Heart Failure and Sleep Apnea.

Obstructive and central sleep apnea are far more common in heart failure patients than in the general population and their presence might contribute to the progression of heart failure by exposing the heart to intermittent hypoxia, increased preload and afterload, sympathetic nervous system activation, and vascular endothelial dysfunction. There is now substantial evidence that supports a role for fluid overload and nocturnal rostral fluid shift from the legs as unifying mechanisms in the pathogenesis of obstructive and central sleep apnea in heart failure patients, such that the predominant t…

Obstructive sleep apnoea and arthrogryposis.

Arthrogryposis is a rare condition characterised by multiple congenital joint contractures. We present a case of a 10-year-old child with arthrogryposis and snoring. Polysomnography revealed significant obstructive sleep apnoea and hypoventilation that improved but did not completely resolve with adenotonsillectomy. With continuous positive airway pressure (CPAP) therapy, there was full resolution of all sleep disordered breathing. Initially, the patient admitted to difficulty tolerating nasal CPAP at home. However, she steadily improved adherence to therapy and admitted that with nasal CPAP u…

[Obstructive sleep apnea in women].

The prevalence of sleep disturbances and their symptomatic manifestations may be different in men and women. Women with obstructive sleep apnea are less likely to be diagnosed with sleep apnea compared to men, probably due to atypical symptoms such as morning headaches, symptoms of depression and daytime fatigue. There is a great importance of diagnosis and treatment of sleep disorders regarding quality of life, co-morbidity and mortality in both genders.

Sleep Disturbances and Suicide Risk.

Suicide occurs in the presence of psychiatric illness, and is associated with biological, psychological, and social risk factors. Insomnia symptoms and nightmares appear to present elevated risk for suicidal ideation, attempts, and death by suicide. Failure to account for the presence of psychopathology and frequent use of single item assessments of sleep and suicidal ideation are common methodological problems in this literature. Preliminary research, addressing these issues, suggests that subjective sleep complaints may confer independent risk for suicidal behaviors.

Common sleep disorders in children.

Up to 50% of children will experience a sleep problem. Early identification of sleep problems may prevent negative consequences, such as daytime sleepiness, irritability, behavioral problems, learning difficulties, motor vehicle crashes in teenagers, and poor academic performance. Obstructive sleep apnea occurs in 1% to 5% of children. Polysomnography is needed to diagnose the condition because it may not be detected through history and physical examination alone. Adenotonsillectomy is the primary treatment for most children with obstructive sleep apnea. Parasomnias are common in childhood; sl…

[Sleep disturbance in Parkinson's disease].

Many patients with Parkinson's disease (PD) complain about sleep disturbances. These symptoms originate from motor symptoms, nocturnal problems, psychiatric symptoms, and other sleep disorders including Excessive daytime sleepiness (EDS), REM sleep behavior disorder (RBD), Restless legs syndrome (RLS), and Sleep apnea syndrome (SAS). Especially, RBD is paid attention to prodromal symptoms of PD. Also, one third of patients with PD have RBD symptoms. Moreover, RBD is one of aggravating factors of motor symptoms, autonomic dysfunctions, and dementia. Now, the evidence based medicine for sleep di…

Sleep medicine - prevalent and relevant.

There is an increasing awareness of different sleep disorders among the public and healthcare professionals, and the impact they can have on an individual. This conference was organised jointly with the British Thoracic Society to discuss some of these pertinent conditions, issues around driving and around service planning to accommodate an increasing specialty.

Disrupted nighttime sleep in narcolepsy.

STUDY OBJECTIVES: Characterize disrupted nighttime sleep (DNS) in narcolepsy, an important symptom of narcolepsy. METHODS: A panel of international narcolepsy experts was convened in 2011 to build a consensus characterization of DNS in patients with narcolepsy. A literature search of the Medline (1965 to date), Medline In-Process (latest weeks), Embase (1974 to date), Embase Alert (latest 8 weeks), and Biosis (1965 to date) databases was conducted using the following search terms: narcolepsy and disrupted nighttime sleep, disturbed nighttime sleep, fragmented sleep, consolidated sleep, sleep …

Sleep-wake disorders and dermatology.

Sleep is an active process that occupies about one-third of the lives of humans; however, there are relatively few studies of skin disorders during sleep. Sleep disruption in dermatologic disorders can significantly affect the quality of life and mental health of the patient and in some situations may even lead to exacerbations of the dermatologic condition. Sleep and skin disorders interface at several levels: (1) the role of the skin in normal sleep physiology, such as thermoregulation, core body temperature control, and sleep onset; (2) the effect of endogenous circadian rhythms and periphe…

[Sleep apnea and heart failure].

Sleep apnea can influence cardiac function, by which the development of heart failure is facilitated. Vice versa, chronic heart failure increases the risk for sleep apnea. Consequently, in patients with symptomatic chronic heart failure, sleep apnea is a frequent comorbidity occuring in up to 75% of cases. More than half of those suffer from central sleep apnea, whereas in the general population, obstructive sleep apnea is far more frequent. Both, the obstructive and the central sleep apnea lead to oxygen desaturations during the night which are followed by increases in serum catecholamines. P…

Management of common sleep disorders.

Sleep disorders are common and affect sleep quality and quantity, leading to increased morbidity. Patients with sleep disorders can be categorized as those who cannot sleep, those who will not sleep, those with excessive daytime sleepiness, and those with increased movements during sleep. Insomnia, defined as difficulty initiating or maintaining sleep that results in daytime impairment, is diagnosed using history findings and treated with cognitive behavior therapy, with or without sleep hypnotics. Restless legs syndrome is characterized by an urge to move the legs that worsens with rest, is r…

Sleep apnea and cardiovascular risk.

Sleep apnea is evident in approximately 10% of adults in the general population, but in certain cardiovascular diseases, and in particular those characterized by sodium and water retention, its prevalence can exceed 50%. Although sleep apnea is not as yet integrated into formal cardiovascular risk assessment algorithms, there is increasing awareness of its importance in the causation or promotion of hypertension, coronary artery disease, heart failure, atrial arrhythmias, and stroke, and thus, not surprisingly, as a predictor of premature cardiovascular death. Sleep apnea manifests as two prin…
