Feasibility and usability of a wearable sensor system for gait assessment in children with neuromuscular diseases.

INTRODUCTION: In children with Neuromuscular diseases (NMDs), gait monitoring is essential for evaluating motor function over time. The 10-meter walk test (10 mWT) and the 6 Minute Walking test (6 MWT) are commonly used timed tests. Wearable inertial measurement units (IMUs) have recently gained increasing interest and use for gait assessment. The primary objective of the study is to verify the technical feasibility and clinical usability of IMUs in children with NMDs during standard 10 and 6 MWT. Secondly, the authors aimed to investigate the agreement between the results of manual and device

Are We Getting What We Ask For: Few Patients Meet Mean Arterial Pressure (MAP) Goals After Intraoperative Neuromonitoring (IONM) Changes During Pediatric Spine Surgery.

BACKGROUND: Hypotension is a critical, modifiable risk factor for neurological deficit in spine surgery. Patients with intraoperative neurophysiological monitoring (IONM) changes are particularly vulnerable, as their spinal cord has already demonstrated sensitivity to correction, and hypotension may exacerbate areas of inadequate perfusion. These patients are admitted postoperatively to the ICU for close neurological and hemodynamic monitoring. This study evaluated adherence to patient-specific mean arterial pressure (MAP) goals in the first 24 hours following IONM changes. METHODS: A retrosp

Status epilepticus. A perspective from the neuroscience intensive care unit.

Patients with GCSE and NCSE are common and may present to the emergency department or the NICU. In the NICU, NCSE is a more common presentation than GCSE. In the emergency department, GCSE commonly evolves to NCSE, either as a late sequela of prolonged SE or due to partial treatment with antiepileptic medication or neuromuscular blocking agents. In the emergency department, acute cerebral injuries are commonly found in patients presenting with SE, regardless of whether they have preexisting epilepsy. In the NICU, almost by definition, SE occurs in patients with acute cerebral injuries. Status

Vocal cord paralysis.

The information presented in this article demonstrates that unilateral or bilateral vocal cord paresis or paralysis in infants and children is difficult to diagnose and difficult to manage. In an attempt to provide the otolaryngologist with a concise set of relevant guidelines, the following rules for management are presented here. 1. Suspect bilateral abductor vocal cord paralysis (BAVP) when a neonate or infant presents with high-pitched inspiratory stridor and evidence of airway compromise. Factors that should increase the suspicion of BAVP include associated Arnold-Chiari malformation; con

Botulinum toxin assessment, intervention and aftercare for lower limb disorders of movement and muscle tone in adults: international consensus statement.

Lower limb disorders of movement and muscle tone in adults significantly impact quality of life. The management of the patient with hypertonia is complex and requires a multidisciplinary team working with the patient and family/carers. Botulinum neurotoxin type A (BoNT-A) has been used as a component of this management to reduce lower limb hypertonia, increase passive range of motion and reduce associated pain and requirements for bracing. Adjunctive treatments to augment the effect of BoNT-A include electrical muscle stimulation of the injected muscles and stretching. When determining suitabi

[Intraoperative identification of the recurrent laryngeal nerves in thyroid surgery].

Accidental injury to the recurrent laryngeal nerve is a common complication of thyroid surgery. Different mechanisms of injury have been described, such as mechanical, electrical, or thermal. Retrosternal operations, secondary bleeding requiring wound revision, or neoplastic tumors are also frequent causes of this complication. Intraoperative direct stimulation of suspected nerve structures as well as vagal nerve stimulation have been proposed to avoid the injury. The effect of stimulation can be assessed by direct observation of the vocal cords (direct laryngoscopy or fiberoscopy via a laryng

Quantitative ultrasound assessment of lower limb muscle morphology in type 2 diabetes with and without peripheral neuropathy.

BACKGROUND: Diabetic peripheral neuropathy (DPN) is associated with progressive skeletal muscle degeneration, particularly in distal lower limb muscles. Advanced imaging modalities such as CT and MRI can detect these changes; however, their routine clinical use is limited. This study aimed to evaluate the utility of quantitative musculoskeletal ultrasound for assessing lower limb muscle morphology in individuals with type 2 diabetes with and without DPN. METHODS: In this cross-sectional study, 215 participants were included and categorized into three groups: T2DM with DPN (n = 129), T2DM with

Weaning From Mechanical Ventilation in Cardiac Surgery Patients: Current Strategies, Monitoring Innovations, and Future Perspectives.

Weaning from mechanical ventilation after cardiac surgery is a dynamic, multifaceted process that extends beyond respiratory function alone. While most patients experience timely extubation, others face delayed or failed weaning due to the complex interplay of perioperative factors. This review provides an integrative overview of the weaning process, beginning with key definitions and classifications specific to the cardiothoracic population. It then explores the underlying pathophysiological determinants, including cardiopulmonary interactions, impaired lung mechanics, right heart failure, an

Respiratory function in Becker muscular dystrophy: a comprehensive longitudinal study.

BACKGROUND: Becker muscular dystrophy (BMD) is a rare X-linked neuromuscular disorder predominantly affecting males. While respiratory function has been characterised in small cohorts over limited time frames, comprehensive long-term longitudinal data remain lacking, as do established care guidelines to inform respiratory surveillance in BMD. METHODS: In this retrospective analysis, we present a large longitudinal analysis of respiratory function in patients with BMD, based on 1360 spirometry measurements from a single-centre cohort of 152 patients. Analysed assessments spanned paediatric to

Reachable Workspace as a Clinical Outcome for Upper Extremity Function: A Narrative Review.

Motion sensing technology can be utilized to capture detailed upper extremity (UE) motion to reconstruct an individual's three-dimensional (3D) reachable workspace (RWS). The RWS can be quantified as relative surface area (RSA), providing an innovative surrogate measure to assess UE mobility and function. Numerous studies have shown that RSA can reliably distinguish between healthy and impaired UE function and also is able to detect clinically relevant longitudinal changes, with sufficient sensitivity to detect small differences even in slowly-progressive conditions. After more than a decade o

Successful Management of Open Depressed Skull Fracture with Superior Sagittal Sinus Injury and Refractory Intracranial Hypertension.

Traumatic dural venous sinus injury is uncommon but potentially fatal because of massive hemorrhage at presentation or during surgery, and intracranial hypertension caused by impaired venous outflow. We report a case of a young woman with an open depressed skull fracture over the vertex caused by repeated blunt assault. Computed tomography revealed a large midline depressed fracture with bony fragments penetrating the superior sagittal sinus. On arrival, the patient was in hemorrhagic shock, with a semicoma status and right hemiparesis. Emergency surgery was performed for hemostasis and wound
