Multiple Rib Stress Fractures.

In brief Stress fracture of the ribs, an uncommon overuse injury, may mimic recalcitrant intercostal or back muscle strain. This report describes a 36-year-old man who sustained multiple rib stress fractures after he took up golf. Fatigue of the serratus anterior muscle appeared to be the mechanism for his injury. Relative rest followed by strengthening exercises helped the patient return to pain-free activity.

Physiological and neurophysiological determinants of postcancer fatigue: design of a randomized controlled trial.

BACKGROUND: Postcancer fatigue is a frequently occurring, severe, and invalidating problem, impairing quality of life. Although it is possible to effectively treat postcancer fatigue with cognitive behaviour therapy, the nature of the underlying (neuro)physiology of postcancer fatigue remains unclear. Physiological aspects of fatigue include peripheral fatigue, originating in muscle or the neuromuscular junction; central fatigue, originating in nerves, spinal cord, and brain; and physical deconditioning, resulting from a decreased cardiopulmonary function. Studies on physiological aspects of p

GDE5/Gpcpd1 activity determines phosphatidylcholine composition in skeletal muscle and regulates contractile force in mice.

Glycerophosphocholine (GPC) is an important precursor for intracellular choline supply in phosphatidylcholine (PC) metabolism. GDE5/Gpcpd1 hydrolyzes GPC into choline and glycerol 3-phosphate; this study aimed to elucidate its physiological function in vivo. Heterozygous whole-body GDE5-deficient mice reveal a significant GPC accumulation across tissues, while homozygous whole-body knockout results in embryonic lethality. Skeletal muscle-specific GDE5 deletion (Gde5 skKO) exhibits reduced passive force and improved fatigue resistance in electrically stimulated gastrocnemius muscles in vivo. GD

Peripheral Levels of Selected Biomarkers in Patients with Post-Sarcoidosis Chronic Fatigue Syndrome.

INTRODUCTION: Chronic fatigue syndrome (CFS) is characterized by persistent fatigue and multiple symptoms such as cognitive impairment and muscle pain, often linked to immune-inflammatory dysfunction. Sarcoidosis, a granulomatous disease with systemic inflammation, commonly causes fatigue, even during remission. This study examined whether fatigue and depressive symptoms in sarcoidosis remission relate to residual inflammation or oxidative stress. Recent studies highlight parallels between post-infectious fatigue syndromes, including Long COVID, and sarcoidosis-related fatigue, emphasizing IL-

Longitudinal evaluation of fatigue in adult patients with spinal muscular atrophy and the impact of disease-modifying drugs.

Fatigue is a frequent and under-recognized symptom in adults with spinal muscular atrophy (SMA). Although motor scales can detect disease progression, they may be insufficiently sensitive over short observation periods, particularly in patients with slower progression. To assess the prevalence and longitudinal course of fatigue in adult SMA patients using validated instruments. General perceived fatigue was assessed using the Fatigue Severity Scale (FSS), while the Modified Fatigue Impact Scale (MFIS) was used to evaluate multidimensional fatigue-comprising the physical, cognitive, and psychoso

Effects of an 18-week exercise programme started early during breast cancer treatment: a randomised controlled trial.

BACKGROUND: Exercise started shortly after breast cancer diagnosis might prevent or diminish fatigue complaints. The Physical Activity during Cancer Treatment (PACT) study was designed to primarily examine the effects of an 18-week exercise intervention, offered in the daily clinical practice setting and starting within 6 weeks after diagnosis, on preventing an increase in fatigue. METHODS: This multi-centre controlled trial randomly assigned 204 breast cancer patients to usual care (n = 102) or supervised aerobic and resistance exercise (n = 102). By design, all patients received chemotherap

Muscle Strength Decline Caused by Eperisone Hydrochloride: A Case Report.

A 65-year-old female with thoracic spinal stenosis and incomplete paraplegia underwent T11-T12 posterior thoracic interbody fusion. During postoperative rehabilitation, she experienced thigh pain, involuntary lower limb convulsions, and muscle fatigue. Despite being prescribed eperisone hydrochloride for relief, her muscle strength decreased after 14 doses. This adverse effect, not listed in the latest Chinese medication instructions, subsided 4 days after discontinuation. This case suggests eperisone hydrochloride potentially caused reversible muscle strength decline, highlighting its potenti…

Inorganic nitrate supplementation and blood flow restricted exercise tolerance in post-menopausal women.

Exercise tolerance appears to benefit most from dietary nitrate (NO3-) supplementation when muscle oxygen (O2) availability is low. Using a double-blind, randomized cross-over design, we tested the hypothesis that acute NO3- supplementation would improve blood flow restricted exercise duration in post-menopausal women, a population with reduced endogenous nitric oxide bioavailability. Thirteen women (57-76 yr) performed rhythmic isometric handgrip contractions (10% MVC, 30 per min) during progressive forearm blood flow restriction (upper arm cuff gradually inflated 20 mmHg each min) on three s

Central excitability contributes to supramaximal volitional contractions in human incomplete spinal cord injury.

Despite greater muscle fatigue in individuals with spinal cord injury (SCI) when compared to neurologically intact subjects using neuromuscular electrical stimulation (NMES)protocols, few studies have investigated the extent of volitional fatigue in motor incomplete SCI. Using an established protocol of 20 repeated, intermittent, maximal volitional effort (MVE) contractions, we previously demonstrated that subjects with incomplete SCI unexpectedly demonstrated a 15% increase in peak knee extensor torques within the first five MVEs with minimal evidence of fatigue after 20 contraction. In the p…

Biomechanical changes in gait before and after fatigue in unilateral transtibial amputees.

PURPOSE: Fatigue can exacerbate gait abnormalities in unilateral transtibial amputees (TTA), but specific changes in walking patterns and compensatory strategies under fatigue remain unclear. This study aimed to investigate gait alterations in unilateral TTA following a fatigue-inducing protocol. METHODS: Ten male unilateral TTA and ten age-matched able-bodied controls underwent three-dimensional gait analysis at self-selected speed under non-fatigued and fatigued conditions. Spatiotemporal parameters, joint kinematics, and ground reaction forces were measured. Two-way ANOVA was used for stat

Current approach to seronegative myasthenia.

The group of patients with weakness or fatigue who have electrophysiological evidence of neuromuscular transmission defects, but no antibodies against either acetyl choline receptor or muscle specific kinase, need special evaluation and therapeutic consideration. Such seronegative patients may have low affinity antibodies but may also be late onset of a congenital myasthenic syndrome. This review discuss the practical approach toward the condition of seronegative myasthenia.

The influence of repetitive eye movements on vergence performance.

We measured the peak velocity of convergence eye movement responses in four normal subjects before and after a large number of either repetitive vergence or repetitive saccadic eye movements. A 20% decrease in the mean value of peak velocity was observed in vergence responses after 100 repetitive step vergence eye movements. However, 100 cycles of slow sinusoidal vergence tracking did not induce any notable change in vergence dynamics. Five hundred repetitive saccadic eye movements also caused an approximately 20% decrease in peak velocity. The reduction in peak velocity was related to the num

Nusinersen improves walking distance and reduces fatigue in later-onset spinal muscular atrophy.

INTRODUCTION: Ambulatory individuals with spinal muscular atrophy (SMA) experience muscle weakness, gait impairments, and fatigue that affect their walking ability. Improvements have been observed in motor function in children treated with nusinersen, but its impact on fatigue has not been studied. METHODS: Post hoc analyses were used to examine changes in 6-minute walk test (6MWT) distance and fatigue in children and adolescents with SMA type II and III who received their first dose of nusinersen in the phase Ib/IIa, open-label CS2 study and were ambulatory during CS2 or the extension study,

Relative effects of exercise training and alendronate treatment on skeletal muscle function of ovariectomized rats.

OBJECTIVE: Increased physical activity and bisphosphonate treatment are both effective in reducing the risk of fracture in postmenopausal women. Physical activity reduces the risk of fracture by improving both bone and skeletal muscle strength. The effects of bisphosphonates have primarily been evaluated on bone. In this study the authors investigated the combined effects of increased physical activity and bisphosphonate treatment on skeletal muscle function using an animal model of postmenopausal osteoporosis. DESIGN: Seven-month-old ovariectomized rats were assigned to one of four combinati
