Altered functional connectivity related to prepulse inhibition in functional movement disorder.

BACKGROUND: Functional neurological disorder (FND) is increasingly understood as a multinetwork disorder. Deficits in subcortical sensorimotor gating, indexed by reduced prepulse inhibition (PPI) of the startle reflex, are reported in FND and may reflect impaired filtering and contextual weighting of sensory input. However, the neural correlates linking impaired gating to intrinsic network dysfunction remain unclear. METHODS: We examined the relationship between PPI and resting-state functional connectivity in 38 patients with functional movement disorder (FMD) and 39 healthy controls (HC). P

Parkinsonism in Childhood, a Clue to Autosomal Recessive ADCY5-Related Movement Disorder?

BACKGROUND: Advances in genetic testing have allowed for refined phenotypic categorization of pediatric-onset genetic movement disorders. ADCY5-related movement disorder is among this group of conditions for which we have begun to better understand a genotype-phenotype correlation. CASES: This case series details the clinical course of three biological brothers with autosomal recessively inherited biallelic variants in the ADCY5 gene. Each brother demonstrates a varied severity and combination of mixed movement disorder phenomenology, including parkinsonism, dystonia, and myoclonus exemplifie…

[Neurodevelopmental and Movement Disorder Due to a Mutation in the GNAO1 Gene: A Case Report].

INTRODUCTION: We present the case of a patient with a de novo heterozygous probably pathogenic variant c.545C>T (p.Thr182Ile) in the GNAO1 gene that is probably pathogenic in relation to a neurodevelopmental disorder and movement disorder. CASE REPORT: A female patient who started at 3 months with severe neurodevelopmental delay, and subsequently myoclonus, orofacial dyskinesia, and choreoathetosis, without seizures. Metabolic and structural causes were investigated and, finally, whole exome sequencing in trio identified a de novo heterozygous, probably pathogenic, variant c.545C>T (p.Thr182I

Automated Analysis of Stereotypical Movements in Videos of Children With Autism Spectrum Disorder.

IMPORTANCE: Stereotypical motor movements (SMMs) are a form of restricted and repetitive behavior, which is a core symptom of autism spectrum disorder (ASD). Current quantification of SMM severity is extremely limited, with studies relying on coarse and subjective caregiver reports or laborious manual annotation of short video recordings. OBJECTIVE: To assess the utility of a new open-source AI algorithm that can analyze extensive video recordings of children and automatically identify segments with heterogeneous SMMs, thereby enabling their direct and objective quantification. DESIGN, SETTI

Episodic mandibular tremor in dogs: an idiopathic movement disorder or a manifestation of pain.

OBJECTIVE: Episodic mandibular tremor (EMT), manifested as teeth chattering, is not well described in dogs. The aim of this study was to describe clinical signs, MRI findings, and outcome of dogs with EMT. ANIMALS: 11 dogs retrospectively and 31 dogs in an online survey. METHODS: A retrospective multicenter study of dogs with EMT between 2018 and 2023 and prospective online questionnaire open to owners of pets with teeth chattering. RESULTS: All dogs had rapid and short-lasting (< 1 minute) episodes of EMT in the absence of other neurological signs. Lip smacking occasionally accompanied the

Sarcopenia in patients with isolated rapid eye movement sleep behavior disorder.

OBJECTIVES: Evaluating of sarcopenia is important for promoting healthy aging, preventing functional decline, reducing the risk of falls and fractures, and improving overall quality of life. This study aimed to investigate sarcopenia in patients with isolated rapid eye movement sleep behavior disorder (RBD) using temporal muscle thickness (TMT) measurement. METHODS: This investigation was retrospectively conducted at a single tertiary hospital. We recruited patients diagnosed with isolated RBD confirmed by polysomnography and clinical history and healthy participants as controls. Patients wit

Kinematic classification of mandibular movements in patients with temporomandibular disorders based on PCA.

This retrospective study aimed to kinematically classify mandibular movements collected during Temporomandibular Disorders (TMD) treatment, employing Fourier transformation (FT), Principal Component Analysis (PCA), and K-means clustering (k-means), and to investigate their correlation with symptoms of pain-related TMD. The study included five TMD participants diagnosed with myalgia (age: 39-86 years, with an SD of 18.96) and three healthy participants (age: 32-42 years, with an SD of 5.13) with no stomatognathic problems. TMD participants underwent tailored treatment for their symptoms, and th

Cognitive Reserve in Isolated Rapid Eye-Movement Sleep Behavior Disorder.

Isolated rapid-eye-movement sleep behaviour disorder (RBD) is considered the prodromal stage of α-synucleinopathies (e.g., Parkinson's disease and dementia with Lewy bodies); however, iRBD patients show a wide variety in the progression timing (5-15 years). The model of cognitive reserve (CR) might contribute to explaining this phenomenon. Our exploratory study aimed to evaluate, for the first time, the impact of CR level on cognitive performance in polysomnography-confirmed iRBD patients. Fifty-five iRBD patients (mean age ± SD: 66.38 ± 7.51; M/F 44/11) underwent clinical and neuropsychologic

Prefrontal network dysfunctions in rapid eye movement sleep behavior disorder.

INTRODUCTION: Resting-state functional connectivity magnetic resonance imaging (rsfcMRI) of rapid eye movement (REM) sleep behavior disorder (RBD) may provide an early biomarker of α-synucleinopathy. However, few rsfcMRI studies have examined cognitive networks. To elucidate brain network changes in RBD, we performed rsfcMRI in patients with polysomnography-confirmed RBD and healthy controls (HCs), with a sufficiently large sample size in each group. METHODS: We analyzed rsfcMRI data from 50 RBD patients and 70 age-matched HCs. Although RBD patients showed no motor signs, some exhibited auton

Fragmentary myoclonus in idiopathic rapid eye movement sleep behaviour disorder.

Fragmentary myoclonus is a result of muscle activity consisting of brief potentials in surface electromyography during polysomnography. Excessive fragmentary myoclonus is defined by increased intensity of the potentials. A few studies report excessive fragmentary myoclonus occurrence in neurodegenerative diseases. Because idiopathic rapid eye movement sleep behaviour disorder is considered as an early stage of neurodegeneration with involvement of the brainstem, we charted the prevalence and quantified the intensity of excessive fragmentary myoclonus in idiopathic rapid eye movement sleep beha

Validation of the movement disorder society criteria for the diagnosis of 4-repeat tauopathies.

BACKGROUND: The Movement Disorder Society criteria for progressive supranuclear palsy introduced the category "probable 4-repeat (4R)-tauopathy" for joint clinical diagnosis of progressive supranuclear palsy and corticobasal degeneration. OBJECTIVES: To validate the accuracy of these clinical criteria for "probable 4R-tauopathy" to predict underlying 4R-tauopathy pathology. METHODS: Diagnostic accuracy for 4R-tauopathies according to the established criteria was estimated retrospectively in autopsy-confirmed patients with progressive supranuclear palsy and corticobasal degeneration (grouped
