Comparison of antimicrobial efficacy of Calcipex and Metapex in endodontic treatment of chronic apical periodontitis: a randomised controlled trial study protocol.

INTRODUCTION: Various intracanal medicaments have been used in cases of chronic apical periodontitis for appropriate disinfection of the root canal system to eliminate microbes especially from the inaccessible areas. Calcium hydroxide is the most common intracanal medicament available in various forms, but its effectiveness with or without iodoform using microbial culture is unknown. Therefore, our aim is to compare the antimicrobial efficacy of Calcipex and Metapex in endodontic treatment of teeth presenting with chronic apical periodontitis by assessing the bacterial load reduction. Analysis of Electronic Apex Locators in Human Teeth Diagnosed With Apical Periodontitis.

Electronic apex locators (EAL) have been used to establish the working length (WL) in root canal treatment. In teeth diagnosed with apical periodontitis, resorption of tooth apical structures can lead to difficulties to obtain an appropriate WL. The aim was to compare the capacity of three EAL's (Root ZX II, Raypex 6 and Endo-Eze Quill) to locate the tip of the K-file between 0 to -0.5 mm from the apical foramen (AF) on teeth diagnosed with asymptomatic apical periodontitis (AAP). Electronic working length was performed on 60 roots with AAP. A K-file #15 was inserted in the root canal until th

Dental pulp exposure, periapical inflammation and suppurative osteomyelitis of the jaws in juvenile Baltic grey seals (Halichoerus grypus grypus) from the late 19th century.

The systematic analysis of museum collections can provide important insights into the dental and skeletal pathology of wild mammals. Here we present a previously unreported type of dental defect and related skull pathology in five juvenile Baltic grey seals that had been collected in the course of a seal culling program along the Danish coast in 1889 and 1890. All five skulls exhibited openings into the pulp cavities at the crown tips of all (four animals) or two (one animal) canines as well as several incisors and (in one animal) also some anterior premolars. The affected teeth showed wide pu

Utilizing the GentleWave® System for Debridement of Undetected Apical Anatomy.

AIM AND BACKGROUND: Debriding and disinfecting complex anatomies within the root canal system pose a major challenge during root canal therapy. Even with current chemomechanical techniques, debris and bacterial remnants are commonly left behind, which are generally believed to increase the risk of endodontic failure. This case details the use of a new technique to debride complex apical anatomy in a maxillary molar. CASE REPORT: A 48-year-old female presented to the clinic with a chief complaint of increasing pain in her tooth. Clinical examination of the right first maxillary molar (#3) reve

The endodontic diagnostic puzzle.

Endodontic diagnosis is the cornerstone of endodontic treatment. Endodontic diagnosis can be likened to a puzzle, where the pieces must be gathered and pieced together before a clinician can see the complete picture. This article discusses how to collect the pieces and fit them together to see the pulpal and periapical diagnosis emerge.

Effects of dentin on the antimicrobial properties of endodontic medicaments.

Successful treatment of apical periodontitis is dependent on the elimination of the infective microflora from the necrotic root canal system. Antimicrobial irrigating solutions and other locally used disinfecting agents and medicaments have a key role in the eradication of the microbes. While most if not all presently used disinfecting agents rapidly kill even the resistant microbes when tested in vitro in a test tube, the effectiveness of the same agents is clearly weaker in the in vivo conditions. Recent studies have given valuable information about the interaction of endodontic disinfecting

[Study of the chronic apical periodontitis influence upon the patient's organism status].

During clinical-immunological study of the patients with chronic apical periodontitis (CAP) some differences were found between the following categories--CAP involving not more than 2 teeth and plural CAP. There were found the changes of both general immunity indices and its local components. Chronic inflammation course in periodontal tissues led to imbalance between cellular and humoral local immune reactions. It was shown that the presence of protracted multiple infectious focuses in periodontal tissues led to organism sensitizing and secondary immune deficiency.

Mandibular second molar with C-shaped canal morphology and five canals: report of a case.

This article reviews a case involving endodontic treatment of a mandibular second molar with C-shaped canal morphology and five separate canals. The C-shaped canal variation of morphology is unusual and can lead to difficulties during treatment. Preoperative radiographs are limited in their ability to detect this entity; for a favorable treatment outcome, knowledge of the C-shaped canal's existence and morphology is essential.

Mediastinitis from odontogenic infection. Report of three cases and review of the literature.

Descending necrotizing mediastinitis secondary to dental infection occurs infrequently. The diagnosis of this condition is difficult and often a surgical approach is delayed due to initial clinical improvement after antimicrobial therapy. An incorrect evaluation of this apparent improvement may result in fatal mediastinitis and septic shock. We report 3 cases of mediastinitis of odontogenic origin. In one patient, a nonproductive cough was the first sign of thoracic involvement. A total of 25 similar cases of mediastinitis from odontogenic infection have been collected from the literature in t

Local anesthesia failure in endodontic therapy: the acute inflammation factor.

The attainment of clinically acceptable local anesthesia is often problematic in the treatment of symptomatic pulpitis and symptomatic apical periodontitis. This article reviews the literature to identify the extent of this problem and to understand the organic components involved with this condition. Inflammation, infection, pH changes, alteration in nerve cell membranes, and anatomic variations are all possible causes for failure of local anesthesia during emergency endodontic therapy.

Acute pulpal-alveolar cellulitis syndrome. IV. Exacerbations during endodontic treatment: a clinical study of specific microbial isolates and their etiologic role. Part 1.

A statement has been made by Sundqvist that it has not been possible to isolate the specific bacteria that is responsible for acute exacerbations during endodontic treatment of existing periapical inflammation. This clinical study presents specific microbial isolates from 34 intact nonvital teeth involved with acute cellulitis exacerbations during endodontic therapy. No obligate anaerobes were isolated. Forty-seven aerobic and facultative microbes were isolated. Streptococci, primarily facultative, were the major group of microbes isolated (80%); alpha hemolytic streptococci and the group D en

Orofacial infections of odontogenic origin.

Purulent infections of the orofacial region are often of odontogenic origin. The clinical manifestations are largely dependent on the complex microbiota and the anatomical routes of spread. Serious complications may ensue, especially if hematogenous spread occurs. Direct invasion of different anatomical spaces may cause mediastinitis, airway obstruction and intracranial spread. Proper surgical treatment is mandatory to evacuate pus as soon as abscess formation is recognized. Anaerobic bacteria belonging to the normal oropharyngeal flora are usually isolated from these abscesses. The drug of ch

[The extraoral fistula (author's transl)].

Extraoral fistulas, whether primary or as the residue of an abscess, have different causes. According to case reports in the literature and from clinical experience the dentogenous cause is the more frequent. For this reason a possible tooth involvement should be ruled out before therapy, loss of dental pulp of various origin and periodontal diseases should be excluded, otherwise the treatment must begin with the cause, after which further intervention is usually superfluous.

Immediate implant placement and loading in the esthetic area when the buccal socket wall is significantly damaged.

OBJECTIVES: Recent clinical recommendations contraindicate immediate implant placement when the socket buccal bone plate is significantly damaged. The connective tissue graft (CTG) is increasingly being used in implant therapy and can replace periodontal defects lacking bone wall in periodontal regenerative surgery. Therefore, CTG could be used to allow immediate implant placement and loading even when the buccal socket wall is damaged, facilitating graft material stability. CLINICAL CONSIDERATIONS: In the first case, deep bone dehiscence was caused by a vertical root fracture. In the second

The impact of oral health on physical fitness: A systematic review.

BACKGROUND: Oral health problems may have numerous effects on general health, including physical fitness and performance. In this review, we aimed to systematically review the available evidence to assess the effect of oral health on general physical fitness. METHODS: We systematically performed a literature search in two different databases (PUBMED and EMBASE) without restriction to the year of publication. Articles were included if the subjects were humans and the primary aim was to assess the effects of dental and/or oral health problems on physical activity using either objective physical

Juvenile Spongiotic Gingival Hyperplasia: Clinicopathological Features of Eight Cases and a Literature Review.

BACKGROUND: Juvenile spongiotic gingival hyperplasia (JSGH) is a benign proliferation of non-keratinised stratified squamous epithelium with evident spongiosis, exocytosis leukocytes and dilated vessels with varying numbers of inflammatory cells. Although uncertain, it has been suggested that the epithelial proliferation is probably related to trauma and local irritants. It emerges as a painless erythematous patch or papule with an irregular surface. CASE REPORTS: We present a series of eight cases and a literature review of the clinical presentation and treatment approaches for JSGH. This se
