A case of nonvisualization of the gallbladder in an adult patient with cystic fibrosis in the absence of acute cholecystitis is reported. Delayed images to 20 hours showed persistent nonvisualization. Review of the literature on cystic fibrosis suggests that nonvisualization of the gallbladder may be secondary to inspissated mucus rather than acute cholecystitis, and therefore a positive hepatobiliary scan in these patients should be interpreted with caution.

Meningiomas are usually benign solitary intracranial tumors. In rare cases, atypical [World Health Organization (WHO) grade II) and anaplastic (WHO grade III) meningiomas can lead to distant metastases. Von Hippel-Lindau disease is a rare multisystem disorder that shows autosomal dominant inheritance. It is a cancer syndrome that is characterized by the development of a variety of benign and malignant tumors-CNS hemangioblastomas, retinal angiomas, endolymphatic sac tumors, renal cysts and tumors, pancreatic cysts and tumors, adrenal pheochromocytomas, and epididymal cystadenomas. Hypokalemia due to loss of potassium through the kidneys can be caused by distal Renal Tubular Acidosis (dRTA). Radiation-induced thyroid dysfunction is very rarely associated with peptide receptor radionuclide therapy (PRRT) for neuroendocrine tumors using β-emitting radionuclides. With the emergence of α radionuclide such as 225Ac, PRRT using these radionuclides has shown better results; however, data on the toxicity profile are limited. This is a case report describing the thyroid dysfunction developed in a patient with inoperable metastatic neuroendocrine tumor with unknown primary after PRRT using 225Ac-DOTATATE, which has never been reported previously.

Lutetium-177 ( 177 Lu) dotatate has been offered at Fox Chase Cancer Center since 2017 as part of a clinical trial and then in 2018 as a commercially available cancer therapy, and we thought we were prepared for most 177 Lu issues by the fall of 2020.
