Meckel's diverticulum, a congenital gastrointestinal malformation, is seldom detected in the neonatal age group. Here, we present a neonate who presented to us with respiratory distress and acute abdomen. An X-ray showed pneumoperitoneum. Exploratory laparotomy revealed perforation at the tip of Meckel's diverticulum with faecal peritonitis. Wedge resection and anastomosis were performed. PURPOSE: Gallstone ileus is an uncommon complication of gallstone disease. It occurs in elderly patients in up to 25% of the cases. The management of gallstone ileus remains controversial. Open surgery has been the mainstay of treatment, but laparoscopic surgery has recently been used. The standard treatment of congenital diaphragmatic hernia is by open operation. We have successfully performed a laparoscopic repair in an asymptomatic 2-year-old child. We recommend this as a safe alternative treatment modality in diaphragmatic hernias presenting past infancy. The role of laparoscopic correction in the newborn period, however, remains to be determined.

Imaging of necrotizing enterocolitis is frustrating, with no single imaging test that is both sensitive and specific for the diagnosis. A technique using direct trocar insertion for laparoscopy was introduced into the Tripler Army Medical Center Obstetrics and Gynecology training program in September 1982. From January 1, 1983 to December 31, 1985, a total of 1249 laparoscopic procedures were performed. Detailed records of 937 laparoscopic procedures performed using this technique were reviewed. The technique was used with increasing frequency: 40% in 1983, 85% in 1984, and 96% of all laparoscopies in 1985. There were no major complications. Two cases of perforated retrocecal appendicitis presenting with the roentgen finding of an air containing subhepatic abscess are discussed. Perineal trauma in children is rare but constitutes a surgical emergency due to the risk of deep injuries, particularly digestive lesions. We report the case of a 14-year-old boy, with no significant medical history, admitted with a perineal wound extending toward the anal margin. Clinical examination revealed low-grade fever (38°C) and a soft but tender abdomen. Abdominal radiography showed extensive pneumoperitoneum, suggesting digestive perforation.
