Physical anhedonia, perceptual aberration, and psychosis proneness.

Two groups of hypothetically psychosis-prone subjects were chosen from among college students who scored deviantly high on scales of Physical Anhedonia (n = 50) or Perceptual Aberration (n = 65). Scores on these two scales had a small negative correlation, indicating that the scales identify different sets of deviant subjects. These experimental subjects and a control group (n = 66) were interviewed using a modification of the Schedule for Affective Disorders and Schizophrenia--Lifetime Version. A second interview covered social and academic adjustment. Psychotic and psychotic-like symptoms (a

Poststroke psychosis in an 8-year-old child with moyamoya disease.

Psychiatric illnesses following cerebrovascular accidents has been a popular area of research in recent times. Though poststroke anxiety and mood disorders including depression have been reported often, reports of poststroke psychosis have been considerably rarer. This case report documents an even rarer occurrence of a psychotic disorder in an 8-year-old child with moyamoya disease following a cerebrovascular accident. The child was managed with low-dose atypical antipsychotic, risperidone and the psychotic symptoms eventually were under control around 3 weeks following the initiation of psyc

An 8q21 deletion in a patient with comorbid psychosis and mental retardation.

Systematic investigations indicate that some of the recognized psychiatric disorders can be identified among those with mental retardation due to chromosomal abnormalities. We report a psychotic patient with mild mental retardation (intelligence quotient: 68) and minor anomalies that had a chromosomal aberration not previously described in a psychotic patient. Our patient highlights the importance of the cytogenetic study in psychiatric patients with comorbid mental retardation or minor anomalies. In addition, her psychosis symptoms may be helpful to propose a new candidate gene for psychosis.

[Carbamazepine in the treatment of psychiatric diseases: effects and side effects].

The use of carbamazepine in psychiatry has widened its scope in the last years. Its efficacy is proven best in manic syndromes, in the prophylaxis of manic-depressive illness and in the treatment of alcohol withdrawal symptoms. Beyond this, carbamazepine has been used successfully in depression, schizophrenia, organic psychoses and intermittent explosive disorder. The manifold therapeutic possibilities should not make forget the side effects. The most severe effects to be expected are impairment of the hematopoietic system and skin reactions. Vegetative and neurotoxic effects most commonly occ

Associations between metabolic features and psychotic symptoms in anxious depression across different ages of onset: the mediating role of symptom severity.

BACKGROUND: Anxiety and psychotic symptoms frequently occur in major depressive disorder (MDD) and are associated with metabolic features. Age of onset plays a crucial role in the variability of MDD patients. AIMS: To investigate associations between metabolic features and psychotic symptoms in anxious MDD patients categorized by age of onset. METHODS: We conducted a cross-sectional analysis involving 894 anxious MDD patients, utilizing standardized assessments for depression (Hamilton Depression Scale, HAMD), anxiety (Hamilton Anxiety Scale, HAMA), and psychotic symptoms (Positive and Negat

Hypocalcemia Masquerading as Schizophreniform Disorder.

Among the symptoms of hypocalcemia, tetany, papilledema, and seizures may occur in patients who develop hypocalcemia acutely. We describe a rare case of hypocalcemia presenting as schizophreniform disorder. Among other presentations of hypocalcemia, one should also look for the relation between psychotic symptoms and calcium levels in a patient presenting with psychotic symptoms.

Neuropsychiatric expression and catatonia in 22q11.2 deletion syndrome: An overview and case series.

Individuals with 22q11.2 deletion syndrome (22q11.2DS) are at elevated risk of developing treatable psychiatric and neurological disorders, including anxiety disorders, schizophrenia, seizures, and movement disorders, often beginning in adolescence or early to mid-adulthood. Here, we provide an overview of neuropsychiatric features associated with 22q11.2DS in adulthood. Results of a new case series of 13 individuals with 22q11.2DS and catatonic features together with 5 previously reported cases support a potential association of this serious psychomotor phenotype with the 22q11.2 deletion. Schizoaffective disorder: diagnostic issues and future recommendations.

OBJECTIVE: Difficulties surrounding the classification of mixed psychotic and affective syndromes continue to plague psychiatric nosology. This paper addresses the controversy regarding the diagnostic validity of schizoaffective disorder (SAD), a diagnosis that is used in both DSM-IV and ICD-10 and one that encroaches on both schizophrenia (SCZ) and bipolar disorder (BD). METHODS: A systematic synthesis of clinical and empirical literature, including evidence from cognitive, neurobiological, genetic, and epidemiological research, was undertaken with the aim of evaluating the utility of the SA

Attenuated positive psychotic symptoms and social anxiety: Along a psychotic continuum or different constructs?

Social anxiety commonly occurs across the course of schizophrenia, including in the premorbid and prodromal phases of psychotic disorders. Some have posited that social anxiety may exist on a continuum with paranoia; however, empirical data are lacking. The study aim was to determine whether attenuated positive psychotic symptoms are related to social anxiety. Young adults (N=1378) were administered the Prodromal Questionnaire (PQ), which measures attenuated positive psychotic symptoms (APPS), and the Social Phobia Scale (SPS), which measures a subset of social anxiety symptoms. Confirmatory f…

Childhood adversities and psychotic symptoms: The potential mediating or moderating role of neurocognition and social cognition.

INTRODUCTION: Childhood abuse and neglect are risk factors for psychotic symptoms. Early adversities may contribute to alterations in neuro/social cognition, which in turn is associated with psychosis. This study explored the possible mediating/moderating role of neuro/social cognition between childhood abuse and neglect on the one hand, and psychotic symptoms on the other. METHOD: The sampling frame was 1.119 patients with a psychotic disorder. Childhood adversity was evaluated with the Dutch version of the Childhood Trauma Questionnaire. Psychotic symptoms were assessed with the Positive an

Antipsychotics in Children and Adolescents at Clinical High Risk for Psychosis.

The prescription of antipsychotics in young children and adolescents is a delicate, complex, and often divisive issue, where official guidelines and real-world prescriptive habits seldom correspond.1 Such discrepancy is even more radical and excruciating in those conditions with an established higher risk of imminent development of an overt psychotic state (ie, Clinical High Risk for Psychosis [CHR-P] and the related diagnostic construct of DSM-5 Attenuated Psychosis Syndrome [APS]) that fall below the severity threshold for a DSM-5 diagnosis of schizophrenia and other psychotic disorders.

When a Shadow Brings Peace: A Trauma-Related Visual Phenomenon in an Adolescent With Posttraumatic Stress Disorder.

Trauma-related perceptual experiences may resemble psychotic symptoms, creating diagnostic challenges in adolescent populations. We report the case of a Filipino preadolescent female diagnosed with posttraumatic stress disorder (PTSD) and major depressive disorder following prolonged intrafamilial sexual abuse. During trauma-focused psychotherapy incorporating expressive techniques, the patient consistently depicted and occasionally perceived a non-vivid, dark, cloud-like visual phenomenon that appeared exclusively during states of emotional calm, safety, or happiness. The experience was trans

Serious mental disorder in 23000 prisoners: a systematic review of 62 surveys.

BACKGROUND: About 9 million people are imprisoned worldwide, but the number with serious mental disorders (psychosis, major depression, and antisocial personality disorder) is unknown. We did a systematic review of surveys on such disorders in general prison populations in western countries. METHODS: We searched for psychiatric surveys that were based on interviews of unselected prison populations and included diagnoses of psychotic illnesses or major depression within the previous 6 months, or a history of any personality disorder. We did computer-assisted searches, scanned reference lists,

Case Report: The Association Between Chromosomal Anomalies and Cluster A Personality Disorders: The Case of Two Siblings With 16p11.2 Deletion and a Review of the Literature.

Although several studies have shown the correlation between chromosomal rearrangements and the risk of developing psychotic disorders, such as schizophrenia, little attention has been given to identifying the genetic basis of pre-disposing personality so far. In this regard, a limited but significant number of studies seem to indicate an association between chromosomal anomalies and cluster A personality disorders (CAPD). Starting from the clinical description of two brothers affected by familial 16p11 deletion syndrome (OMIM #611913), both sharing cluster A and C personality traits, the aim o…

Sleep disturbances and first onset of major mental disorders in adolescence and early adulthood: A systematic review and meta-analysis.

Despite several high-quality reviews of insomnia and incidence of mental disorders, prospective longitudinal relationships between a wider range of sleep disturbances and first onset of a depressive, bipolar, or psychotic disorders during the peak age range for onset of these conditions has not been addressed. Database searches were undertaken to identify publications on insomnia, but also on other sleep problems such as hypersomnia, short sleep duration, self-identified and/or generic 'sleep problems' and circadian sleep-wake cycle dysrhythmias. We discovered 36 studies that were eligible for…

Longitudinal associations of naturalistic psychedelic use with psychotic and manic symptoms.

BACKGROUND: Findings from contemporary clinical trials suggest that psychedelics are generally safe and may be effective in the treatment of various psychiatric disorders. However, less is known about the risks associated with psychedelic use outside of medically supervised contexts, particularly in populations that are typically excluded from participation in clinical trials. METHODS: Using a preregistered longitudinal observational research design with a purposive sample of US residents between 18 and 50 years old (N=21,990), we investigated associations between self-reported naturalistic p
