The role of community-based blood pressure screening in improving hypertension care.

In their recent Viewpoint, Frieden and colleagues argue that mass blood pressure screening diverts resources from improving hypertension care. We present a counterargument that community-based blood pressure screening can complement health-care services by increasing hypertension detection, particularly in populations with limited access to health care. Opportunistic community-based screening can be delivered at relatively low cost and reach individuals who might not otherwise engage with health-care systems. In settings where access to health-care facilities is constrained, such approaches pr

Targeted adult hearing loss screening in the UK: policy options for equitable implementation through health-service pathways.

Adult hearing loss is common, disabling, and frequently under-detected, with help-seeking often delayed and unmet need unevenly distributed across populations. In the UK, adult hearing-loss screening is not currently recommended, and detection largely depends on self-recognition, primary-care presentation, and referral. This Policy Brief examines three policy options: maintaining the status quo, introducing age-based screening at 60, 65 and 70 years, and implementing targeted risk-based screening through existing health-service pathways for adults at elevated risk of cognitive decline. Apprais

Opportunistic CT Screening: Clinical Applications, Technical Foundations, and Best Practices.

Opportunistic CT screening refers to the extraction of clinically relevant imaging biomarkers from CT examinations routinely performed for unrelated primary indications. This screening does not require additional radiation exposure, imaging time, or patient burden. Advances in automated image analysis and artificial intelligence (AI) have substantially expanded the feasibility of opportunistic CT screening across a range of conditions, including low bone mineral density, hepatic steatosis, sarcopenia, adiposity, and vascular calcifications. These biomarkers are increasingly being recognized as

Family Screening in Glaucoma: A Scoping Review.

TOPIC: To assess the prevalence of glaucoma among relatives of known patients with glaucoma, describe the response rate of relatives in different glaucoma family screening programs, and summarize the barriers to family members' participation in these screening programs. CLINICAL RELEVANCE: Population-based screening for glaucoma is a challenge since the prevalence in the general population is low. The presence of a positive family history is a known risk factor for glaucoma, but implementing family screening programs has not become standard practice yet. Further, the literature regarding the

Heavy Menstrual Bleeding in the Gynecology Clinic: A Call for Awareness and Standardized Screening for Underlying Bleeding Disorders.

IMPORTANCE: Heavy menstrual bleeding (HMB) is a common problem existing in nearly 30% of women worldwide. An underlying bleeding disorder has been identified as a cause of HMB in a significant proportion of women seeking gynecological care for HMB. In practice, however, there is a significant underdiagnosis of bleeding disorders in these patients, highlighting a need for improvement in awareness and detection. OBJECTIVE: To present a comprehensive review of the literature on HMB and bleeding disorders and to increase awareness of their interrelation. In addition, to propose a standardized dia

Rheumatoid arthritis-associated interstitial lung disease: screening, diagnosis, and treatment-an expert group consensus statement.

Interstitial lung disease (ILD) is a common and morbid complication of rheumatoid arthritis. Despite its prevalence and impact on patients, there is no agreement on how to screen for rheumatoid arthritis-associated ILD, when to initiate treatment, and the optimal treatment regimen. Due to a scarcity of high-quality studies to guide strict evidence-based guidelines, a multidisciplinary group of rheumatoid arthritis-associated ILD experts was convened to discuss the current body of evidence, synthesise the evidence qualitatively, and provide clinically useful consensus statements pertaining to t

Optimizing Screening Strategies for Hypertension in India: A Consensus Statement.

Hypertension is a highly prevalent issue worldwide. The prevalence of underdiagnosed hypertension in India calls for the need for this consensus article, which aims to optimize screening strategies. In this article, a modified Delphi approach was used to develop consensus statements. Key recommendations include the adoption of standardized screening protocols across diverse healthcare settings, incorporating both office-based and out-of-office blood pressure measurements (OBPM). By implementing these evidence-based strategies, the consensus aims to improve the early detection of hypertension,

Phase angle: a novel application of bioelectrical impedance technology in osteoarthritis screening and diagnosis.

Bioelectrical impedance technology (EBI) offers a non-invasive, cost-effective method for body composition assessment, showing promise in diagnosing and managing musculoskeletal disorders, particularly osteoarthritis (OA). OA is a major global health concern, notably affecting the knee and hip joints. Conventional imaging techniques like X-rays and MRI have limitations in early detection, as they cannot capture microscopic cartilage changes. Phase angle (PhA), an essential EBI parameter, is widely applied in evaluating sarcopenia, tumors, and body fluids, and is increasingly valuable in OA res

Recommendations for implementing rheumatic heart disease echocardiographic detection in remote Australia: A narrative review and lesson-drawing from diabetic retinopathy screening.

BACKGROUND AND OBJECTIVES: Australia has some communities with the highest rates of rheumatic heart disease (RHD) in the world. Echocardiographic detection of RHD through active case finding has been proposed for early detection. Lessons can be learnt from the implementation of a similar program - diabetic retinopathy screening. The aim of this paper is to identify transferable lessons from the implementation of diabetic retinopathy screening that could be applied to RHD echocardiographic detection. METHOD: This paper presents a narrative review and expert recommendations. RESULTS: Transfera

Application of Pragmatic Criteria to Assessments of Frailty and Physical Function Among People With HIV.

Routine screening for any condition in a clinical setting should be pragmatic, meaning that the screening is feasible, relevant, and actionable to patients and providers. Glasgow and Riley (Am J Prev Med 2013) proposed a set of criteria to evaluate whether or not a screening tool can be considered pragmatic. Although recent guidelines recommend screening for frailty among older adults with HIV, routine frailty screening may be challenging given that the tools are often time-consuming and the diagnosis is not always directly actionable given that it can encompass multiple systemic deficits. Expanded voluntary counselling and testing services in Türkiye: insights from an HIV model.

BACKGROUND: Voluntary Counselling and Testing (VCT) centres provide free and anonymous Human Immunodeficiency Virus (HIV) testing along with counselling services. They remove barriers to testing and lead to higher HIV awareness. The current healthcare system in Türkiye achieves high treatment rates in the continuum of care. However, the estimated diagnosis rates are far below the global targets, posing a significant challenge. This also creates an opportunity to intensify prevention efforts against the burden of the disease. Expanded VCT services is a suggested strategy for Türkiye, but resear

Active search for tuberculosis in three youth detention centers in Peru.

Motivation for the study. With tuberculosis being the leading cause of death due to infectious diseases in the world, and having a detection gap of 20-40%, there is a need to implement active search strategies for the disease in different vulnerable populations such as juvenile correctional facilities. Main findings. We found a screening rate by active search of 1250 per 100,000 screened, being 10 times more than the rate in the general population. Seventy-five percent of GeneXpert-positive cases were asymptomatic. Implications. By using innovative TB screening mechanisms and algorithms, cases

Addressing the challenge of tuberculosis screening in diabetic populations: a narrative review of methods and strategies.

The comorbidity of diabetes and tuberculosis (TB) poses a major public health challenge. Patients with diabetes have impaired immune function, increasing their risk of contracting TB. They often present with atypical clinical symptoms after infection, which can lead to delayed diagnosis. Therefore, TB screening in the diabetic patients is essential. This study systematically reviews advances in TB screening strategies for diabetic patients by searching databases including PubMed, WHO Global Index Medicus, Web of Science, China National Knowledge Infrastructure (CNKI), and Wanfang Data Knowledg

A proposal to simplify the definition of pediatric hypertension: bridging the gap between perception and action.

BACKGROUND: The importance of routine hypertension screening in children and adolescents is now well recognized. However, it is often undiagnosed in clinical practice, partly due to the reliance on a complex blood pressure (BP) percentile-based table with hundreds of cutoffs by age, sex, and height. MAIN TEXT: Many studies have explored simplified tools for screening hypertension in children and adolescents, such as simplified formulas, simplified BP tables by age and sex group, by age group, or by height group, and the BP to height ratio. Nevertheless, validation studies have demonstrated th
