The work-up for a discrepant immunoassay result is challenging. A 34-year-old woman with normal menstruation and ovarian function had high follicle-stimulating hormone (FSH) around 100-200 IU/L by Roche elecsys e602 (reference interval 3.0-14.0 IU/L). Magnetic resonance imaging excluded pituitary tumors. A dilution study indicated an interference, however, a heterophilic antibody was not identified. Sciatic nerve endometriosis is a rare and often underdiagnosed form of deep infiltrating endometriosis involving the lumbosacral plexus or sciatic nerve. We present the case of a 40-year-old woman with chronic left thigh and pelvic pain exacerbated during menstruation, accompanied by limited mobility. Pelvic MRI revealed a lesion along the left sciatic nerve with characteristic hemorrhagic and inflammatory features, as well as muscle atrophy indicative of chronic denervation. We present the case of a 44-year-old woman with a recent history of liver transplantation who underwent 99m Tc-RBC scan to identify the source of melena. Initial imaging revealed a zone of increased radiotracer uptake in the hypogastric region. SPECT/CT localized this uptake to the uterus. Delayed imaging performed the following day showed a focal zone in the same region, which was localized to the uterine cavity on the second SPECT/CT, consistent with endometrial shedding during menstruation. Our case emphasizes the importance of SPECT/CT to reduce the false positives of the RBC scan.

Women with the rare Mayer-Rokitansky-Kuster-Hauser (MRKH) syndrome are born without a uterus. Detailed pathologic studies over the past decade suggest a distal fallopian tube origin for the majority of "ovarian" high-grade serous carcinomas (HGSC). It has been known since the eighteenth century that postpartum psychoses can begin several weeks after childbirth, not during the first fortnight. There are almost 400 non-organic episodes in the literature, starting more than 3 weeks after the birth; some of them are recurrent. This paper reports a case of endometriosis of the lung in a 29-year-old woman with long-term periodic catamenial hemoptysis. Acute urinary retention is unusual in children and is usually a candidate for visiting the emergency department upon initial discovery. We describe an adolescent Japanese girl with acanthosis nigricans and irregular anovulatory menstruation following menarche. Serum LH levels were elevated, whereas serum FSH levels were within normal range. An exaggerated response to LHRH was observed. Further, serum androstenedione levels were markedly elevated.
