

BACKGROUND: Gallstone ileus is a rare complication of cholelithiasis and an uncommon cause of intestinal obstruction. It accounts for about 1-4% of all mechanical bowel obstructions. The clinical symptoms and signs of gallstone ileus are mostly nonspecific. CASE REPORT: An 82-year-old woman with recurrent diverticulitis of the sigmoid was admitted because of cramping right upper abdominal pain and signs of large bowel obstruction. Abdominal ultrasound revealed pneumobilia and severe diverticulitis of the sigmoid with signs of ileus. Endoscopic retrograde cholangiography visualized the cholecy

Enterolithiasis: a case report and review.

BACKGROUND: Enterolithiasis is an uncommon finding of intraluminal calcified meconium. Whereas extraluminal calcifications are commonly reported and usually indicate intrauterine intestinal perforation with intraperitoneal extravasation of meconium (meconium peritonitis), true intraluminal calcifications of meconium in newborns are rare. CASE REPORT: We report a case of a newborn boy with pneumothorax and pneumoperitoneum because of a transmediastinal air leakage who unfortunately died on the fifth postnatal day after a pneumopericard. The baby had lung hypoplasia and a hypoplastic thorax. Ol

[Aspects of the complications after restorative proctocolectomy in the treatment of ulcerative colitis. Review of the literature and personal experience].

PURPOSE: Analysis of complications and causes of failure after stapled restorative proctocolectomy with ileal J pouchanal anastomosis in patients with ulcerative colitis is presented. PATIENTS AND METHODS: The procedure was performed in 28 patients, 19 males (68%) and 9 females (32%); diverting ileostomy was always performed. RESULTS: There was no perioperative mortality. The overall morbidity rate was 31%. Six patients (21%) had pelvic abscess, 2 (7%) pelvic hematoma, 4 patients (14%) presented ileo-anal anastomotic stricture, 1 patient (3.6%) had pouch-vaginal fistula, three patients (11%)

[The use of biodegradable valtrac-bar rings for intestinal anastomosis in advanced ovarian cancer patients].

Intestinal anastomoses using Valtrac-Bar biofragmentable rings were performed in 9 advanced ovarian cancer patients presenting with symptoms of acute or subacute intestinal obstruction. Two ileo-transversal anastomoses, six sigmo-rectal and one ileo-ileal ones were performed. A part of sigmoid colon with tumour causing obstruction was resected in seven patients and in two patients a resection of an ileal loop was performed. In two patients a colostomy was necessary. The result of anastomosis with Valtrac-Bar was satisfactory in all patients: no cases of peritonitis, anastomosis leak or fistula

Chyme reinfusion nutritional management for enterocutaneous fistula: first international application of a novel pump technique.

AIM: High-output enterocutaneous fistulas (ECFs) are an established cause of intestinal failure. Parenteral nutrition (PN) remains the gold standard for nutritional management but is complex, expensive and associated with significant complications. Chyme reinfusion (CR) has been reported by multiple centres as a viable option for nutritional management that improves nutritional status, provides the capacity to cease PN and is cost-effective. The aim of this paper is to describe the first use of a novel pump device (The Insides System™) by an independent centre in Australia for the nutritional

Endoscopic ultrasonography in inflammatory bowel disease, paracolorectal inflammatory pathology, and extramural abnormalities.

Endoscopic ultrasound (EUS) is easy to perform, noninvasive, and well tolerated in individuals with inflammatory bowel disease. In the anorectal region, the examination takes minimal preparation and has become a routine procedure in the diagnosis and therapeutic follow-up of malignant and nonmalignant disorders. Topics discussed in this article include techniques, abscesses, and fistulas.

Iliac Artery-Uretero-Colonic Fistula Presenting as Severe Gastrointestinal Hemorrhage and Hematuria: A Case Report and Review of the Literature.

Arterioenteric fistulas represent an infrequent but potentially fatal cause of gastrointestinal hemorrhage. Patients often present in extremis from shock and sepsis. This mandates a rapid diagnosis so that prompt, potentially life-saving interventions can be performed. We report the case of a 35-year-old man who presented with hematuria and hematochezia secondary to an iliac artery-uretero-colonic fistula that developed years after open common iliac artery aneurysm repair. His condition rapidly progressed to hemorrhagic shock, and he underwent successful endovascular treatment with a covered s

Right-sided diaphragmatic hernia complicated with broncho-pleuro-colonic fistula presenting as fecoptysis.

A 37-year-old woman presented with a chief complaint of fecoptysis since 1 month. She belongs to low socio-economic status. There was no history of pulmonary tuberculosis or trauma. With the help of radiological findings, the authors were able to establish this rare diagnosis 'right-sided diaphragmatic hernia complicated with broncho-pleuro-colonic fistula presenting as fecoptysis'. Diaphragmatic hernia is a common finding in day to day practice. Fistula between bowel loops and other viscera are also common entity, but fistula between pulmonary air-way passage and colon is a rare finding. Right

Rupture of gastroduodenal artery aneurysm into the portal vein.

Aneurysms of the gastroduodenal artery are very rarely observed. Rupture occurs in about 60% of cases and is usually associated with gastroduodenal bleeding. A case of rupture into the portal vein and secondary portal hypertension is presented. The combined use of computed tomography (CT scan) and selective visceral angiography was very useful for the diagnosis.
