BACKGROUND: Insertion of an implant in the edentulous posterior maxilla is a challenging procedure because of poor bone quality and increased pneumatization of the maxillary sinus after tooth extraction. To increase the amount of bone, several surgical bone grafting techniques have been used-with considerable morbidity for patients. Osteotome sinus floor elevation (OSFE) is a less invasive technique. The clinical and radiographic outcome of 53 implants placed with this technique without bone graft has been reported previously.
PURPOSE: Here we report the clinical and radiographic findings aft

BACKGROUND: Craniomaxillofacial bone defects are difficult to reconstruct because they combine irregular anatomy, functional loading, contamination risk, and high esthetic demands. Patient-specific 3D printing improves geometric reconstruction, but most conventional scaffolds remain static after implantation. Four-dimensional printing and stimuli-responsive smart scaffolds add a temporal dimension and may enable programmable shape change, controlled release, immunomodulation, antibacterial activity, vascular support, or electromechanical signaling.
METHODS: A narrative review was conducted us

PURPOSE: To determine the prevalence of maxillary sinus pathology in patients presenting for implant rehabilitation involving sinus augmentation procedures.
MATERIALS AND METHODS: Three-dimensional images of 275 patients were evaluated. Age and gender were recorded to see if they had any relationship to the prevalence of pathology. Scans were classified into 1 of the 5 categories based on the type of sinus pathology detected: healthy, mucosal thickening > 5mm, polypoidal mucosal thickening, partial opacification and/or air fluid level, and complete opacification.
RESULTS: Overall, 54.9% scan

Azithromycin is widely used in dentistry as a systemic antibiotic, particularly for odontogenic infections and as an alternative in patients with β-lactam hypersensitivity. Beyond its antimicrobial activity, azithromycin possesses unique pharmacokinetic, anti-inflammatory, immunomodulatory, and anti-biofilm properties. Together with growing concerns regarding antimicrobial resistance and antibiotic stewardship, these characteristics have stimulated interest in local drug-delivery strategies that may reduce systemic antibiotic exposure while maintaining therapeutic efficacy. This narrative revi

Severe atrophic posterior maxillary ridge (residual bone height < 3 mm) could be a challenging situation to place dental implants. Several treatment options have been proposed, but some of them may require advanced surgical skills to achieve best results. In this article, we present a novel and easier technique to allow implant placing in localized areas of severe atrophy. In a first step, a 4.5-length extra-short (unloaded) implant is placed after a transcrestal maxillary sinus floor augmentation (MSFA). After the gained apical bone consolidation, this "temporary implant" is atraumatically re…

Maxillary sinus grafting is required to increase bone volume in the atrophic posterior maxilla to facilitate dental implant placement. Grafting with autogenous bone (AB) is ideal, but additional bone harvesting surgery is unpleasant. Alternatively, bone substitutes have been used but they limit new bone formation. The strategy of single-visit clinical stem cell therapy using bone marrow aspirate concentrate (BMAC) to facilitate new bone formation has been proposed. This study aimed to assess bone regeneration capacity of autologous BMAC mixed with bovine bone mineral (BBM) in maxillary sinus g

AIM: The first objective of this study was to qualitatively and quantitatively assess the bone formation process, particularly the long-term behavior and three-dimensional volume stability of subsinusal bone regeneration, using titanium (Ti) or bovine hydroxyapatite (BHA) granules, in a rabbit model. The second objective was to evaluate the effect of the hydration of the BHA particles with a therapeutic concentration of doxycycline solution on the osteogenesis and biomaterial resorption.
MATERIALS AND METHODS: Rabbits underwent a double sinus lift procedure using one of three materials: grade

At dental implantology's inception, patients seeking tooth or teeth replacement were confronted with the concern of whether they were viable "candidates" for treatment. Largely, this criteria was based on 3-dimensional bone volume present in edentulous sites selected for implant fixture insertion. When surgeons determined this volume as insufficient for implant placement, patients were encouraged to seek alternative restorative therapy. As the ability to regenerate lost tissue, both hard and soft, has evolved, the number of patients now considered "candidates" for implant therapy has increased

PURPOSE: To analyze the risk factors associated with the survival of self-threading implants placed with the ridge expansion technique using self-tapping osteotomes after at least 24 months.
MATERIALS AND METHODS: This retrospective study followed patients for at least 2 years in whom implants had been inserted by means of the ridge expansion technique. The patient was taken as the unit of analysis, and the loss of any implant as a result of mobility at any time during the follow-up period was considered as a failure. A number of patient variables were recorded: sociodemographic (age/sex), an

BACKGROUND: Extensive alveolar bone resorption in the maxilla limits the possibility of successful placement and osseointegration of endosseous implants for future prosthetic rehabilitation. Autogenous bone from the iliac crest may be used as lateral onlays in the atrophic maxilla, both as block and particulate bone. To our knowledge, there is no three-dimensional 2-year follow-up study measuring the volumetric reduction of the augmented areas comparing particulate and block bone grafts.
PURPOSE: The aim of this study was to conduct a radiographic 2-year follow-up study, using computed tomogr

Transcrestal sinus membrane elevation is a surgical procedure performed to increase the bone volume in the maxillary sinus cavity. Because of visual limitations, the potential for maxillary sinus membrane perforations may be greater than with the lateral approach technique. The aim of this study was to macroscopically investigate ex vivo the occurrence of sinus membrane perforation during surgery using 3 transcrestal sinus floor elevation methods. Twenty fresh human cadaver heads, with 40 intact sinuses, were used for simultaneous sinus membrane elevation, placement of graft material, and dent

BACKGROUND: The primary objective of this study is to use histomorphometric techniques to evaluate the concept that the new bone formed in the maxillary sinus lift procedure emanates from the endosteum of the sinus floor. In addition, the effect of the residual crest vertical dimension on the graft outcome and assessment of osteoclast numbers as an indirect measure of a connection between the crest and graft compartment are reported.
METHODS: After grafting the maxillary sinus with irradiated allogenic bone, 37 intact, vertical bone cores with a 2.7 mm diameter were trephined at right angles

UNLABELLED: : In patients undergoing maxillary sinus floor elevation (MSFE) for dental implant placement, bone substitutes are currently evaluated as alternatives for autologous bone. However, bone substitutes have only osteoconductive properties and lack osteoinductive potential. Therefore, this phase I study evaluated the potential additive effect on bone regeneration by the addition of freshly isolated, autologous but heterologous stromal vascular fraction (SVF), which is highly enriched with adipose stromal/stem cells when compared with native adipose tissue. From 10 patients, SVF was proc

OBJECTIVES: Graft resorption remains a major disadvantage of the various employed materials in maxillary sinus augmentation. Our aim was to assess the percentage of graft volume reduction in deproteinized bovine bone (DBB) using cone beam computed tomography (CBCT).
MATERIALS AND METHODS: A retrospective analysis of 29 augmented sinuses in 19 patients was conducted. The graft material was either DBB alone or DBB mixed with autologous bone chips (80% DBB/20% bone chips). CBCT scans of the augmented sinuses were obtained pre- and post-operatively up to 2-year follow-up.
RESULTS: In total, 55 d
