A cavernous sinus lesion clinically responsive to steroids.

Tolosa Hunt syndrome (THS) is characterized by painful ophthalmoplegia secondary to idiopathic granulomatous inflammation of the cavernous sinus. The characteristic finding on MRI is an enhancing T1 isointense and T2 hypo- or hyperintense cavernous sinus mass lesion, which may result in focal narrowing of the ipsilateral internal carotid artery. Although the incidence is quite rare, it is a common diagnostic consideration in cases that present with multiple cranial neuropathies. However, the differential diagnosis for a unilateral cavernous sinus lesion in adults is broad and includes neoplast

A useful maneuver to simplify sellar floor repair following endoscopic transnasal pituitary surgery.

The watertight closure of the skull base after endonasal surgery is and has always been critical in cranial base surgery. Nowadays, endoscopic transsphenoidal surgery is the standard of treatment for most of the sellar tumors. Reconstruction of the sella is not always deemed necessary, and generally it is reserved for patients with macroadenomas or with intraoperative evidence of cerebrospinal fluid leak. We herein report our experience with a simple maneuver that, in our opinion, simplifies sellar floor reconstruction.

Occam's razor in minimally invasive pituitary surgery: tailoring the endoscopic endonasal uninostril trans-sphenoidal approach to sella turcica.

BACKGROUND: Since the introduction of the endoscopic endonasal approaches in the field of skull base surgery during the last two decades, several variants of the sella turcica endoscopic surgery have been described. The aim of this study is to provide a stepwise description of one of these variants in a minimally invasive/maximally efficient perspective. METHOD: For the majority of our sella turcica pathologies, we have progressively adopted a uninostril endoscopic approach that is very conservative towards the nasal mucosa with a very limited mucosal incision, resection of the vomer and allo

A 6-year-old girl with an extra-axial mass in the middle cranial fossa.

A 6-year-old Japanese girl presented with psychomotor seizures. Magnetic resonance (MR) images disclosed a mass lesion in the left middle cranial fossa with an internal irregular-shaped area, which was hyperintense on T1-weighted images and hypointense on T2-weighted images. Gross total resection of the tumor was performed through left occipital craniotomy. The tumor was white-to-grayish hard tissue, focally showing blackish pigmentation. Histological, immunohistochemical and electron-microscopical analyses revealed that white-to-grayish hard tissue corresponded to an atypical meningioma, and

Effects of the subcranial approach on facial growth and development.

OBJECTIVE: To analyze the long-term effects of subcranial surgery for anterior skull base tumors on facial growth. STUDY DESIGN AND SETTING: Retrospective study (1994 to 2004) in a university-affiliated hospital. Of 108 patients who underwent a subcranial surgical approach for anterior skull base tumors, six adolescents and five young adults fulfilled study entry criteria for age and follow-up data availability (mean, 3.2 +/- 2 years). Cephalometric x-ray films monitored postoperative facial growth. Anteriorly and posteriorly measured horizontal and vertical maxillary growth were compared wit

Removal of clival chordoma in an adolescent thorough combned pterional transsylvian and anterior temporal approach.

Extensive and aggressive surgical removal is treatment of choice for patients who have chordomas of the cranial base. Well-developed microsurgical techniques, as well as good surgical judgment learned from experience are essential to avoid major morbidity. A 14-year-old female presented with progressive headaches and diplopia for three months. Cranial magnetic resonance imaging demonstrated a well-circumscribed mass in the clival region. The mass was totally excised via right combined pterional transsylvian and anterior temporal approach (+ orbitozygomatic osteotomy). The tumor was located ext

Successful treatment of a skull base malignant rhabdoid tumor with surgery, chemotherapy and gamma Knife-based stereotactic radiosurgery in a young child.

Most childhood rhabdoid tumors occur in the kidney or central nervous system but they can occur in other sites and they usually run an aggressive clinical course. We report a case of an 8-month-old boy with a right temporal bone rhabdoid tumor treated with surgery, chemotherapy and Gamma Knife-based stereotactic radiosurgery. The patient remained alive after 61 months and repeat magnetic resonance imaging (MRI) of the brain showed no evidence of recurrence. There were no obvious endocrine deficits or growth abnormalities at last follow-up. Gamma Knife-based stereotactic radiosurgery may have a

Modified infratemporal fossa approach via lateral transantral maxillotomy: a microsurgical model.

BACKGROUND: Lateral approaches have traditionally been used to gain access to lesions of the infratemporal fossa (ITF). However, dysfunction of the facial nerve secondary to its translocation, conductive hearing loss, and dental malocclusion because of mandibular head resection or dislocation are significant limitations associated with some of these approaches. Although facial nerve translocation and extended maxillotomy approaches avoid some of these drawbacks, they are invasive and require extensive osteotomies and facial incisions. To avoid these potential complications and maintain an extr

Pneumocephalus associated with ethmoidal sinus osteoma--case report.

A 35-year-old female suffered sudden onset of severe headache upon blowing her nose. No rhinorrhea or signs of meningeal irritation were noted. Computed tomography (CT) with bone windows clearly delineated a bony mass in the right ethmoid sinus, extending into the orbit and intracranially. Conventional CT demonstrated multiple air bubbles in the cisterns and around the mass in the right frontal skull base, suggesting that the mass was associated with entry of the air bubbles into the cranial cavity. T1- and T2-weighted magnetic resonance (MR) imaging showed a low-signal lesion that appeared to

Endoscopic Endonasal Craniofacial Surgery for Recurrent Skull Base Meningiomas Involving the Pterygopalatine Fossa, the Infratemporal Fossa, the Orbit, and the Paranasal Sinus.

OBJECTIVE: Skull base meningiomas carry a nonnegligible risk of recurrence. In particular, those arising from the sphenoid wings or middle cranial fossa penetrate into extracranial regions, uncommonly showing massive expansion into the craniofacial regions on recurrence. The role of endoscopic endonasal surgery for those intractable lesions remains unclear. METHODS: We performed endoscopic endonasal craniofacial surgery for 8 recurrent meningiomas invading into the pterygopalatine fossa, infratemporal fossa, nasopharynx, paranasal sinus, or orbit, comprising 2 meningothelial and 1 fibrous men

[Distant metastases to nasal cavities and paranasal sinuses, from the organs outside the head and neck].

INTRODUCTION: The metastases from below the clavicles, to the head and neck region, are relatively uncommon, and usually demonstrate during the progression of the primary disease. At that time, the correct diagnosis requires only to compare the pathology report from the primary biopsy, with the biopsy from the lump in the head and neck. The majority of distant metastases to the head and neck region are localized within the lymph nodes. The metastases to nasal cavity and paranasal sinuses are very rare and usually localized within the maxillary sinus. MATERIAL: 4 cases, out of 46 all distant m

The Role of the Lumbar Drain in Endoscopic Endonasal Skull Base Surgery: A Retrospective Analysis of 811 Cases.

OBJECTIVE: With increasing use of the endoscopic endonasal transsphenoidal approach, wider attention is being paid to treatment and prevention of its complications. The aim of this study was to determine whether lumbar drainage (LD) has been effective in preventing and treating postoperative cerebrospinal fluid (poCSF) leakage or in assisting in achieving tumor gross total removal (GTR). METHODS: Retrospective analysis of purely endoscopic endonasal transsphenoidal cases at a single center between 2008 and 2017 was done. We studied intraoperative cerebrospinal fluid (ioCSF) and poCSF leakage

Pharyngeal Motor Evoked Potential Monitoring During Skull Base Surgery Predicts Postoperative Recovery from Swallowing Dysfunction.

OBJECTIVE: Monitoring pharyngeal motor evoked potential (PhMEP) with a modified endotracheal tube is useful for predicting postoperative swallowing dysfunction. However, the relationship between intraoperative PhMEP findings and recovery from postoperative swallowing dysfunction has not been clarified. The aim of this study was to determine whether PhMEP monitoring predicts swallowing dysfunction not only immediately after surgery but also in the postoperative recovery period. METHODS: We analyzed PhMEPs in 36 patients during treatment for skull base tumors. Recovery from postoperative swallo

Long-term effectiveness of a reconstructive protocol using the nasoseptal flap after endoscopic skull base surgery.

OBJECTIVE: To describe the effect on postoperative cerebrospinal fluid (CSF) leak after anterior skull base (ASB) surgery and complications associated with the addition of the vascularized nasoseptal flap (NSF) to an existing reconstruction protocol. METHODS: A prospective database of all patients undergoing endoscopic ASB approaches was reviewed. Patients were divided into three groups based on the date the use of the NSF was adopted. Group A included patients with high-volume CSF leak closed using the NSF in addition to a multilayer closure. Group B included patients operated on during the

[Biomaterials in reconstructive skull base surgery].

Reconstruction materials and techniques for the skull base have undergone rapid developments and differentiation in recent years. While mostly autotransplants, collagens or resorbable alloplastic materials are preferred for duraplasties, pronounced organ-specific differences can be observed IN the reconstruction of hard tissues. The use of polymethylmethacryl bone cement, once wide-spread, has decreased greatly due to the release of toxic monomers. Bony autotransplants are still used primarily for smaller skull-base defects, intraoperatively formable titanium meshes may be also used for larger
