The incidence of surgical intervention following a suspected scaphoid fracture.

AIMS: The underlying natural history of suspected scaphoid fractures (SSFs) is unclear and assumed poor. There is an urgent requirement to develop the literature around SSFs to quantify the actual prevalence of intervention following SSF. Defining the risk of intervention following SSF may influence the need for widespread surveillance and screening of SSF injuries, and could influence medicolegal actions around missed scaphoid fractures. METHODS: Data on SSF were retrospectively gathered from virtual fracture clinics (VFCs) across a large Scottish Health Board over a four-year period, from 1

Functional and radiologic outcomes of non-displaced scaphoid waist fractures in adolescents approaching skeletal maturity: Comparison between conservative treatment and percutaneous screw fixation.

BACKGROUND: Treatment of scaphoid waist fractures is generally conservative in children but surgical in adults, given the relatively high risk of non-union in adults. In adolescents, the required therapeutic strategy is less well defined. The objective of this study was to compare the radiographic and clinical parameters, and the rate of complications, between non-surgical orthopedic treatment (OT) and surgical treatment (ST) by percutaneous screw fixation of these fractures in adolescents approaching skeletal maturity. HYPOTHESIS: ST of non-displaced scaphoid waist fractures in adolescents a

Nonunion of the carpal scaphoid: the Russe procedure.

Symptomatic carpal scaphoid nonunion was treated by the volar bone graft technique described by Russe in 31 patients reviewed. The technique was successful in attaining union in 27 wrists; nonunion persisted in 4 wrists. The presence of n unstable nonunion did not seem to affect the end result if the small necrotic proximal pole fracture is excluded. Obviously this last fracture should be treated by other methods.

Muscle-based gene therapy and tissue engineering to improve bone healing.

Failed fracture healing is a significant problem in orthopaedics, often seen in patients with scaphoid fractures, high-energy injuries, and osteoporosis. Current treatments often result in poor outcomes and donor site morbidity. Gene therapy has been the focus of much recent research to improve bone healing. In the current review, the authors specifically evaluate the use of muscle-derived cells as a gene delivery vehicle and inducible osteoprogenitor cell that can enhance bone regeneration. Muscle-derived cells have been used to deliver bone morphogenetic protein-2 and produce ectopic bone. T

Common treatments and procedures used for fractures of the distal radius and scaphoid: A review.

The distal radius and the scaphoid are the most commonly injured carpal bones among both active adults and the osteoporotic elderly. The purpose of surgical treatment is to restore form and function to the wrist. Depending on the nature of the fracture, either topical procedures or invasive surgery can be applied. This article critiques the treatments currently used for fixation of wrist fractures in order to drive the development of new materials to improve patient outcomes.

The treatment of delayed union and non-union of the carpal scaphoid by screw fixation.

A simple technique for screw fixation of the carpal scaphoid in cases of delayed union and non-union is described. The results obtained in 32 patients treated by this method are reported. The average follow-up was three years ranging from 10 months to 12 years. Union was obtained in 28 patients. The causes of failure to unite are discussed and the advantages of the reported method over other techniques such as bone grafting are stressed.

Dorsal open repair of proximal pole scaphoid fractures.

Proximal pole fractures of the scaphoid are well suited for comprehension screw fixation. A dorsal approach allows for direct visualization of the fracture site, accurate reduction, and internal fixation. Bone grafting can also be achieved through the same incision without additional significant dissection. Successful uncomplicated union in the majority of cases with a considerably shortened period of immobilization has lead to a growing interest in this surgical procedure.

[The combined luxation of the trapezoid and 2nd metacarpal bones. Apropos of a case].

The authors report a case of posterior dislocation of the trapezoid bone together with the second metacarpal. Among the 25 cases of trapezoid dislocations reported in the literature, no similar case has been found. The patient was seen 4 months after the injury and there was also a scaphoid instability. Open reduction and a scapho-trapezio-trapezoid arthrodesis were performed. The diagnosis, the mechanism and the treatment of this very unusual injury are discussed.

Comparing Outcomes of Surgical and Nonsurgical Management for Minimally Displaced Scaphoid Waist Fractures: A Systematic Review and Meta-Analysis of randomized Controlled Trials.

BACKGROUND: Scaphoid waist fractures are among the most common carpal fractures, and their management remains debated, particularly for minimally displaced fractures. Surgical and nonsurgical treatments are widely utilized, but their comparative effectiveness lacks consensus. OBJECTIVE: To evaluate and compare the outcomes of surgical versus nonsurgical management for minimally displaced scaphoid waist fractures. METHODS: A comprehensive search was conducted in PubMed, Google Scholar, and the Cochrane Library to identify relevant RCTs. We identified 2936 articles during the research. 2707 ar

Scapholunate Instability: Diagnosis and Management - Classification and Treatment Considerations - Part 2.

Intervention for scapholunate instability is aimed at halting the degenerative process by restoring ligament integrity and normalizing carpal kinematics. Part 1 of this Current Concepts article reviewed the anatomy, kinematics, and biomechanical properties of the scapholunate ligament as well as its critical stabilizers. In this section, we provide a foundation for understanding the spectrum of scapholunate ligament instability and incorporate meaningful new anatomical insights that influence treatment considerations. These updates clarify the importance of the critical stabilizers of the scap

Correlation between dorsal capsulo-scapholunate septum (DCSS) lesion and predynamic scapholunate instability.

OBJECTIVE: Scapholunate instability is the most common ligament injury of the wrist. In case of predynamic instability, diagnosis can be difficult, even under arthroscopy. Scapholunate instability is not the result of an isolated injury to the scapholunate interosseous ligament. Scapholunate stability is dependent not only on the scapholunate ligament, but on a complex that is comprising intrinsic and extrinsic ligaments. The dorsal capsule-scapholunate septum (DCSS) is one of the secondary stabilizers of the scapholunate complex. The objective of the present study was to investigate the corre

Management of acute scaphoid fractures: a pragmatic approach for the non-specialist.

Scaphoid fractures are common and can cause significant morbidity if treated incorrectly. Thus, a working knowledge of the initial assessment and management of scaphoid fractures by non-specialists is crucial to allow quick diagnosis and avoid potentially catastrophic complications of scaphoid fracture. This article summarises the anatomy of the scaphoid, discusses methods to assess for scaphoid fractures and delineates management plans (conservative or operative) for fractures of the scaphoid based on location of vascular compromise. This article can also help the clinician predict which frac

[Perilunares dislocations of minor arch what is the best therapeutic method?].

INTRODUCTION: Carpal dislocations and fracture-dislocations are uncommon injuries and represent approximately 5-7% of all wrist injuries, or about 10% of all carpal injuries. Multiple treatments are proposed to treat these injuries. The aim of our paper is to review the evidence about the treatment of lesser arc perilunate dislocations and to assess the results of the different therapeutic methods. MATERIAL AND METHODS: A systematic review was carried out through the electronic search engines PubMed, Bireme and Timbó. The search reached a total of 898 articles, according to the inclusion and

Casting Position for Distal Radius Fractures Changes Radiocarpal Joint Forces: A Cadaveric Study.

PURPOSE: Nonsurgical distal radius fracture treatment requires immobilization and classical teaching suggests varying cast positions. We investigated the effect of cast position on the force and pressure experienced by the articular cartilage in the scaphoid and lunate fossae. METHODS: Ten fresh-frozen cadaveric specimens were used. A standardized extra-articular distal radius fracture was made. Force sensors were affixed to the articular cartilage of the scaphoid and lunate fossae. Baseline data were obtained. Specimens were then placed into a short arm cast with the wrist either neutrally a

MRI Utility in Hand Surgery.

Background: Magnetic resonance imaging (MRI) is frequently used in the diagnosis of upper extremity conditions. We hypothesize that the results often do not change treatment. The purpose of the present study was to prospectively assess the extent to which MRI findings alter patient management. Methods: Ten hand surgeons who were ordering an MRI (elbow or distal) completed a standardized questionnaire. Prior to obtaining the MRI, the surgeon recorded their pre-MRI diagnosis and recommended treatment if no MRI could be obtained. Following the MRI, the surgeon recorded their post-MRI diagnosis an
