Subacute sclerosing panencephalitis with pontine involvement.

In subacute sclerosing panencephalitis, brainstem involvement is rare. This paper reports an 11-year-old boy with the disease with prominent pontine involvement. Diffusion magnetic resonance imaging had superior information with respect to involvement of the nerve fibers. In the ventral part of the pons, the regions containing the longitudinal pontine nerve bundles were spared bilaterally and symmetrically. The pontine tegmentum was also spared. High signal in the affected regions was revealed by b = 1000 s/mm2 images. The apparent diffusion coefficient values in these regions were low, rangin…

Spontaneous resolution of a prepontine arachnoid cyst. Case report and review of the literature.

Prepontine arachnoid cysts are rare developmental anomalies that occur almost exclusively in children. The symptomatic child typically suffers from hydrocephalus, visual impairment, endocrine dysfunction and/or cranial neuropathies. Some cysts, however, are discovered incidentally upon prenatal or postnatal imaging for other indications. While there is little doubt that surgical treatment should be initiated to help the symptomatic child, appropriate therapy for the asymptomatic patient is unclear. Although arachnoid cysts are often managed conservatively using serial imaging, the consequences

[Acquired pendular nystagmus after pontine hemorrhage].

A 60-year-old hypertensive woman had a pontine hemorrhage that caused slight right hemiplegia, deep sensory disturbance on her right side an … vertical and horizontal optokinetic nystagmus were absent. Caloric stimulation did not evoke any responses bilaterally. There were no rhythmical movements at similar frequencies in other parts of the body such as palatal myoclonus. MRI revealed not only hematoma mainly at the dorsal pontine tegmentum but also hypertrophy of the inferior olive nucleus, …

Endoscopic observation of a slit-valve mechanism in a suprasellar prepontine arachnoid cyst: case report.

OBJECTIVE AND IMPORTANCE: There are several theories regarding the genesis of arachnoid cysts. However, controversy continues over what mech … ed. During inspection of the cyst, we clearly observed a slit-valve mechanism that was obviously responsible for the formation and enlargement of the cyst. The valve was formed by an arachnoid membrane surrounding the basilar artery. Synchronous with pulsation, the valve opened and closed rapidly. CONCLUSION: A slit-valve mechanism seems to be at leas …

Organization of the pontine nuclei.

The pontine nuclei provide the cerebellar hemispheres with the majority of their mossy fiber afferents, and receive their main input from the cerebral cortex. Even though the vast majority of pontine neurons send their axons to the cerebellar cortex, and are contacted monosynaptically by (glutamatergic) corticopontine fibers, the information-processing taking place is not well understood. In addition to typical projection neurons, the pontine nuclei contain putative GABA-ergic interneurons and complex synaptic arrangements. The corticopontine projection is characterized by a precise but highly

Incisor growth acceleration after lesions in the lateral pontine reticular formation of rats.

The length of upper and lower incisors of Long-Evans hooded rats between gingiva and top were measured before and after small bilateral symmetric electrolytic lesions of the pontine reticular formation between the ventromedial parabrachial nucleus and the motor trigeminal nucleus. Ten days after lesions of this region 8.5 mm posterior to bregma the length of upper incisors was 13.4 +/- 0.7 mm compared with 6.1 +/- 0.2 mm before lesion (p less than 0.01) and the length of the lower incisors 17.1 +/- 0.4 mm compared with 10.2 +/- 0.2 mm before lesion (p less than 0.01). A second group with a sma

[An autopsy case of progressive supranuclear palsy with central pontine myelinolysis].

An autopsy case of progressive supranuclear palsy (PSP) associated with central pontine myelinolysis (CPM) is reported. A 73-year-old male p … globus pallidus, subthalamic nucleus, substantia nigra and dentate nucleus, and at the base of the pons, bilateral and symmetrical demyelination was found. In addition, myelin staining revealed circumscribed pallor in the cerebral white matter. The histologic diagnosis was PSP associated with CPM. An association of PSP with CPM is rare in the elderly …

Cortical asymmetry of REM sleep EEG following unilateral pontine hemorrhage.

A 24-year-old woman with a left pontine hematoma showed marked asymmetry in the EEG of REM sleep, suggesting that a unilateral pontine lesion is sufficient to disrupt normal REM sleep EEG in the ipsilateral hemisphere. Other REM sleep characteristics (rapid eye movements, muscle atonia) were unaffected by this lesion.

Pure motor hemiplegia due to meningovascular neurosyphilis.

Two young male homosexuals developed prodromal syndrome followed by penicillin-responsive meningitis and the acute onset of pure motor hemiplegia. The clinical and laboratory features are consistent with meningovascular neurosyphilis. Basis pontis infarctions were subsequently demonstrated on magnetic resonance imaging scans. To our knowledge, this is the first description of syphilitic arteritis as a pathophysiologic basis for pure motor hemiplegia.

[The double pontine lesion of Fisher].

C. Miller Fisher described separately two pontine syndromes, that he called "the one-and-a-half syndrome" and "ataxic hemiparesis". The authors report the case of a woman who developed simultaneously the two syndromes, caused by a stroke of pontine localization, and comment about the structures involved by the lesion.

Cryptic disseminated tuberculosis presenting as gaze palsy.

We present a case of gaze palsy caused by a pontine tuberculoma associated with cryptic disseminated tuberculosis. The only symptoms in this patient were those caused by her gaze abnormality despite the presence of disseminated tuberculosis that was later confirmed at autopsy.

[Central pontine myelinolysis--pathogenesis and review of the literature].

A 77-year-old, nonalcoholic man was admitted to the Omiya Red Cross Hospital with the complaint of fever and delirium state of two days' duration. Two months prior to admission he had had cough and sputum. Chest X-ray revealed honey comb lungs. Cultures of sputum revealed mycobacterium tuberculosis after eight weeks incubation. He had no liver disease in his past history. The patient appeared cachetic. His vital signs were as follows; temperature 38 degrees C, blood pressure 132/68 mmHg, with a pulse rate of 84/min. He was delirium and excited. Findings of the cranial, motor and sensory nerve

[Morphology of primary and secondary hemorrhages into the brain stem].

On the basis of morphological examinations of 5 cases of primary and 15 cases of secondary hemorrhages to the brain stem histological characteristics of those hemorrhages are presented. A high frequency of preceding disorders of the brain stem circulation in the forms of infarctions at various stages of development, perivascular hemorrhages and cysts due to changes (mostly of hypertensive origin) in the cerebral arterie is noted. These changes are regarded as factors promoting the spreading, and, possibly, the development of primary hemorrhages to the stem, as well as aggravating the degree of
