BACKGROUND: Intracranial nail impalement injury is a rare type of low-energy penetrating brain injury resulting from the forceful entry of a nail into the cranial cavity without exiting it. Clinical outcome depends on the number of impaling nails, the depth of penetration, the extent of the primary brain injury, associated vascular injury, and prompt surgical management to limit secondary injuries. The authors report a case and the management outcome in a low-middle-income country.
OBSERVATIONS: A 27-year-old man was reportedly found by his relatives on the floor of his room, having generaliz

BACKGROUND: Needs assessments in patients with juvenile idiopathic arthritis (JIA) have revealed a need for disease information, self-management skills, and peer support. We previously developed and tested the acceptability of an in-person and videoconference-based self-management program (SMP) to address these needs.
OBJECTIVE: The aim of this pilot randomized controlled trial (RCT; the VISTA-JIA trial) is to evaluate the feasibility and preliminary effectiveness of a virtual group-based SMP for adolescents with JIA in comparison to a waitlist control group.
METHODS: A total of 100 particip…

BACKGROUND: In addition to endemic communicable diseases, resource-poor health systems in low- and middle-income countries (LMICs) are buckling under an increasing prevalence, morbidity, and mortality attributable to non-communicable diseases (NCD) - 'NCDs crisis.' Supported self-management (SM) is a recommended approach for improving patient outcomes. There is yet no robust evidence on the effectiveness of SM interventions for people with long-term conditions (LTCs) in LMICs.
METHODS: Underpinned by a comprehensive search of seven databases, we conducted a systematic review and evidence synt…

INTRODUCTION: Older adults with cancer are living longer, but their unique concerns and approaches to self-management are not well understood. Self-management strategies (SMS), historically studied in other chronic diseases, are a promising approach for empowering older patients with cancer and reducing their symptom burden. Self-management strategies are behaviors aimed at coping with the physical sequelae of and psychosocial adjustment to life-limiting disease. We sought to characterize the concerns and self-management strategies of older patients with advanced cancer.

BACKGROUND: Self-expandable valves are emerging complements to balloon-expandable valves for transcatheter pulmonary valve replacement in dysfunctional right ventricular outflow tracts, though their safety and efficacy remain underexplored. We aim to compare patient characteristics and outcomes of self-expandable valves and balloon-expandable valves in transcatheter pulmonary valve replacement.
METHODS: Baseline and early follow-up data were prospectively analyzed for 145 patients who underwent transcatheter pulmonary valve replacement with Edwards SAPIEN 3 (ES3) balloon-expandable valves or

BACKGROUND: Self-management knowledge and behaviors may develop differently depending on age of multiple sclerosis (MS) onset.
QUESTIONS: 1) Are age of MS onset and self-management associated? 2) Does age of MS onset modify the relationship between self-management and clinically important outcomes?
METHODS: Adult participants from the Canadian Prospective Cohort Study to Understand Progression in MS, completed the Multiple Sclerosis Self-Management Scale-2 (MSSM-2). We tested the relationship between age of onset and MSSM-2 scores using multivariable linear regression, adjusting for age and …

AIMS: Behaviour modification and mood management are essential to recovery after a cardiac event. Recent times have seen a major shift to remote delivery of cardiac services. This study assessed behavioural and psychological outcomes of the Back on Track online self-management programme, comparing the programme undertaken alone (self-directed) vs. with telephone support (supported). Relevance for people with depression was also assessed.
METHODS AND RESULTS: Participants with cardiac conditions (n = 122) were randomly assigned to self-directed or supported groups and given access to the onlin…

BACKGROUND: Gestational diabetes mellitus is a common complication during pregnancy, and its prevalence rates have increased dramatically in recent years. Treatment of gestational diabetes requires the active self-management, however, this can be challenging. Understanding the barriers and facilitators of adherence to self-management recommendations is essential for designing effective interventions.
AIM: To identify and synthesize barriers and facilitators to self-management of gestational diabetes reported by pregnant women.
METHODS: This was a mixed-methods systematic review, including qu…

INTRODUCTION: In-office use of the Trendelenburg position has been shown to be a beneficial clinical tool to help decipher if a CSF pressure/volume component is part of the underlying etiologic process for a patient's persistent headache. Utilizing the Trendelenburg position at home could potentially be an additional diagnostic tool for the treating headache physician.
CASE SERIES: Our headache practice has been using at-home self-Trendelenburg for the past 2 years and will present the clinical scenarios in which it seems to be the most helpful utilizing a case series of patients. These inclu

BACKGROUND/OBJECTIVES: The event-rate of recurrent acute pancreatitis (RAP) in patient populations is critical for powering research studies. We hypothesize that some patients manage RAP attacks at home, reducing event rate estimations based on counting emergency department (ED) visits and hospitalizations only. The aim of this study was to determine the rates of home self-management of recurrent acute pancreatitis compared to ED visits and hospitalizations.
METHODS: An anonymous 8-question survey was sent to 1825 individuals on an email list of individuals with a history of acute pancreatiti…

BACKGROUND: Conditioned pain modulation (CPM) is a potential predictor of treatment response that has not been studied in temporomandibular disorders (TMD).
OBJECTIVES: We conducted a randomised, double-blind, placebo-controlled trial (RCT) of duloxetine in addition to self-management (SM) strategies to investigate its efficacy to reduce pain intensity in painful TMD patients. Moreover, we investigated whether baseline CPM would predict the duloxetine efficacy to reduce TMD pain intensity.
METHODS: Eighty participants were randomised to duloxetine 60 mg or placebo for 12 weeks. The primary o…

OBJECTIVE: The purpose of this interdisciplinary study was to determine Dominican adolescents' preferences for the content, functionality (i.e. food's glycemic index list, and amount of insulin based on foods' carbohydrate count), and design of a culturally and linguistically relevant mobile application (app) for type 1 diabetes mellitus (T1DM) self-management. The app would facilitate T1DM self-management education and support by addressing providers' challenges in monitoring patients' disease progression and promoting patient adherence to recommended lifestyle changes. Findings inform an app…

In people with a diagnosis of bipolar I, a smartphone app for symptom monitoring and behavioral guidance (with coaching on how to use it and encouragement to do so) lowered the risk of relapse into a mood episode but only in patients who were inherently at lower risk of such a recurrence. By comparison, participants who had continuous or prodromal symptoms before the intervention did not show an intervention effect, perhaps because this group also had multiple indicators of more severe illness and more challenging life circumstances.
