[Cannabinoid and opioid system in the mechanisms and control of pain].

Opium and Hashish have been classically employed for the control of pain. The pharmacologic rationale for the use of these substances lies in the fact that they are able to modulate the endogenous opioid and cannabinoid systems respectively. Both systems, which depress the central nervous system (CNS), are capable of producing analgesia both in experimental animals and in humans by interfering with the transmission of pain signals (nociceptive) from the periphery to the superior centers of the CNS. We will review the main theories that explain the peripheral effects on both systems and its pos

Primary pachydermoperiostosis associated with pigmented villonodular synovitis: An unknown association?

Primary pachydermoperiostosis is a rare genetic disease affecting the skin and musculoskeletal system. In contrast to secondary hypertrophic osteoarthropathy, primary pachydermoperiostosis is considered a benign condition. While a variety of associated abnormalities have been described in this form, any association with tumors was previously reported in the literature. We hereby describe the first case of a 20-year-old man with primary pachydermoperiostosis revealed by a knee synovial tumor.

Osseous Manifestations of Primary Hyperparathyroidism: Imaging Findings.

Primary hyperparathyroidism is a systemic endocrine disease that has significant effects on bone remodeling through the action of parathyroid hormone on the musculoskeletal system. These findings are important as they can aid in distinguishing primary hyperparathyroidism from other forms of metabolic bone diseases and inform physicians regarding disease severity and complications. This pictorial essay compiles bone-imaging features with the aim of improving the diagnosis of skeletal involvement of primary hyperthyroidism.

Ultrasound as a Biomarker in Rheumatic Diseases.

Rheumatic diseases are a heterogeneous group of disorders which often affect the musculoskeletal system. Given the lack of definitive testing, there are limited diagnostic tools at clinicians' disposal. Over the recent decades, ultrasonography has gained widespread use within rheumatology due to its accessibility, safety, and relatively low cost. This review describes the clinical utility of ultrasound as a biomarker in the diagnosis and management of several rheumatic diseases.

Arthroscopic findings in the knee in nail-patella syndrome: a case report.

Nail-patella syndrome is a rare genetic disorder that can cause significant morbidity in several organs, including the musculoskeletal system. This case report describes our arthroscopic findings in the knees of an adolescent with bilateral hypoplastic and subluxed patellae. Our findings suggest that early intervention should be considered whenever this syndrome is recognized. This may modify some of the long-term joint problems seen in adults with nail-patella syndrome.

Marfan's syndrome in pregnancy: implications for advanced practice nurses.

Marfan's syndrome is a genetic disorder that often affects the cardiovascular, pulmonary, ocular, and musculoskeletal systems. When a woman with Marfan's syndrome becomes pregnant, the hormonal stresses and changes to the cardiovascular system can put the mother at risk for serious complications. Advanced practice nurses need to understand the maternal changes associated with Marfan's syndrome as well as the genetic factors involved in order to provide holistic care.

Diagnostic imaging of tarsal and metatarsal stress fractures. Part I.

As the preoccupation with physical fitness grows in this country, the problem of "stress fracture" has become more clinically significant. Reliable techniques for imaging the musculoskeletal system have also continued to grow, allowing the clinician to better diagnose and manage this condition. However, the radiographic findings of stress fracture are sometimes difficult to interpret. This review compares plain radiography, scintigraphy, CT, and MRI in the imaging of stress fractures.

Improving Reliability of MRI Lumbar Spinal Stenosis Assessment Across Radiology and Spine Specialties: Impact of a Structured Education Intervention.

RATIONALE AND OBJECTIVES: Magnetic Resonance Imaging (MRI) is the preferred modality for evaluating Lumbar Spinal Stenosis (LSS), yet grading remains subjective and inconsistent. Currently, no standardized reporting system, such as the American College of Radiology Reporting and Data systems (RADS), exists for LSS. The objective of this study is to assess the impact of a structured educational intervention using a classification system, on correctness and interobserver reliability of MRI LSS grading among radiology and non-radiology spine physicians. MATERIALS AND METHODS: Health Insurance Po

Pregnancy Characteristics and Outcomes among Women at Risk for Disability from Health Conditions Identified in Medical Claims.

BACKGROUND: Women with disabilities are at risk for poor birth outcomes. Little is known about specific potentially disabling health conditions and their effects on pregnancies. Using hospital claims, we identified women at risk for disability and evaluated the relationship between disability risk and demographic characteristics, pregnancy risks, and infant and maternal outcomes. METHODS: The 2006 through 2009 Massachusetts Pregnancy to Early Life Longitudinal data system linked birth certificate and hospital claims one year pre-pregnancy through delivery. Lumbo-Costovertebral Syndrome with Congenital Inferior Lumbar Hernia in a Female Infant.

Herein, we report a rare case of a 1 year old female who was diagnosed with congenital lumbar hernia along with vertebral, ribs, and abdominal wall anomalies suggestive of lumbo-costovertebral syndrome. It was also associated with musculoskeletal, genital, ocular, and cardiovascular defects. The patient presented with a palpable swelling in the lumbar region, which was initially suspected to be a simple hernia. Other apparent ocular and musculoskeletal findings revealed complex systemic anomalies that necessitated further investigation. Imaging studies, including ultrasonography and computed t

Case report: Pancreatitis-associated polyarthritis, panniculitis, and bone infarcts in a patient with alcohol use disorder.

Musculoskeletal complications secondary to pancreatitis, including polyarthritis, panniculitis, and bone infarction (Pancreatitis Panniculitis Polyarthritis syndrome), are rare and under-recognized. We report a case of a 31-year-old man with alcohol use disorder who initially presented with acute pancreatitis. Two months later, he developed migratory polyarthritis and subcutaneous nodules. Radiographs revealed permeative bone lesions, and MRI demonstrated multiple bone infarcts. Subcutaneous biopsy confirmed panniculitis. His clinical course was complicated by infected pancreatic necrosis and

[Topical NSAIDs: ineffective or undervalued?].

Topical NSAIDs are currently recommended in several national and international guidelines for knee osteoarthritis, hand osteoarthritis and acute musculoskeletal pain. However, there is still widespread skepticism about the effectiveness of this treatment. This article discusses different reasons for this skepticism, a short summary of the pharmacokinetics and pharmacodynamics of topical NSAIDs and an overview of available evidence regarding efficacy and safety. Based on this evidence topical NSAIDs have a clear place in the treatment of knee and hand osteoarthritis and acute musculoskeletal pa

Benign joint hypermobility syndrome: evaluation, diagnosis, and management.

Benign joint hypermobility syndrome (BJHS) is a connective tissue disorder with hypermobility in which musculoskeletal symptoms occur in the absence of systemic rheumatologic disease. Although BJHS has been well recognized in the rheumatology and orthopedic literature, it has not been discussed in the family medicine literature. Because most patients with musculoskeletal complaints are first seen by family physicians, it behooves primary care physicians to be familiar with recognizing and diagnosing BJHS. When patients with this syndrome are first seen by a physician, their chief complaint is

"Exploring The Nexus": Chronic musculoskeletal pain in diabetic vs non-diabetic population.

INTRODUCTION: Diabetes-mellitus (DM) has transcended the boundaries and affected populations across globe, it predisposes individual to stiffness and musculoskeletal-pain due to accumulation of glycation-end-products. Musculoskeletal-pain is a common yet frequently neglected complication. Pain mechanisms have been categorized as nociceptive, neuropathic, nociplastic, and idiopathic. Four criteria were put by Kosek-et-al to identify nociplastic pain that affects the musculoskeletal-system. Study aimed to evaluate prevalence of chronic musculoskeletal (cMSK) pain and its association with diabete
