An approach to vertigo in general practice.

BACKGROUND: Dizziness is a common and very distressing presentation in general practice. In more than half of these cases, the dizziness is due to vertigo, which is the illusion of movement of the body or its surroundings. It can have central or peripheral causes, and determining the cause can be difficult. OBJECTIVE: The aim of this article is to provide a clear framework for approaching patients who present with vertigo. A suggested approach to the assessment of vertigo is outlined. DISCUSSION: The causes of vertigo may be central (involving the brainstem or cerebellum) or peripheral (invo

A case of bilateral internal auditory canal osteomas.

Osteomas of the skull base are rare, benign, slowly progressing growths of dense cortical bone. Osteomas occurring in the internal auditory canal are extremely rare. These lesions have sometimes been linked with dizziness, sensorineural hearing loss, and/or tinnitus. Although there have been documented cases in which surgical excision has improved these symptoms, symptomatic relief is not always achieved with surgical management. Here we present, to the best of our knowledge, only the third reported case of bilateral osteomas of the internal auditory canal. An 82-year-old woman presented with

Computed tomography findings of labyrinthitis ossificans secondary to meningitis: a case report.

A 26-year-old man was admitted to our hospital with chronic left ear drainage. He had a history of meningitis when he was a child. Computed tomography (CT) of the temporal bone showed complete obliteration of the otic labyrinth by sclerotic tissue. Based on CT findings, the patient was diagnosed as labyrinthitis ossificans. Computed tomography is an appropriate method of examination for the identification of labyrinthitis ossificans and is of particular importance for the evaluation of patients before cochlear implantation.

Recurrent inhibition in humans.

Methods have been developed to investigate recurrent inhibition (RI) in humans. A conditioning reflex discharge is used to evoke in motoneurones (MNs) supplying homonymous and synergistic muscles, an inhibition the characteristics of which are consistent with RI: it appears and increases with the conditioning motor discharge, has a short latency and a long duration, and is enhanced by an agonist of acetylcholine. As in the cat, homonymous RI exists in all explored motor nuclei of the limbs except those of the digits and the pattern of distribution of heteronymous RI closely matches that of mon

[Lipoma of the internal auditory canal. Report of two cases simulating acoustic neuroma].

We report two cases of intra-auditory canal lipoma, presenting as acoustic neuroma, observed in respectively 45 and 53 year old patients. Cerebellopontine angle lipomas are unusual lesions, more exceptional than their intracranial counter-parts. The knowledge of these tumoral lesions and of their close relationships with cranial nerves incite to perform a frozen section when imagery is evocative, to limit functional postoperative sequelae.

Labyrinthine involvement in Langerhans' cell histiocytosis.

BACKGROUND: Langerhans' cell histiocytosis, a rare condition caused by the proliferation of abnormal Langerhans' cells ('LCH cells') and an accompanying granulomatous infiltrate, can affect several organs including the ear. External and middle ear involvement are common with a reported incidence as high as 61%. The bony labyrinth is resistant to erosion by the granulation tissue, thereby protecting the cochlea and vestibular structures. Probably for this reason, involvement of the inner ear is rare, with few case reports in the literature. PATIENTS: We report two girls, one with bilateral and

Visual object localization through vestibular and neck inputs. 1: Localization with respect to space and relative to the head and trunk mid-sagittal planes.

Object localization in space signals in the absence of an external reference (visual, auditory, haptic) involves a signal of the head in space (vestibular). The present study asks whether signals of body position relative to the support surface (proprioceptive) are involved as well, by investigating the role of vestibular-neck interaction (dissociating head and trunk position). Normal human subjects saw a light spot (object) and continuously nulled displacement steps of the spot. They did so before and after vestibular and/or neck rotational stimulation in the horizontal plane, reproducing a p

Effects of systemic and lateral semicircular canal administration of aminoglycosides on normal and hydropic inner ears.

Streptomycin was injected subcutaneously into guinea pigs (n = 12) with unilateral endolymphatic hydrops. In the unoperated ears, sensory cells were degenerated at the basal ends of all cochleas and to varying extents in the vestibular sense organs. Hydropic ears, in contrast, showed greater ototoxicity in the cochlear and saccular sensory cells, while the drug's effect on the utricular and cristae sensory cells was increased, but less remarkable. In another series of normal animals (n = 16), streptomycin was applied to a fenestra of the lateral semicircular canal. Sensory cells of all three c

Perilymphatic fistula: surgical experience in the United States.

One survey sent to 6953 individual otolaryngologic practices and 106 departments of otolaryngology at teaching hospitals in the United States, and a more limited survey of 75 patients operated on for perilymphatic fistula (PLF) at the House Ear Institute, addressed aspects of managing PLF: surgical incidence, reliability of diagnostic test, preoperative observations, and disability after surgery. Of surgeons sampled, 93% estimated incidence of PLF surgery to be less than or equal to 1 per 1000 otolaryngologic outpatient visits. The most reliable diagnostic indicators were history, symptomatolo

Surgical management of cholesteatoma in an only hearing ear.

The patient with a cholesteatoma in an only hearing ear presents a management dilemma: how best to treat the patient to minimize the chances of developing a severe hearing loss in that ear. Twelve patients managed surgically for cholesteatoma in their only hearing ear are reviewed. The location and extent of the cholesteatoma, the type of operation performed, and postoperative hearing results are presented. Eleven of the 12 patients maintained their bone-conducted speech reception threshold to within 5 dB of the preoperative level during follow-up periods of 2 months to 4 years. Recommendation

[Secretory otitis media and mastoiditis in children].

In 1984-1987 the authors followed up a selected group of 15 children with a long-standing history of secretory otitis media and latent mastoiditis. In there of them the condition was complicated by labyrinthitis. Mastoidectomy which was performed in 12 children contributed to the healing of chronic inflammatory changes in the middle ear.

[Recurrent purulent meningitis and deafness: Mondini's malformation].

A 5-year-old girl with severe deafness (90 dB) had two successive attacks of bacterial meningitis. A vesicular cochlea was found on polytomography, and a bilateral defect in the stapes footplate with cerebrospinal fluid leakage during surgery (Mondini dysplasia). Although rare, the eventuality of this malformation emphasises the utility of polytomography of the inner ear in children with congenital deafness.

Sodium fluoride: effectiveness of treatment for cochlear otosclerosis.

We examined the effectiveness of sodium fluoride in treatment of cochlear otospongiosis in ninety-four patients with cochlear otosclerosis and ninety-eight patients with stapedial otosclerosis and sensorineural hearing loss. The drug halted or slowed the progression of sensorineural hearing impairment in 63% of the patients with cochlear otosclerosis and 46% of the patients with stapedial otosclerosis. The single factor that best predicted which patients would respond most favorably to treatment was rate of progression before treatment. Sodium fluoride therapy was successful for 79% of the pat

Vestibular Meniere's disease.

The clinical entity of vestibular Meniere's disease (VMD) is assessed and described. Twenty percent of cases eventuating in typical Meniere's disease presented with VMD first. In other patients VMD alone persisted for many years (30 or more). VMD, usually unilateral, is characterized by recurrent episodes of vertigo typical of classical Meniere's disease. Positional vertigo can occur between or during episodes. Aural pressure is second to vertigo in importance, and electronystagmographic findings are third. Although hearing is normal, tinnitus or loudness intolerance may be present. Concepts o

Multidirectional and computerized tomography of the vestibular aqueduct in Meniere's disease.

Multidirectional and computerized tomography have been used for the radiographic assessment of the vestibular aqueduct in Meniere's disease. The results of studies performed by us and/or other authors indicate that there is a statistically significant difference in the size of the vestibular aqueduct between the Meniere's and control groups (in Meniere's disease the vestibular aqueduct is often narrowed or obliterated); and that periaqueductal pneumatization is often decreased or absent in ears affected by Meniere's disease. Recent histological studies performed on temporal bones of patients w

The incidence, site, treatment and fate of labyrinthine fistula.

Although the incidence of labyrinthine fistula appears to be similar in most reported series, the number of patients reported to have a positive response to the fistula test do not. This may be due to a lack of uniformity in the performance of the test. The method of the author, the direction of eye movement and its diagnostic significance are described. The occurrence and mechanism of severe sensorineural hearing loss in 5 out of 20 patients in whom the matrix was removed accidently from the fistula during mastoidectomy is discussed.
