[Longitudinally extensive spinal cord lesion in a case of Neuro-Behçet disease].

A 56-year-old right-handed man with recurrent orogenital aphtoid ulcers and bilateral uveitis had presented with memory disturbance, dressing apraxia and constructional apraxia at age 53. Neuro-Behçet disease was diagnosed based on pathergy test results and positivity for HLA-B51. Four months after azathioprine was introduced, he presented with subacute spastic paraparesis and urinary retention at age 56. Neurological examination demonstrated hyperreflexia in the lower limbs without pathological reflexes. He also showed memory disturbance, dressing apraxia and constructional apraxia. Spinal co

[Study of the blink reflex in spinocerebellar degeneration].

Blink reflex (BR) was composed of R1 and R2. R1 is considered to indicate the brainstem function. The purpose of this study was to evaluate the brainstem function of spinocerebellar degeneration (SCD) by the R1 latency (R1). BR was evoked by the percutaneous stimulation of supraorbital nerve. And R1 was measured by the action potential of orbucularis oculi muscle. Direct response was recorded by the percutaneous stimulation of facial nerve to determine the terminal latency of facial nerve (D). By these methods R1, D, R1/D and difference between at right and left side of these data (delta R1, d

Window within the web: Bilateral florid persistent pupillary membranes.

A 14-year-old boy was found to have bilateral persistent pupillary membranes with best-corrected visual acuity of 6/6 in both eyes. (A, B) Diffuse slit-lamp photographs of the right and left eyes show extensive pigmented pupillary membrane strands arising from the iris collarette and interlacing across the pupillary apertures, producing a dense web-like configuration with multiple irregular fenestrations. (C, D) Retroillumination sharply delineates the membrane architecture against the red reflex, revealing residual optical windows, including clear openings along the visual axis. Despite the f

Multifocal Stroke and Gait Ataxia in the Elderly: A Peculiar Case of Meningovascular Neurosyphilis With Posterior Circulation Infarction.

An 83-year-old male presented with a months-long ataxic gait and recent frequent falls. Initial evaluation revealed ventriculomegaly and acute ischemic infarction to the right cerebellar and occipital regions in addition to severe cervical stenosis. Syphilis serology with reflex returned positive, and neurosyphilis was confirmed on cerebrospinal fluid with a reactive Venereal Disease Research Laboratory test, lymphocytic pleocytosis, and elevated protein. The patient was put on a two-week course of intravenous penicillin G while completing inpatient physical rehabilitation and demonstrated sig

Post-surgical left oculomotor nerve neuromyotonia: Cause or coincidence?

Ocular neuromyotonia (ONM) is characterized by episodes of binocular diplopia usually triggered by an eye movement requiring contraction of the affected extraocular muscle. It consists of an involuntary, sometimes painful contraction of one or more extraocular muscles. It is most often secondary to radiotherapy of the para-sellar region, although other aetiologies have been reported. Some cases do not have a clearly identified aetiology and are classified as idiopathic. Most cases of ONMs are unilateral but bilateral ONMs have also been described.1-4 We report a case of left ONM in a 55-year-o

Vestibular Migraine Following Radiosurgery for Vestibular Schwannoma.

Vestibular schwannoma (VS) is associated with dizziness and vertigo during all stages of treatment. This report describes a patient who presented with a one-year history of intermittent motion sickness, dizziness, headache, imbalance, and nausea. MRI showed a right-side VS in the cerebellopontine angle and internal auditory canal. The patient elected to undergo Gamma Knife radiosurgery for treatment. Within two to three months, she continued to experience recurring dizziness, vertigo, neck stiffness, and head pressure. She was referred for neurotology evaluation, which led to a diagnosis of ve

Ross Syndrome: A Patient with a 23-Year History.

We present a 60-year-old female with a 23-year history of anhidrosis with concomitant heat intolerance. At examination, we found a right-sided tonic pupil, absent tendon reflexes, and a segmental patch of compensatory hyperhidrosis in the left lower quadrant of her trunk. To further confirm this finding, a minor test (a starch-iodine test, which is used to evaluate the sudomotor function, sweating) was performed. Nerve conduction studies revealed evidence of a mild distal sensory polyneuropathy of the axonal type. Tilt-table testing showed signs of orthostatic hypotension with evidence of redu

Transient supranuclear paresis of the abduction in viral encephalitis of the brainstem.

The supranuclear paresis of the abducens system, also known as posterior internuclear ophthalmoplegia of abduction, is a very rare disorder clinically characterized by unilateral or bilateral abduction paresis sometimes associated with nystagmus of the contralateral adducting eye, slowing of abduction saccades, and intact horizontal vestibulo-ocular reflex. Here, we report a 35-year-old woman who presented transient left side abduction deficit in conjunction, as the only symptom of self-limited viral encephalitis of the brainstem. Brain MRI including DWI and ADC maps showed an area of abnormal

[Dissociated sensory loss caused by acupuncture injury to the cervical spinal cord].

A 31-year-old woman developed dissociated sensory loss below the right C-3 dermatome within an hour after acupuncture therapy for the left posterior neck pain. Moxa has been applied on the top of the acupuncture needle: Moxa needle. T2-weighted MR images on the following day showed a high-signal cord lesion at the C1/2 level on the left. After 10 days, the MRI lesion became clear; it involved the left lateral spinothalamic tract and the lateral corticospinal tract; however, her muscle strength and deep tendon reflexes were normal. The C1/2 focus reduced slightly 2 months after the accident, an

Modification of compensatory saccades after aVOR gain recovery.

The recruitment of extra-vestibular mechanisms to assist a deficient angular vestibulo-ocular reflex (aVOR) during ipsilesional head rotations is well established and includes saccades of reduced latency that occur in the direction of the lesioned aVOR, termed compensatory saccades (CS). Less well known is the functional relevance of these unique saccades. Here we report a 42 y.o. male diagnosed with right unilateral vestibular hypofunction due to vestibular neuronitis who underwent a vestibular rehabilitation program including gaze stabilization exercises. After three weeks, he had a signific

Unique myoclonic pattern in corticobasal degeneration.

We describe two similar patients with a clinical diagnosis of corticobasal ganglionic degeneration (CBGD). After a period of increased action tremor, both patients developed a fixed posture in the right arm with a slow rhythmic myoclonus, which appeared to be caused by trains of highly synchronized and stimulus sensitive myoclonic discharges. Resetting of the spontaneous myoclonic discharges by peripheral and central stimulation and a jerk-locked cortical potential were demonstrated in one case. The somatosensory evoked potentials (SEPs) showed abnormal parietal curves with small N20-P25 ampli

Reflex sympathetic dystrophy involving three limbs: a case study.

Reflex sympathetic dystrophy (RSD) is a symptom complex characterized by pain, hyperesthesia, and vasomotor instability. RSD has long been considered a manifestation of sympathetic nervous system dysfunction because of characteristic edema and vasomotor instability in addition to frequent alleviation of RSD symptoms by sympathetic blockade. RSD generally only affects one limb although it is recognized that the contralateral limb is also abnormal and at risk of also developing clinical RSD without the advent of a new injury. An unusual case of three-limb RSD is presented where classical RSD was

[A case of juvenile onset ataxia with dystonia, myoclonus, sensorineural hearing loss and mental retardation].

A 35-year-old female was reported who presented early onset and slowly progressive ataxia and retrocollis which appeared at the age of nine. On admission, neurological examination revealed cerebellar ataxia, dystonia of the neck and the right arm, myoclonus of the neck and the shoulder, slight mental retardation, supranuclear upper gaze palsy, and sensorineural hearing loss. Laboratory examination showed high serum CK activity. Electromyography and muscle biopsy findings suggested slight muscular involvement. CSF level of HVA and 5-HIAA were reduced. MRI demonstrated marked cerebellar atrophy

[Rupture of the diaphragm of late manifestation].

Twenty one cases of delayed diagnosis of ruptured diaphragm caused by closed trauma are reported: 14 on the left side and 7 on the right side. The clinical signs and the modes of presentation are non-specific. Although the history of thoraco-abdominal trauma and the chest x-ray are sufficient to establish the diagnosis of rupture of the left hemidiaphragm, they can only suggest the diagnosis in cases of righ-sided rupture. The mechanism of rupture is more often due to sudden reflex contraction of the diaphragm against a closed glottis than to excessive abdominal pressure caused by the trauma.

Isolated environmental tilt associated with lateral medullary compression by dolichoectasia of the vertebral artery. Is there a cause and effect relationship?

An elderly woman with the persistent isolated symptom of monocular and binocular tilting of environmental objects, was found on magnetic resonance imaging to have compression of the lateral medulla by an ectatic vertebral artery. The double Maddox rod test showed no relative torsion of either globe although 5 degrees of right excyclotorsion and left incyclotorsion (rightward ocular counter-rolling) was detected on testing with monocular viewing. There was no accompanying hyperdeviation, nystagmus or other neurologic signs. Symmetrical tilting of each globe associated with head tilt and skew de

Otoconia in health and disease. A review.

Mammalian otoconia are made up of inorganic calcium carbonate crystallites and organic substance, which is thought to be sulfated glycosaminoglycans and glycoprotein. Mouse otoconia are formed in situ, and the primitive otoconia assume spindle, trigonal, dumbbell, or quadrilobed forms. As the otoconia mature, they assume the typical barrel-shaped body with pointed tips formed by three surfaces. A variety of otoconial disorders have been reported. Otoconial degeneration can result from ototoxic drugs, infection, trauma, and aging. Abnormal otoconia are also known to occur as a result of genetic

Measurement of intestinal progression of a meal and its residues in normal subjects and patients with functional diarrhoea by a dual isotope technique.

A new double isotopic method was used to measure the gastrointestinal progression of a meal in nine healthy subjects and seven patients with functional diarrhoea. 51 Chromium chloride (colonic marker) was ingested eight hours before the beginning of the scintigraphic study so that it was by then located in the colon at that time. A second marker, 99m Technetium sulphur colloid labelled the meal. Scintigraphic images were taken before and after the meal for two hours, detecting simultaneously the two isotopes. In the 51 Cr window right colon was localised and intracolonic propulsion was studied

Endothelial cell population changes of human cornea during life.

A photo slit lamp was used to obtain color, specular reflex, high magnification photographs of the corneal endothelium of subjects ranging in age from 3 to 88 years. Multiple areas of the cornea were examined to determine the endothelial cell population. No appreciable difference in cell density was found between the right and left eyes of the subjects nor between male and female subjects of similar age. Apparent defects in the endothelial cell coverage of Descemets membrane were found in subjects as young as 20 years of age and with increased frequency in older age groups. These defects were
