OBJECTIVE: The authors assess the present position of catatonia in diagnostic classification systems and consider the merits of designating … er than in those with schizophrenia. It is found in many conditions and presents mainly as retarded-stuporous or excited-delirious forms. Catatonia responds to specific treatments, including sedative anticonvulsants (barbiturates and benzodiazepines) and ECT.
CONCLUSIONS: Catatonia can be distinguished from other behavioral syndromes by a recognizable c …

A higher plasma fibrinogen value was observed in patients with stupor or coma compared with those with alert consciousness (p < 0.05). In patients who died during hospitalization, fibrinogen levels were higher than those of subjects who were discharged (p < 0.005). The results indicate that in the early phase of cereb …

OBJECTIVE: To report a fatal case of refractory status epilepticus precipitated by flumazenil use in a mixed benzodiazepine-tricyclic antidepressant overdose.
CASE SUMMARY: A 39-year-old woman was brought to the emergency room (ER) in a stupor from a suspected suicidal overdose of an unknown mixture of drugs. Past medical history included seizures and psychiatric disorders managed with benzodiazepine and tricyclic antidepressants. Initial ER electrocardiogram showed a QRS interval of 136 milliseconds. The patient developed refractory seizures after being given flumazenil. Lorazepam, phenytoin…

Differences in response to behavioral and electroencephalographic parameters were delineated between two inbred rat strains, Lewis (LEW) and Fischer 344 (F344), upon acute exposure to IV administration of ethylketocyclazocine (EKC) at doses of 1.25, 2.5, and 5 mg/kg. Duration of EKC-induced EEG slow-wave bursts and associated behavioral stupor increased in a dose-related manner, and was greater in duration for the LEW animals; a less robust, quadratic trend was displayed by the F344 group. For both rat strains, latency to onset of slow-wave sleep increased proportionately with EKC dose. Durati…

BACKGROUND AND PURPOSE: We examined whether the pattern of cerebral hemorrhage changed after the introduction of computed tomography.
METHO … ntroduction of computed tomography. This was basically due to a decreased incidence of comatose and stuporous patients with severe neurological deficit paralleled by a decrease of mean systolic and diastolic blood pressure values at admission.

A case of generalized choreic movement associated with subarachnoid hemorrhage is reported. A 71 year-old hypertensive woman suddenly developed severe headache 14 days before admission. Consciousness disturbance and involuntary movement involving the face and upper extremities appeared about 8 days after onset. The involuntary motion was diagnosed as generalized choreic movement. CT scans showed subarachnoid hemorrhage with ventricular dilatation and periventricular lucency involving bilateral caudate nuclei. On admission the patient was stuporous with Hunt & Kosnik Grade 4. She showed involun…

Six patients were admitted after erroneous massive intake of levothyroxine (70-1200 mg over an interval of 2-12 days). All patients developed classical symptoms of thyrotoxicosis within 3 days of the first dose; five patients presented grade II-III coma and one became stuporous (days 7-10). Two patients developed left ventricular failure and three had arrhythmias (days 8-11). Total thyroid hormone levels in serum on admission ranged 935-7728 nmol/l for T4 (TT4) and 23-399 nmol/l for T3 (TT3). All patients received treatment with hydrocortisone and Propranolol. Propylthiouracil was also given i…

Tricyclic antidepressant overdose is widely felt to be associated with cardiac arrhythmias which may occur without warning, sometimes late in the clinical course. For this reason, many institutions routinely monitor cardiac rhythm in such patients for up to 72 h. A retrospective study was carried out to analyze the clinical course of such patients with emphasis on cardiac complications. Thirty-eight cases of TCA overdose were reviewed. Fourteen patients (36.9%) were classified as lethargic or stuporous on admission while 23 (60.5%) were either comatose or semicomatose, nine requiring endotrach…

We represent a case of cystic dilatation of the fourth ventricle resulted from obstruction of the foramina of Luschka and Magendie and of the aqueduct of Sylvius which developed after a surgery of AVM. A 42-year-old woman was admitted to our clinic for the operation of AVM located at the trigone of lateral ventricle and the lateral wall of midbrain. VPS had been performed previously because of acute hydrocephalus resulted from several attacks of ventricular hemorrhage. Total removal of the AVM was performed. The patient was stuporous after the surgery, and CT scan showed marked distension of t…

Basilar artery perforator aneurysms (BAPAs) are rare causes of spontaneous subarachnoid hemorrhage and may be occult on initial vascular imaging. We report a 55-year-old woman who presented with headache and was found to have subarachnoid hemorrhage (modified Fisher grade 3, Hunt-Hess grade 2). Initial computed tomography angiography, same-day digital subtraction angiography, and follow-up magnetic resonance angiography on hospital day 4 were negative. On hospital day 6, her mental status deteriorated to stupor, and computed tomography demonstrated a new hemorrhage in the interpeduncular and s…

BACKGROUND AND PURPOSE: Delirium frequently complicates acute stroke and worsens outcomes. Because delirium is potentially preventable, predicting its occurrence is essential. Although several prediction scores have been proposed, nurses need to quickly predict delirium in stroke care units (SCUs). We aimed to develop a simple tool for this purpose by examining a comprehensive set of potential predictors.
METHODS: This is a prospective cohort study on acute stroke patients admitted to an SCU. Patients without stupor, coma, or delirium upon admission were eligible. Participants were followed f…

INTRODUCTION: Bilateral thalamic strokes are rare manifestations of posterior circulation infarcts. Usually the etiology is cardioembolic or small vessel disease combined with individual anatomical predisposition. The symptoms include a variety of neurological deficits depending on thalamic structure involvement, such as paresthesias or numbness, hemiparesis with increased reflexes and Babinski sign, third cranial nerve palsy, speech and cognition disturbance, memory impairment and stupor. Neuroimaging usually reveals ischemic loci in adequate thalamic nuclei.
CASE PRESENTATION: We report a c…

BACKGROUND: Supratentorial intracerebral hematomas often are evacuated in rapidly deteriorating patients. Surgery may prevent death but not necessarily disability. The authors studied the outcome of emergent clot evacuation in patients with worsening massive intracerebral hemorrhage.
METHODS: The authors reviewed data on 26 consecutive, acutely worsening patients with nontraumatic intracerebral hemorrhage who had surgery for clot evacuation. All patients had clinical (stupor or coma, loss of pontomesencephalic brainstem reflexes, extensor posturing) or radiologic (midline shift of septum pell…

Coma is the disorder of consciousness because of the damage to diffused bilateral cerebral hemisphere cortex or reticular activating system. Coma can be caused by neurogenic (head brain injury), metabolic (endogenic), and toxic (exogenic) factors. To determine the cause of metabolic and toxic coma, laboratory tests are performed; in case of neurogenic coma, the neurologic examination is essential, when five systems are evaluated: the level of consciousness (according to Glasgow Coma Scale or Full Outline of Unresponsiveness Scale), photoreaction of pupils and ophthalmoscopic examination, oculo
