Preserving the Modiolus: Enhanced Functional and Aesthetic Restoration with the Modified Bernard-Webster Technique in Lip Reconstruction.

Optimal functional and aesthetic reconstruction of the lip remains a surgical challenge. Traditional Bernard-Webster procedures effectively … 25% and 37.5% of patients, respectively, with complete resolution by 12 months. Oral competence and speech intelligibility were preserved in all patients. Patient and Observer Scar Assessment Scale (POSAS) scores confirmed favorable aesthetic outcomes. This modified technique offers a reliable reconstructive option characterized by modiolus preservation, stable blood su …

Conceptually accurate signed English transliteration: Effects of speaking rate and lag time on production accuracy.

This paper is the fifth in a series concerned with factors affecting the level of access that educational interpreters provide to deaf and hard-of-hearing students. In previous papers, we have examined accuracy and intelligibility of educational interpreters who use Cued Speech (CS) and Signing Exact English (SEE). In this study, accuracy, or the proportion of the message correctly produced by the interpreter, was evaluated in 12 Conceptually Accurate Signed English (CASE) transliterators with varying degrees of experience at 3 different speaking rates (slow, normal, fast). Results were simila

Reconstruction of Tongue Malignancy Defects: A Systematic Review and Treatment Algorithm.

INTRODUCTION: The tongue is the most common site of oral malignancies, with surgical resection the mainstay of treatment. Tongue cancer resections have significant functional impacts. Ideal reconstruction would restore speech intelligibility, facilitate swallowing of liquids and solids without significant dysphagia, and have minimal morbidity. We aimed to review and synthesize published outcomes of tongue reconstructive methods into a useful clinical treatment algorithm. METHODS: A systematic electronic search was conducted in accordance with the Preferred Reporting Items for Systematic Revie…

A new metaphor for speaking of trauma: the toroidal island model.

This article provides a model for speaking of trauma based on the author's ethnographic and clinical experiences mainly in Japan. It is often assumed that a person with more serious trauma has the right and ability to speak out. But those who are at the center of trauma and unable to survive cannot testify. Based on this notion, a toroidal island (TI) model, shaped like a doughnut with a landlocked inner sea, is developed. The voices come from the survivors on the inner slope and from the supporters on the outer slope on the island. The model integrates various aspects of trauma by employing m

Positive auditory cortical responses in patients with absent brainstem response.

OBJECTIVE: To compare the detectability of the different auditory evoked responses in patients with retrocochlear lesion. METHODS: The 40-H … ents (amplitude less than 1fT), but no apparent ASSFs were observed in one patient, in whom maximum speech intelligibility was extremely low and the latency of N1m was most prolonged. CONCLUSION: The N1m response and 40-Hz ASSR could be detected in patients with absent ABR, but the N1m response appeared to be more detectable than the 40-Hz ASSR. SIGNIFICANCE: Combined …

Digital conversation companions.

Advances in voice recognition and artificial intelligence (AI) could soon yield digital conversation companions-software that combines conversational AI with health monitoring and promotion. We describe how this technology, once developed and deployed, could transform the way older adults age and receive care. By obviating the need to read or type, the software could be used by individuals with reduced dexterity, vision, or literacy. Repeated conversations could facilitate the consistent longitudinal tracking of mental acuity and sentiment. Over time, the conversations could be tailored to str

Articulatory Correlates of Stress Pattern Disturbances in Talkers With Dysarthria.

Purpose Reduced stress commonly occurs in talkers with Parkinson's disease (PD), whereas excessive and equal stress is frequently associated with dysarthria of talkers with amyotrophic lateral sclerosis (ALS) and multiple sclerosis (MS). This study sought to identify articulatory impairment patterns that underlie these two impaired stress patterns. We further aimed to determine if talkers with the same stress pattern disturbance but different diseases (ALS and MS) exhibit disease-specific articulatory deficits. Method Fifty-seven talkers participated in the study-33 talkers with dysarthria and

Exploring consequences of short- and long-term deafness on speech production: a lip-tube perturbation study.

Studies have reported strong links between speech production and perception. We aimed to evaluate the role of long- and short-term auditory feedback alteration on speech production. Eleven adults with normal hearing (controls) and 17 cochlear implant (CI) users (7 pre-lingually deaf and 10 post-lingually deaf adults) were recruited. Short-term auditory feedback deprivation was induced by turning off the CI or by providing masking noise. Acoustic and articulatory measures were obtained during the production of /u/, with and without a tube inserted between the lips (perturbation), and with and w

[Application of single-layer forearm flap in the repair of perforating defect of palate].

Objective:To evaluate the feasibility and clinical efficacy of a single-layer radial forearm free flap (RFFF) for through-and-through palatal defects. Methods:The single-layer forearm flap was used to repair 6 cases of perforating palatal defect due to oral malignant tumor. Preoperative Allen test and ultrasound doppler examination were used to judge the forearm vessels. Patients were followed up to evaluate their chewing, swallowing, speech function, surgical outcomes and quality of life. Results:All flaps survived completely (100.0%). At 6 months all patients resumed normal oral intake witho

[A case report of dementia with cluttering-like speech disorder and apraxia of gait].

A 57-year-old woman presented with a slowly progressive gait disturbance in 1992 (53 years of age). Over the next year, she gradually began to talk less, but her speech itself became more rapid than before. He speech was frequently too fast even for family members to understand. In 1997, she was admitted to our hospital. On admission, the patient was disoriented but able to follow simple verbal commands, to name things, and to write simple words. Neither apraxia, aphasia, hemispatial neglect, nor a corpus callosum disconnection syndrome was observed. There was no muscle weakness or atrophy. Sh

Acoustic analysis in stuttering: a machine-learning study.

BACKGROUND: Stuttering is a childhood-onset neurodevelopmental disorder affecting speech fluency. The diagnosis and clinical management of stuttering is currently based on perceptual examination and clinical scales. Standardized techniques for acoustic analysis have prompted promising results for the objective assessment of dysfluency in people with stuttering (PWS). OBJECTIVE: We assessed objectively and automatically voice in stuttering, through artificial intelligence (i.e., the support vector machine - SVM classifier). We also investigated the age-related changes affecting voice in stutte

Using Language Processing and Speech Analysis for the Identification of Psychosis and Other Disorders.

Increasingly, data-driven methods have been implemented to understand psychopathology. Language is the main source of information in psychiatry and represents "big data" at the level of the individual. Language and behavior are amenable to computational natural language processing (NLP) analytics, which may help operationalize the mental status examination. In this review, we highlight the application of NLP to schizophrenia and its risk states as an exemplar of its use, operationalizing tangential and concrete speech as reductions in semantic coherence and syntactic complexity, respectively.

Acoustic and Kinematic Predictors of Intelligibility and Articulatory Precision in Parkinson's Disease.

PURPOSE: This study investigated relationships within and between perceptual, acoustic, and kinematic measures in speakers with and without dysarthria due to Parkinson's disease (PD) across different clarity conditions. Additionally, the study assessed the predictive capabilities of selected acoustic and kinematic measures for intelligibility and articulatory precision ratings. METHOD: Forty participants, comprising 22 with PD and 18 controls, read three phrases aloud using conversational, less clear, and more clear speaking conditions. Acoustic measures and their theoretical kinematic parall

The effect of listener experience and anchors on judgments of dysphonia.

PURPOSE: To determine the effect of auditory anchors on judgments of overall severity (OS) and vocal effort (VE) in dysphonic speech when judgments are made by experienced and inexperienced listeners, and when self-rated by individuals with dysphonia (speaker-listeners). METHOD: Twenty individuals with dysphonia and 4 normal controls provided speech recordings. Speaker-listeners judged their own speech samples for OS and VE without auditory anchors, and then in the presence of anchors, using 100-mm visual analog scales (VAS). Twenty inexperienced and 10 experienced listeners evaluated the sam

A longitudinal study of speech timing in young children later found to have reading disability.

PURPOSE: This study examined the development of timing characteristics in early spontaneous speech of children who were later identified as having reading disability (RD). METHOD: Child-adult play sessions were recorded longitudinally at 2 and 3 years of age in 27 children, most of whom were at high familial risk for RD. For each speaking turn, the number of syllables was determined and an acoustic analysis measured the time allocated to articulation, pausing before speaking, and pausing during speaking. RESULTS: In grade school, a reading battery identified 9 children with RD and 18 childre

Bite block vowel production in apraxia of speech.

PURPOSE: This study explored vowel production and adaptation to articulatory constraints in adults with acquired apraxia of speech (AOS) plus aphasia. METHOD: Five adults with acquired AOS plus aphasia and 5 healthy control participants produced the vowels [i], [epsilon], and [ae] in four word-length conditions in unconstrained and bite block conditions. In addition to acoustic and perceptual measures of vowel productions, individually determined idealized vowels based on each participant's best performance were used to assess vowel accuracy and distinctiveness. RESULTS: Findings showed (a)

Managing the language and learning needs of the communication-impaired preschool child. A proactive approach.

If a proactive approach to assessment and intervention had been used in the case study presented at the beginning of this article, the following might have occurred: The SLP would have asked the parents and brother of the 3 1/2-year-old child referred for a communication evaluation to participate in the assessment activities. The parents would have been asked to prioritize their expectations for their daughter's communication, behavior, and school success. They would have been told the SLP would do the same based on her knowledge of performance expectations in these areas for a 3 1/2-year old.
