PURPOSE OF THE STUDY: The sagittal equilibrium of the spine and pelvis has been examined in numerous studies looking for the origin of certain posture disorders of the spine and the cause of lower back pain. Sagittal x-rays of the pelvis provide an analysis of the degree of inclination of the pelvis from the horizontal and the bi-coxo-femoral axis and information on the form of the sacrum. There is no radiographic parameter however which analyzes the transition between the pelvis-sacrum component and the femoral component, i.e. the periacetabular region. In the sagittal plane, a line tangent t

OBJECT: More information about the association between preoperative anterior translation of the C-7 plumb line and clinical outcomes after decompression surgery in patients with lumbar spinal canal stenosis (LSS) would help resolve problems for patients with sagittal imbalance. The authors evaluated whether preoperative sagittal alignment of the spine affects low-back pain and clinical outcomes after microendoscopic laminotomy.
METHODS: This study was a retrospective review of prospectively collected surgical data. The study comprised 88 patients with LSS (47 men and 41 women) who ranged in a

OBJECTIVE: Bent spine syndrome (BSS), defined as an abnormal forward flexion of the trunk resolving in supine position, is usually related to parkinsonism, but can also be encountered in myopathies. This study evaluates whole-body muscle MRI (WB-mMRI) as a tool for detecting underlying myopathy in non-extrapyramidal BSS.
MATERIALS AND METHODS: Forty-three patients (90 % women; 53-86 years old) with a non-extrapyramidal BSS were prospectively included. All underwent a 1.5-T WB-mMRI and a nerve conduction study. Muscle biopsy was performed if a myopathy could not be eliminated based on clinical

OBJECTIVES: Bent spine syndrome (BSS) is characterized by involuntary forward flexion of the trunk in the standing position but not in the recumbent position. We assessed the causes of BSS based on findings in 63 patients.
METHODS: We retrospectively reviewed the records of all patients with BSS evaluated at a teaching hospital in Toulouse, France, between 1995 and 2006. For each patient, we recorded the findings from the following investigations: physical examination, electromyogram (EMG), creatine phosphokinase (CPK) assay, computed tomography (CT), and surgical muscle biopsy.
RESULTS: We

STUDY DESIGN: A retrospective cohort study.
OBJECTIVE: To evaluate the C2 pelvic angle (C2PA) as a novel compensatory parameter integrating C2 (center of gravity) and pelvic morphology, and its association with physical function and health-related quality of life (HRQOL). The T4-L1-hip axis concept was applied to assess individualized spinal alignment.
BACKGROUND: Adult spinal deformity (ASD) impairs physical function and HRQOL in older adults. Pelvic retroversion compensates for spinal degeneration to maintain posture, but standardized evaluative indices linking pelvic parameters to outcome

UNLABELLED: MINI: Spinal sagittal realignment necessitates a reference posture, and thus far this has only been defined for an "averaged" curve via horizontal offsets from the gravity line (GL). This prospective study of 169 healthy subjects demonstrates normative sagittal spatial orientation of each vertebra, for all Roussouly curve types, using vertebral slopes and horizontal offsets from the GL.
STUDY DESIGN: Prospective study.
OBJECTIVE: To map the healthy standing alignment of the adult spine, grouped according to Roussouly curve types, using both horizontal offset distance from the gra

STUDY DESIGN: Retrospective cohort study.
OBJECTIVES: To investigate spinopelvic alignment and spine shape in patients surgically treated for adolescent idiopathic scoliosis (AIS) and to assess the distribution and clinical applicability of the Roussouly classification.
SUMMARY OF BACKGROUND DATA: How spinopelvic alignment is affected in AIS patients is not well established. Roussouly et al. proposed a classification based on the sagittal spinal profile and spinopelvic alignment that may have clinical utility in these patients.
METHODS: A consecutive cohort of 134 surgically treated AIS pat

INTRODUCTION: The variations of the cervical lordosis after correction of sagittal imbalance have been poorly studied. The aim of our study is to verify whether the cervical lordosis changes after surgery for sagittal imbalance.
MATERIALS AND METHODS: Thirty-nine patients were included in the study. Cervical, thoracic and lumbar spine, pelvic and lower-limb sagittal parameters were recorded. The cranial alignment was measured by the newly described Cranial Slope.
RESULTS: The global cervical kyphosis (preop -43°, postop -31.5°) and the upper (preop -24.1°, postop -20.2°) and lower cervical k

INTRODUCTION: Treatment of spine imbalance by posterior osteotomy is a valuable technique. Several surgical techniques have been developed and proposed to redress the vertebral column in harmonious kyphosis in order to recreate correct sagittal alignment. Although surgical techniques proved to be adequate, preoperative planning still is mediocre. Multiple suggestions have been proposed, from cutting tracing paper to ingenious mathematical formulas and computerised models. The analysis of the pelvic parameters to try to recover the initial shape of the spine before the spine imbalance occurred

OBJECT: Spinal deformities are significant problems in children with spastic cerebral palsy. The treatment of their spasticity by selective dorsal rhizotomy (SDR) may worsen or improve these problems. The purpose of this study was to determine the incidence of and change in degree of thoracolumbar scoliosis, thoracic kyphosis, and lumbar lordosis in children who have undergone SDR.
METHODS: A retrospective review was conducted of children younger than 18 years of age, who had undergone SDR at British Columbia's Children's Hospital from 1987 to 2001 and in whom preoperative and postoperative s

BACKGROUND: Fixed sagittal imbalance (a syndrome in which the patient is only able to stand with the weight-bearing line in front of the sacrum) has many etiologies. The most commonly reported technique for correction is the Smith-Petersen osteotomy. Few reports on pedicle subtraction procedures (resection of the posterior elements, pedicles, and vertebral body through a posterior approach) are available in the peer-reviewed literature. We are aware of no report involving a substantial number of patients with coexistent scoliosis who underwent pedicle/vertebral body subtraction for the treatme

STUDY DESIGN: Rat lumbar spines with posterior destabilizing surgery were evaluated radiographically and histologically.
OBJECTIVES: To create an appropriate rat model showing the vertebral slippage and deformities frequently observed in pediatric patients with spondylolysis, and to clarify their pathogenesis.
SUMMARY OF BACKGROUND DATA: There are controversial hypotheses regarding the pathogenesis of slippage and deformities of the pediatric spine with pars defects. Furthermore, there is no appropriate animal model mimicking those conditions.
METHODS: After posterior destabilizing surgery,

Man, with his erect posture, evolves in a world subject to the laws of gravity. His spine reflects these constraints. The morphology and static of human spine and biomechanical relationships between spine and pelvis are in direct relation with bipedia. Owing to this position the pelvis widened and straightened, characteristic sagittal spinal curves appeared and the perispinal muscles were deeply reorganized. Each pelvis is characterized by an important anatomical landmark: the pelvic incidence that reflects the sagittal morphology of the pelvis. Based on this anatomical characteristic, a chain

Forward head posture (FHP) is a common postural deviation linked to musculoskeletal disorders and altered cervical spine biomechanics. This study used a validated finite element model (C0-T1, THUMS v4.2) to quantify the biomechanical effects of FHP, with validation against cadaveric data for sagittal balance and range of motion in flexion-extension, axial rotation, and lateral bending. Sagittal balance parameters, including craniovertebral angle (CVA), occipital protuberance to C2 (OP-C2), cervical lordosis (C1-C2 and C2-C7), greater occipital nerve (GON), and C2 nerve root (C2-NR), were measu

Pathogenic variants in CCDC22 were initially described in 2012 in a large Australian family with intellectual disability and were subsequently noted to cause a phenotype consistent with the previously described Ritscher-Schinzel syndrome (RSS). The phenotypes of the original family were not described in detail and remains limited phenotypic data reported in medical literature. We detail the phenotypes of the original family, including newly diagnosed family members. With these eight phenotypic descriptions, more than triple the number of individuals for whom detailed clinical information is av

Idiopathic scoliosis (IS) is an abnormality of the vertebral column with a spine curvature of at least 10 degrees. It is the most common spinal deformity in children with a prevalence of 2%-3%, and its aetiology is unknown. Genetic factors are known to play a role and a number of linkage analyses showed associations of various loci. Here we describe a new case of a de novo interstitial deletion 8q11.21q11.2 disrupting SNTG1 gene, identified by array-CGH in a girl with cognitive impairment and a scoliosis that 'appears' like to IS. SNTG1 encodes γ-1 Syntrophin protein that is part of the dystro

The purpose of this study was to introduce infinite models in scoliosis and to analyze personal experience. Based on a three-dimensional patient-specific finite element model of the spine, rib cage, pelvis and abdomen, a parametric individual model of a thoracolumbosacral orthosis was built. Three standard strap tensions (20, 40, 60 N) were loaded on the back of the brace to simulate the strap tension. The I-Scan distribution pressure measurement system was used to measure the pressure of the different regions and the equivalent forces in these regions were calculated. The spinal curve changes

A 12-week growth experiment was conducted to establish the necessity of vitamin C in the nutrition of Heterobranchus longifilis fingerlings. Vitamin C was supplemented at levels of 0 and 50 mg x kg(-1) to a basal diet (42.5% CP), which was fed to triplicate groups of H. longifilis fingerlings. Fish receiving the vitamin C-supplemented diet had significantly improved weight gain (20.7 vs. 16.7 g per fish), feed efficiency ratio (1.03 vs. 1.42), specific growth rate (3.00 vs. 2.74%), protein efficiency ratio (2.26 vs. 1.64), and survival rate (90% vs. 50%). There was a significant decrease in ha
