'There was no other way things could have been.' Greenlandic women's experiences of referral and transfer during pregnancy.

The World Health Organization (WHO) has stated that referral practices along with midwifery care are a means of heightening the quality of perinatal care and lessening perinatal mortality and morbidity. In 2002, in response to high perinatal mortality and morbidity, a referral system was instituted nationally in Greenland, transferring all at-risk pregnancies to its national referral hospital. Little or no current research has focused on evaluation of the perinatal referral system or on the thoughts, beliefs, opinions and challenges faced by women and Greenlandic families themselves. The aim o…

Maternal and perinatal outcome after expectant management of the HELLP syndrome compared with pre-eclampsia without HELLP syndrome.

OBJECTIVE: To compare maternal and perinatal outcome of pregnancies complicated by pregnancy induced hypertension and HELLP syndrome with th … ational age on admission and antihypertensive treatment on admission as significant contributors to perinatal mortality or major handicap. Whether the patient belonged to the HELLP group or the pre-eclamptic group had no influence on outcome. CONCLUSION: Expectant management of pregnancy induced hypertension with HELLP syndrome and pre-eclampsia without HELLP syndro …

Perinatal injury of the central nervous system in Lithuania from 1997 to 2014.

BACKGROUND: Perinatal CNS injuries are significant for the health of neonates and for child development at a later period. The aim of this study was to evaluate the dynamics of the frequency of perinatal CNS lesions (corresponding to ICD 10 code P91) over a 20-year period, using the data collected from the Lithuanian Medical Data of Births (Registry of Births). MATERIAL AND METHODS: In total, data of 559,164 newborns were analyzed. RESULTS: During the period from 1997 to 2014, the frequency of term newborns with perinatal CNS injury decreased almost two times, from 20.4/1000 live births in 1

Recurrence rate and outcome of postterm pregnancy, a national cohort study.

OBJECTIVE: To assess the recurrence rate of postterm delivery (gestational age at or beyond 42+0 weeks or 294 days) and to describe maternal … d without a postterm delivery in their first pregnancy. We compared perinatal outcome (composite of perinatal mortality, Apgar score <7 and birth injury) and adverse maternal outcome (composite of maternal death, abruptio placentae, PPH>1000ml and blood transfusions) between women with a recurrent and a de novo postterm second pregnancy. RESULTS: Our study populatio …

Optimizing Psychological Health Across the Perinatal Period: An Update on Maternal Cardiovascular Health: A Scientific Statement From the American Heart Association.

Perinatal psychological health conditions (eg, perinatal depression, anxiety) are some of the leading causes of maternal mortality in the United States and are associated with adverse pregnancy outcomes, long-term cardiovascular outcomes, and intergenerational effects on offspring neurodevelopment. These risks underscore the importance of addressing maternal psychological health as a key determinant of perinatal cardiovascular health. Thus, it is vital to recognize the spectrum of perinatal psychological health and to provide guidance for both patients and clinicians on screening and managemen

Pregnancy outcomes following ovarian tissue cryopreservation: an Australian cohort study.

BACKGROUND: Ovarian tissue cryopreservation is an increasingly utilized fertility preservation method in young women, with this technique being largely implemented across Europe and Australia. Australia is a pioneer of ovarian tissue cryopreservation, but the clinical perinatal outcomes are currently unknown for the Australian population. These outcomes are important as they contribute to the global understanding of which patients can benefit from this fertility preservation option. OBJECTIVE: To examine the pregnancy and mortality outcomes of patients who have undergone ovarian tissue cryopr

[Precarious situation of obstetric practice in Gabon].

In 1993, 2,008 deliveries were recorded at the Provincial Maternity Hospital at Franceville in Gabon. The frequency of cesarean section was 3%. The perinatal mortality of children born by cesaraean section was high, 213 per thousand. The principal indications for cesarean section were the baby being too large to pass through the pelvis, bicicatricial uterus, breech presentation and toxemia during pregnancy. The maternal mortality rate was 200 per 100,000, similar to the rates of most African countries, and 75% of the women that died had undergone cesarean section. The mortality rate for cesare…

[The organisation of perinatal care in a rural area. An enquiry into the Limousin Region (author's transl)].

Improvement in mortality and perinatal morbidity in rural areas is the result of: The appointment of qualified obstetricians. The provision of modern methods of obstetrical monitoring. The fact that "'avoidable" mortality and morbidity persist in these same rural districts is related to: Premature deliveries and the difficulties of transporting the premature babies. The survival of "country deliveries". We propose three solutions to improve perinatal care in these regions: Transformation of rural maternity units which are under-equipped into Centres where pregnancy can be monitored properly. A

Plasma-urate measurements in predicting fetal death in hypertensive pregnancy.

The relation of perinatal mortality to plasma-urate concentrations and blood-pressure was studied in 332 pregnant patients with hypertension. Perinatal mortality was markedly increased when maternal plasma-urate concentrations were raised, generally in association with severe pre-eclampsia of early onset. Plasma-urate was a better indicator than blood-pressure of prognosis for the fetus. Maternal hypertension, even severe, without hyperuricaemia, was associated with an excellent prognosis for the fetus. Conversely, when maternal hypertension was mild and hyperuricaemia was severe, the prognosi…

Umbilical artery velocimetry as a predictor of adverse outcome in pregnancies complicated by oligohydramnios.

Oligohydramnios is associated with increased perinatal mortality and morbidity. Traditional methods of in utero fetal evaluation have been unsuccessful in reliably distinguishing oligohydramnios with normal outcome from that associated with increased perinatal morbidity. A prospective study was undertaken to establish the predictive value of Doppler velocimetry in identifying the fetus with oligohydramnios at increased risk of adverse perinatal outcome. Twenty-two gravid women with subjective oligohydramnios on ultrasound had continuous-wave umbilical artery velocimetry. Subjects were divided …

Predictive value of doppler cerebroplacental ratio for adverse perinatal outcomes in postdate pregnancies in Northwestern Nigeria.

BACKGROUND: Postdate pregnancy is a very common obstetric condition, increasing the risk of perinatal morbidity and mortality from uteroplacental insufficiency. AIM: To determine the predictive values of Doppler cerebroplacental ratio (CPR), that is, the ratio between middle cerebral artery (MCA) and UA pulsatility indices (UA PI), and other potential velocimetric predictors of adverse perinatal outcomes in women with postdated pregnancies. PATIENTS AND METHODS: A prospective cohort study was conducted on consenting 212 low-risk pregnant women beyond 40 weeks' gestational age. The pulsatilit

Prenatal detection and evaluation of an extralobar pulmonary sequestration in the posterior mediastinum.

Extralobar pulmonary sequestration (EPS) is a rare developmental anomaly with aberrant nonfunctioning parenchymal tissue, associated with an increased risk of perinatal morbidity and, rarely, mortality owing to possible neonatal respiratory distress. In most cases supernumerary lobes are detected as isolated intra- or extrapleural lesions with independent systemic arterial blood supply. We report an atypical case of prenatal detection and perinatal outcome of a mediastinal EPS.

A Case of Miliary Perinatal Tuberculosis in a Preterm Newborn Infant Presenting as Peritonitis.

Perinatal tuberculosis (TB) is a rare disease with nonspecific clinical symptoms that can result in delayed treatment, and contribute to high morbidity and mortality. We report a case of perinatal TB in a 25-day-old newborn who presented with fever, respiratory distress, disseminated intravascular coagulation (DIC), and marked abdominal distension with hepatosplenomegaly. Further workup revealed culture negative sepsis, peritonitis, and diffuse nodular infiltrate in the lungs, liver, and spleen. After an extensive diagnostic workup for potential gastrointestinal causes of sepsis, the diagnosis

Development and implementation of an entity relationship diagram for perinatal data.

OBJECTIVE: Severe maternal morbidity and mortality are higher in the United States than in other high-income countries, and unacceptable disparities persist. To facilitate research on these outcomes, we developed a standardized approach for extracting perinatal data from electronic health records (EHRs). MATERIALS AND METHODS: To support data model building and validation, we harmonized perinatal EHR data in a common data model, building on lessons learned from multiple prior projects. RESULTS: We developed an Entity Relationship Diagram (ERD) that aggregates perinatal EHR data at appropriat

[Fetal and neonatal mortality from 22 weeks of amenorrhea in the Loire area].

BACKGROUND: The Pays de Loire has a low perinatal mortality indicators among French regions but this could be due to under-notification. OBJECTS: To explore this hypothesis we undertook a survey in order to identify all fetal and neonatal deaths occurring at a gestionnal age of 22 weeks or more. We also tried to examine and analyze the causes of death. METHODS: All maternity (26) and neonatal wards (5) in the region took part in the survey in 1995. Clinicians were asked to fill out a questionnaire for all deaths occurring from gestational age (GA) 22 weeks and/or concerning a birthweight of …

Temporal Trend, Causes, and Timing of Neonatal Mortality of Moderate and Late Preterm Infants in São Paulo State, Brazil: A Population-Based Study.

Moderate and late preterm newborns comprise around 85% of live births < 37 weeks gestation. Data on their neonatal mortality in middle-income countries is limited. This study aims to analyze the temporal trend, causes and timing of neonatal mortality of infants with 320/7-366/7 weeks gestation without congenital anomalies from 2004-2015 in the population of São Paulo State, Brazil. A database was built by deterministic linkage of birth and death certificates. Causes of death were classified by ICD-10 codes. Among 7,317,611 live births in the period, there were 545,606 infants with 320/7-366/7
