Patient outcome after surgical management of the spinal accessory nerve injury: A long-term follow-up study.

OBJECTIVES: A lesion in the spinal accessory nerve is typically iatrogenic: related to lymph node biopsy or excision. This injury may cause paralysis of the trapezius muscle and thus result in a characteristic group of symptoms and signs, including depression and winging of the scapula, drooped shoulder, reduced shoulder abduction, and pain. The elements evaluated in this long-term follow-up study include range of shoulder motion, pain, patients' satisfaction, delay of surgery, surgical procedure, occupational status, functional outcome, and other clinical findings. METHODS: We reviewed the m

First-rib stress fracture in two adolescent swimmers: a case report.

First-rib stress fractures have been described in adolescent athletes in various sports, with only one prior case report of first-rib stress fractures in an adolescent female swimmer. There is a need for research on the cause, management, and prevention of these injuries as they lead to significant morbidity and critical time away from sport for these aspiring athletes. We aimed to describe first-rib stress fractures as a potential cause for non-specific atraumatic chronic shoulder pain in adolescent swimmers and to discuss the different presentations, unique risk factors, treatment, and poten

Within-session reliability and smallest real difference of muscle strength following nerve transfers in patients with brachial plexus injuries.

PURPOSE: To quantify the smallest real difference (SRD) of muscle strength in patients with brachial plexus injuries (BPI) after nerve transfer. METHODS: This study enrolled 16 patients with BPI who had C5-C6 and C5-C7 root injuries and who received nerve transfer of the spinal accessory nerve to the suprascapular nerve and fascicles of the ulnar nerve to the branch of the musculocutaneous nerve that innervates the biceps muscle. The quantitative peak strength of the shoulder abductor, external rotator, and elbow flexor in both arms were measured by the hand-held dynamometer. The SRD of each

Team Approach: Treatment of Shoulder Instability in Athletes.

»: Shoulder instability is a complex problem with a high rate of recurrence in athletes. Treatment of a first-time subluxation or dislocation event is controversial and depends on patient-specific factors as well as the identified pathology. »: Athletic trainers and physical therapists are an integral part of the treatment team of an in-season athlete who has experienced a shoulder instability event. Through comprehensive physiological assessments, these providers can effectively suggest modifications to the patient's training regimen as well as an appropriate rehabilitation program. »: Surg

[Arthropathy of instability. Causes, treatment options and results].

Glenohumeral arthritis may develop after primary or recurrent shoulder dislocation or after surgery for stabilization. Postoperatively, the incidence is reported to be from 12% to 62%, depending on different risk factors. There is no certain correlation between the surgical technique and the rate of arthrosis. Capsulorraphy arthropathy is biomechanically and clinically a well defined entity. The risk of developing severe arthrosis of the shoulder following dislocation of the shoulder is between 10 and 20 times greater in comparison to the normal population. Risk factors are the age during the

[Complication management after unsuccessful operative shoulder stabilization].

Operative repair of shoulder instability may fail because of multiple causes: a constitutional predisposition, a new trauma, incorrect diagnosis, inadequate operative techniques or inappropriate rehabilitation can be involved. The key to successful revision surgery is a thorough analysis of errors of the primary repair and the revision also has to deal with the decisive pathological factors. The management of revision surgery after failed surgery for patients with instability has to focus on the decisive pathological factors and has to take a higher complication rate and lower success rate tha

Magnetic resonance imaging of the shoulder.

MR imaging has proved to be of great value in the assessment of musculoskeletal imaging. Its superiority in evaluating knee and spine pathology is well established. Recent studies also have established an important role for MR imaging in the assessment of shoulder disorders. Multiplanar capabilities coupled with excellent contrast resolution that allows simultaneous delineation of osseous and soft-tissue structures make MR imaging a useful method for evaluating the complex anatomy of the shoulder.

[Possibilities of magnetic resonance tomography in diagnostic imaging of the shoulder joint].

By virtue of its multiplanar representation, magnetic resonance imaging (MRI) allows clear visualization of the complex anatomical relationships of the shoulder joint. In addition to axial planes, slices perpendicular and parallel to the glenoid cavity are used to good advantage. In tears of the rotator cuff an increase in signal intensity within the cuff is recognized in T2- and proton-density-weighted images. Lesions of the glenoid labrum following luxations of the glenohumeral joint can be detected and classified using MRI. The diagnostic value of MRI as compared with other imaging modaliti

MR arthrogram findings of luxatio erecta in a pediatric patient-arthroscopic confirmation and review of the literature.

Luxatio erecta or inferior glenohumeral dislocation is a rare type of shoulder dislocation, accounting for less than 1 % of all reported shoulder dislocations. We describe a 15-year-old male who presented with luxatio erecta following an injury to his shoulder that resulted from a mountain biking accident. Clinically, the patient had shoulder pain and fixed abduction of the arm. Radiographs confirmed the diagnosis of luxatio erecta. A magnetic resonance arthrogram (MRA) performed 9 days after presentation demonstrated both a greater tuberosity fracture and avulsion of the anterior and posterio

Proximal Biceps Tendon and Rotator Cuff Tears.

The long head of biceps tendon (LHBT) is frequently involved in rotator cuff tears and can cause anterior shoulder pain. Tendon hypertrophy, hourglass contracture, delamination, tears, and tendon instability in the bicipital groove are common macroscopic pathologic findings affecting the LHBT in the presence of rotator cuff tears. Failure to address LHBT disorders in the setting of rotator cuff tear can result in persistent shoulder pain and poor satisfaction after rotator cuff repair. Tenotomy or tenodesis of the LHBT are effective options for relieving pain arising from the LHBT in the setti
