Metformin-Loaded Schwann Cell Exosomes Reprogram Macrophages and Enhance Neurogenesis in Spinal Cord Injury Through PI3K/AKT Activation.

Spinal cord injury (SCI) creates a hostile microenvironment characterized by persistent inflammation and glial scarring, which severely limits endogenous neural regeneration. To address these multifactorial barriers, we developed a targeted nanotherapeutic system comprising glutathione-functionalized Schwann cell-derived exosomes loaded with metformin (Exos-GSH@Met). In vitro and in vivo evaluations showed that GSH functionalization enabled the exosomes to effectively cross the blood-spinal cord barrier and selectively accumulate in macrophages at the injury site. Transcriptomic sequencing ide

Multiplication of Compound Signals in Neural Array Signal Processing Improves Signal Quality and Detectability of Action Potentials.

Electrode arrays along peripheral nerves can record sensory, motor and autonomic signals propagating in afferent and efferent direction at different conduction velocities. To extract this temporal information in a delay-and-sum approach, velocity-selective recording (VSR) utilizes summation to amplify time-aligned action potentials and average out noise. This improved contrast subsequently improves the automated detection and classification of action potentials-an essential prerequisite to interface with the peripheral nervous system. In this study, we replaced the summation with multiplicatio

Electrospun polycaprolactone (PCL)-amnion nanofibrous membrane prevents adhesions and promotes nerve repair in a rat model of sciatic nerve compression.

Adhesion and scarring after neural surgery are detrimental to nerve regeneration and functional recovery. Amniotic membranes have been used in tissue repair due to their immunogenicity and richness in cytokines. In this study, an electrospun polycaprolactone (PCL)-amnion nanofibrous membrane was prepared for the treatment of sciatic nerve compression in a rat model. The effects of the PCL-amnion nanofibrous membrane on the prevention of adhesion formation and nerve regeneration were evaluated using electrophysiology and histological analyses. Compared with the medical chitosan hydrogel dressin

PGE1 improves diabetic peripheral neuropathy in patients with type 2 diabetes.

BACKGROUND: Diabetic peripheral neuropathy (DPN) is the most common chronic complication of diabetes. We aim to investigate the efficacy of Prostaglandin E1 (PGE1) treatment in addition to intensive insulin therapy on DPN in patients with type 2 diabetes. METHODS: Seventy-seven patients with DPN received daily intravenous injection of Prostaglandin E1 (PGE1) in lipid microspheres (Lipo-PGE1) for 10days as an additional therapy to standard glucose control therapy (PGE1 group). Another 42 patients with DPN receiving only standard glucose control therapy (intensive insulin therapy) acted as a co

The toxic oil syndrome.

Hundreds died and thousands were poisoned by rapeseed oil adulterated with aniline and sold illegally in Spain in 1981. The clinical manifestations, now known as the toxic oil syndrome, include pulmonary hypertension and right ventricular hypertrophy plus widespread vascular and neural lesions in other organs. Many of the late deaths ended with a scleroderma-like illness. Because scleroderma involves the heart, an examination was made of the small and large coronary arteries, the neural structures, and the conduction system from 11 victims dying with the toxic oil syndrome. Dense fibrosis, atr

Neurophysiologic study of beta-thalassemia patients.

Neurophysiologic investigations were performed in 34 Chinese patients with beta-thalassemia major maintained on long-term desferrioxamine treatment to look for subclinical toxicity in the auditory, visual, peripheral, or central neural pathways. In the auditory pathway study, four patients (12%) had mild sensorineural hearing impairment. Two patients (6%) had increased P 100 latencies in the visual evoked potential study, and nine patients (26%) had abnormal electroretinogram results. All had normal electrooculograms. Ophthalmoscopic examination was abnormal in three patients (9%), and three (

From adaptive to maladaptive myelination: White matter dynamics in cognition and neurologic disorders.

Myelin is a fatty substance formed by processes of mature oligodendrocytes, which wrap around neuronal axons. Previously, myelin was believed to be important solely for the rapid conduction of electrical signals. However, over the past decade, research has revealed that its role goes far beyond this, including providing metabolic support to neurons and exhibiting significant plasticity. Myelin has also been thought to play a role in the pathogenesis of several neurodegenerative diseases. Furthermore, myelin has been implicated in various psychiatric disorders, including anxiety, depression, an

Carpal tunnel release surgery restores force control but not force coordination in CTS patients.

Efficient manipulation of objects is crucial for daily interactions with the world. Compression neuropathies like carpal tunnel syndrome (CTS) can have detrimental effects on hand function, which can be improved by surgical intervention. Twelve patients with CTS grasped and lifted an instrumented handle while maintaining static equilibrium under different torque conditions. We compared fingertip forces and grip force variability across trials before and after surgery. Additionally, we evaluated the coordination of fingertip forces using a metric defined as synergy index to determine if there w

The red flags of ulnar neuropathy in leprosy.

The diagnosis of pure neural leprosy is more challenging because patients share characteristics with other common pathologies, such as ulnar compression, which should be taken into consideration for differential diagnosis. In this study, we identify ulnar nerve conduction characteristics to aid in the differential diagnosis of ulnar neuropathy (UN) in leprosy and that of non-leprosy etiology. In addition, we include putative markers to better understand the inflammatory process that may occur in the nerve. Data were extracted from a database of people affected by leprosy (leprosy group) diagno

Electropolymerized Poly(3,4-ethylenedioxythiophene) (PEDOT) Coatings for Implantable Deep-Brain-Stimulating Microelectrodes.

Conducting polymers have been widely explored as coating materials for metal electrodes to improve neural signal recording and stimulation because of their mixed electronic-ionic conduction and biocompatibility. In particular, the conducting polymer poly(3,4-ethylenedioxythiophene) (PEDOT) is one of the best candidates for biomedical applications due to its high conductivity and good electrochemical stability. Coating metal electrodes with PEDOT has shown to enhance the electrode's performance by decreasing the impedance and increasing the charge storage capacity. However, PEDOT-coated metal e

Fibromyalgia: Increased reactivity of the muscle membrane and a role of central regulation.

OBJECTIVE: Fibromyalgia (FM) is characterized by widespread muscle pain and central neural deregulation. Previous studies showed increased muscle fiber conduction velocity (CV) in non-painful muscles of FM patients. This study investigates the relationship between central activation and the CV in FM. METHODS: Twenty-two females with primary FM and 21 controls underwent surface electromyography of the non-painful biceps brachii. Mean CVs were calculated from the motor unit potential velocities (CV-MUPs), and the CV-MUPs' statistical distributions were presented as histograms. The amount of mus
