Diuretics and potassium/magnesium depletion. Directions for treatment.

The effects of diuretics on renal handling of potassium and magnesium can result in depletion of these electrolytes. Associations between these deficits and the occurrence of increased ventricular ectopy raise the issue of potential relationships to increased risk of sudden unexpected death in hypertensive patients taking diuretics, especially patients with electrocardiographic and electrolyte abnormalities. Review of diuretic effects on the kidneys provides evidence that diuretic regimens that conserve potassium also conserve magnesium, and suggests the important role of these agents in prote…

Effects of magnesium deficiency on the pathogenesis of myocardial infarction.

The death rate due to myocardial infarction appears to vary with dietary consumption of Mg. This could be due to effects on atherosclerosis, coronary artery spasm, altered pathogenesis of myocardial infarction, increased vulnerability to arrhythmia, or some combination of these. Mg deficiency (MD) has been found to increase the severity of a coronary occlusive event in terms of the amount of necrosis produced by a given occlusion. MD is also associated with increased likelihood of arrhythmia development. In addition, reduced extracellular magnesium concentration (Mgo) is associated with contra…

Renal handling of magnesium: drug and hormone interactions.

Magnesium is the fourth most abundant cation in the human body and the second most common cation in the intracellular fluid. The abundance and distribution of this divalent cation implies an essential role of magnesium in intracellular metabolism. Although no single homeostatic control has been demonstrated for magnesium, the cellular availability of this cation is closely regulated by the gastrointestinal tract, kidney and bone. The purpose of this review is to survey some of the events involved in renal magnesium handling. The excretory side of magnesium balance involves appropriate changes …

Magnesium and potassium. Inter-relationships in cardiac disorders.

Magnesium and potassium are the 2 major intracellular cations. The intracellular concentrations of these 2 ions appear to be closely correlated, but the existence of a relationship between the plasma concentrations of these ions has been controversial. A major function of potassium is to maintain the excitability of nerve and muscle tissue. Alterations in either the intracellular or extracellular potassium concentration affect membrane excitability by alterations in the resting membrane potential. The critical factor is not the actual potassium concentration in either compartment but rather th…

[Treatment of torsade des pointes by intravenous magnesium].

A 72 year old woman was admitted with decompensation of a hypertensive cardiopathy and treated with diuretics. She developed recurrent syncopal torsades de pointes during the 24th hour which were reduced by a bolus intravenous injection of 3 g of magnesium sulphate (Mg SO4). There was a recurrence 30 minutes later which regressed after a second injection of 3 g of Mg SO4. A continuous intravenous infusion of 18 g/day of Mg SO4 prevented further recurrences of the arrhythmia. Biochemical analysis showed intra and extracellular magnesium deficiency at the time of the torsades de pointes but the …

Alteration of prostanoid metabolism in rats with magnesium deficiency.

Plasma and tissue concentrations of prostanoids PGE2, PGF2 alpha. 6-keto-PGF1 alpha (a stable metabolite of prostacyclin) and TXB2 (a stable metabolite of thromboxane A2) were measured in normal and magnesium-deficient rats. The mean values for prostanoids in plasma were significantly higher in magnesium-deficient rats than in normals (515 +/- 43 vs 296 +/- 31 pg/ml for 6-keto-PGF1 alpha, p less than 0.01, 3700 +/- 322 vs 346 +/- 33 pg/ml for TXB2, p less than 0.001 and 1234 +/- 132 vs 434 +/- 51 pg/ml for PGE2, p less than 0.001). Tissue levels of prostanoids were also significantly higher in…

Magnesium deficiency in the pathogenesis of mitral valve prolapse.

Idiopathic mitral valve prolapse (MVP) is the commonest valvular disorder in industrialized nations. It is predominantly a familial condition, showing Mendelian dominance with delayed and variable penetrance. Although hyperkinesis and hypertrophy of the left ventricle have been described in MVP, its histopathology, somatic morphology and genetics support the leading theory that MVP results from a hereditary disorder of connective tissue. Latent tetany (LT) due to chronic Mg deficit (Mg-D) occurs in over 85% of MVP cases; MVP complicates 26% of LT. Mg-D can explain many clinical features of the

Magnesium deficiency and myocardial infarct size in the dog.

Although epidemiologic data suggest a relation between myocardial infarction death rates and dietary intake of magnesium, there are no experimental studies reflecting such a phenomenon. It is now reported that beagle dogs kept on a severely magnesium-deficient diet for 100 days develop a larger infarct than do control animals. Control animals were either kept on the same diet as experimental animals with supplementary magnesium, or were fed standard dog chow. The control groups were indistinguishable and were therefore pooled. Infarction was produced by occlusion of the left anterior descendin…

Inhibition of tumor growth by magnesium depletion of rats.

In earlier studies tumor growth was inhibited in rats that first were made magnesium-deficient and subsequently implanted with tumors. The objective of this study was to determine if the reverse sequence is also effective. Namely, is tumor growth inhibited if tumors first are established and then a magnesium deficiency imposed? Rats with palpable mammary adenocarcinomas, or with sham implants were pair-fed either a control or magnesium-deficient diet for 32 days. Rats were then killed and tumors, plasma and several tissues analyzed. In tumors from magnesium-depleted rats, growth was inhibited …

Cardiovascular toxicity of diuretics related to magnesium depletion.

The chronic administration of common loop or distal tubular diuretics may lead to somatic depletion of Mg2+. The resultant deficiency of this cation causes an increase in intramyocardial cytosolic Ca2+ and aggravates the decrease in intramyocardial K+ provoked by diuretics through their hyperkaliuretic effect. Thus the myocardium is electrically destabilized and cardiac arrhythmias may develop. Mg2+ deficiency positively contributes to the development of atherogenic alterations in lipid metabolism, vasospastic phenomena in the coronary and cerebrovascular territories, myocardial infarction and

The pathophysiology of potassium and magnesium disturbances. A cardiac perspective.

Sodium (Na+) and calcium (Ca++) ions are both involved in the activation of cardiac muscle cells. Na+ ions initially depolarise the cell, and the cell-inward flux of Ca++ ions maintains the plateau phase of the action potential. Ca++ ions trigger the release of more Ca++ ions from the sarcoplasmic reticulum. The free Ca++ ions inside the cell subsequently activate the formation of actomyosin complexes and initiate the contraction. Potassium (K+) and magnesium (Mg++) ions modulate the actions of Na+ and Ca++. Hyperkalaemia lowers the membrane potential, and thereby reduces excitability and cond…

The effects of maternally administered magnesium sulfate on the neonate.

The effects of parenterally administered magnesium sulfate on maternal and neonatal calcium and magnesium metabolism in nonasphyxiated, term pregnancies complicated by pregnancy-induced hypertension were studied prospectively. In addition, the neurobehavioral effects of neonatal hypermagnesemia were investigated by means of a neonatal assessment scale that specifically measures reflex activity and both passive and active muscle tone. Maternal magnesium sulfate infusion was associated with maternal and neonatal hypermagnesemia when compared with that of control subjects (1.8 +/- 0.10 to 3.6 +/-…

Pathogenesis of arrhythmogenic changes due to magnesium depletion.

Magnesium and potassium deficiency may complicate prolonged treatment with any diuretic causing hypermagnesiuria and hyperkaliuria. Hypomagnesaemia is usually associated with reduced intracellular magnesium and potassium levels and increases in sodium and cytosolic calcium concentrations within the myocardium. These changes give rise to nonspecific clinical signs and symptoms. No satisfactory routine procedure exists for the evaluation of total body magnesium levels.

Potentiation of magnesium-induced neuromuscular weakness by gentamicin.

A term neonate with hypermagnesemia secondary to maternal administration of MgSO4 for pre-eclampsia had a respiratory arrest after receiving gentamicin im. The neuromuscular tracings and sequential physician exams done before and after administering gentamicin suggest aminoglycosides may potentiate a magnesium-induced impairment of neuromuscular transmission and cause muscular weakness in neonates.

Serum calcium and magnesium levels in chronic schizophrenics.

Serum calcium and magnesium levels were studied during neuroleptic treatment in a group of 29 chronic schizophrenic inpatients who had previously remained drug free for at least 4 weeks. The drug-free values of both electrolytes were not significantly different between patients and controls. In three patients who developed catatonic stupor during the drug-free period, calcium was significantly increased at the onset of the catatonic manifestations. Both electrolytes were significantly decreased in the whole group during treatment; the magnitude of the decrease was independent of the neurolepti…
