20 patients with histologically verified carcinoma of the vulva fulfilled the following conditions: general operability, preoperative lymphography, radical vulvectomy with inguinal lymph node resection and histological examination of all lymph nodes. Clinical, lymphographic and morphologic non tumor induced lymphnode lesions of 164 patients are described, and a roentgenographic morphological correlation is performed. Diffuse non specific inflammatory and reactivy lymphnode lesions are divided into three groups. These changes cannot be distinguished from lymphnodes with micrometastases. Typical cirteria for segmental lymphnode fibrosis and hyperplasia are described. Fatty lymphnode degeneration may be segmental or diffuse, their characteristics are presented as well.

A female in her 70s was referred to the endocrine surgery unit with a 22 mm right thyroid nodule that was incidentally identified on CT. Cytology from an ultrasound-guided fine needle aspirate revealed papillary thyroid carcinoma. After discussion of surgical options, the patient opted to undergo a total thyroidectomy. The presence of tumour cells of high mean nuclear area (MNA) in axillary nodal metastases from breast carcinoma is shown to be related to the finding of tumour cells in the efferent nodal vessels. In cases without efferent vascular invasion (EVI) the MNA was on average lower. EVI positivity combined with a high MNA is associated with a poor prognosis. However, a subgroup was identified, i.e. the MNA 1, with the smallest nuclei, in which prognosis approached that in the EVI negative. In the latter the differences related to MNA groups were not statistically significant.

Serial sections were performed on the axillary lymph nodes from 28 women with mammary carcinoma and intramammary lymphatic tumor emboli. Occult metastases were demonstrated in nine women (32%), all less than 2 mm in diameter. During the 10 years of follow-up, no difference in recurrences was found between this group (33%) and those without occult metastases (37%). Serial sectioning of lymph nodes in this group of patients does not appear to be helpful in prognosis.

From the central part of the labium majus the lymphatic transport of 198Au is invariably bilateral to inguinal and pelvic lymph nodes. Thus, bilateral inguinal and low pelvic lymph-adenectomy is logical in the treatment of carcinoma of the vulva. 198Au injected subcutaneously and Lipiodol injected intralymphatically in the dorsum of the foot were transported exclusively to ipsilateral inguinal and pelvic lymph nodes. Bladder cancer with cutaneous metastasis is a rare manifestation of the advanced stage of the disease. It can result from direct invasion, lymphatic or hematogenous spread, or iatrogenic implantation. We present a case of a 67-year-old patient initially diagnosed with urothelial carcinoma (UC) in situ of the bladder, who underwent transurethral resection of bladder tumor, along with induction and maintenance Bacillus Calmette-Guerin immunotherapy. Six years post-diagnosis, the patient developed multiple ulcerating fungating lesions in the right lower extremity, confirmed as metastases from UC.

INTRODUCTION: Testicular germ cell tumours metastasize in a very predictable fashion involving the retroperitoneal lymph nodes first followed by either lymphatic spread via thoracic duct or hematogenous spread to distant organs like lungs, liver and brain.

Retrocrural tumors of the lower posterior mediastinum are rare. Most tumors in this region involve lymphatic metastasis of germ cell tumors. Formerly these tumors were resected through a combined thoracoabdominal approach, with division of the diaphragm and effecting an at least partial phrenic paresis. The ventral approach via bilateral subcostal incision with cranial extension avoids this problem and allows a bilateral access to the retrocrural space. A 63-year-old man with recurrent metastatic malignant melanoma presented with a painful right twelfth thoracic dermatomal eruption initially thought to be herpes zoster. However, 3 weeks later, the patient clearly had melanoma skin metastases confined exclusively to this dermatome. Radiotherapy and opiate analgesics provided effective pain relief. We propose that in this patient, no herpes zoster infection occurred, but rather that skin metastases presented in a zosteriform pattern due to lymphatic spread from a previously excised paravertebral skin metastasis.
