Results of 261 paracervical blocks done with a dose of 12 ml of 1% chloroprocaine hydrochloride solution containing 1:200,000 epinephrine were recorded. The blocks were followed by abnormal heart rate patterns for 3 to 15 minutes in six of 104 (5.8%) of the fetuses. Excellent pain relief was obtained with 142 of 172 blocks or 83%. The management of a 24-yr-old parturient with Takayasu's arteritis (TA) presenting at term for Caesarean section is discussed. The effects of epidural analgesia on fetal heart rate, fetal heart rate variability, uterine activity, maternal blood pressure, newborn Apgar scores, neonatal acid base status, and the early neonatal neurobehavioral status were studied in 150 parturients during labor and delivery. Local anaesthetics with well-known pharmacology are found to radiosensitize hypoxic and radioprotect oxic murine L5178Y cells. Under the specified experimental conditions, radiation-modifying effects exerted by local anaesthetics are dose-dependent. It appears that in order that the drugs should act as oxic radioprotectors or as hypoxic radio-sensitizers, the agent must be present during the period of irradiation. During general anaesthesia some pigs develop a hyperthermic response which is often fatal. The syndrome is most common in pigs which are susceptible to stress and have a low proportion of body fat. On the basis of an experimental investigation in Pietrain pigs recommendations are presented for the clinical management and treatment of susceptible animals.
