Rerupture after flexor tendon repair of the hand and wrist: a retrospective risk factor analysis.

INTRODUCTION: Rerupture after primary flexor tendon repair of the hand and wrist is a serious complication that can substantially impair hand function and restrict activities of daily living, occupational performance, and recreation. Identifying factors associated with rerupture is essential to optimize surgical techniques and postoperative rehabilitation strategies and to improve patient outcomes. METHODS: A retrospective observational cohort study was conducted including patients who underwent surgical repair of flexor tendon injuries of the hand and wrist at a single institution between 201…

[Ischemic contracture of the forearm and hand. Staging and indications for surgical treatment].

Results of long-term follow up of 66 patients with ischemic contracture of the forearm and hand, all surgically treated in the "Unfallkrankenhaus Hamburg)) between 1961 and 1982, are presented. Whereas ischemic contracture of the forearm flexors resulted mainly from fractures, ischemic contracture of the intrinsic muscles of the hand was most often seen after pressure injuries. All patients in this study presented to us with fully established ischemic contractures. Muscle and nerve damage was retrospectively evaluated according to operative notes, and the degree of damage could be classified i

Current treatment concepts for Achilles tendon rupture.

Achilles tendon rupture is a common and primary cause of lower limb tendon injury suffered during sports-related activities. The causes of Achilles tendon rupture include the calf muscle and tendon overuse, poor tendon quality, and various medical conditions. Historically, acute Achilles tendon rupture was treated conservatively. However, historical techniques are now associated with an increased risk of rerupture. To address this problem, open repair has been proposed. Open repair is associated with a reduced risk of rerupture; however, it is also closely associated with wound complications,

Traumatic dislocations of the peroneal tendons.

Traumatic dislocation of the peroneal tendons is an often unrecognized injury which has been reported to occur most commonly during snow skiing. The strength of the peroneal retinaculum is exceeded during resistance to violent passive dorsiflexion or to inversion stress. Pain, swelling, and ecchymosis may hinder early diagnosis; however, intense retromalleolar pain on active eversion is a specific, highly suggestive finding. Fracture of a thin shell of the lateral malleolar cortex is diagnostic. In chronic cases, marked dislocation of the tendons is frequently demonstrable, with more than the

Acute Peroneal Injury.

A high clinical suspicion and greater understanding of the anatomy and pathophysiology of lateral ankle injuries have enabled early diagnosis and treatment-improving outcomes of acute peroneal tendon tears. Multiple conditions can be the cause of lateral ankle pain attributed to the peroneal tendons: tenosynovitis, tendinosis, subluxation and dislocation, stenosing tenosynovitis, abnormality related to the os peroneum, as well as tears of the peroneal tendons. It is imperative for the clinician to maintain a high suspicion for peroneal tendon abnormality when evaluating patients with lateral a

Complications of plate fixation in the hand skeleton.

Plate fixation of metacarpal and phalangeal fractures is designed to provide rigid internal fixation in order to facilitate early motion and thereby minimize joint and tendon complications. In a series of plate fixations, 16 of 38 (42%) of proximal phalangeal and metacarpal shaft fractures developed complications of stiffness, malunion, nonunion, and tendon rupture. Complications occurred more frequently for phalangeal than for metacarpal fractures and more frequently when there were associated bone or soft tissue injuries. The technique is demanding and secondary procedures are frequently req

Direct reconstruction of chronic extensor digitorum longus tendon rupture using interposed scar tissue in the foot: A case report.

RATIONALE: Primary repair of acute ligament injury is possible due to the proximity of the ends. In the case of chronic injury, however, primary repair is difficult because the ends of ruptured ligament will have receded, and tendon graft, transfer, or reconstruction is needed. Satisfactory clinical results have been reported after reconstruction with newly formed interposed scar tissue between the ends of the ruptured tendon in chronic Achilles tendon injury and chronic extensor halluces longus (EHL) tendon injury. Here, we report a patient treated with reconstructive surgery using well-forme

Ultrasound Detection of Patellar Fracture and Evaluation of the Knee Extensor Mechanism in the Emergency Department.

Traumatic injuries to the knee are common in emergency medicine. Bedside ultrasound (US) has benefits in the rapid initial detection of injuries to the patella. In addition, US can also quickly detect injuries to the entire knee extensor mechanism, including the quadriceps tendon and inferior patellar ligament, which may be difficult to diagnose with plain radiographs. While magnetic resonance imaging remains the gold standard for diagnostic evaluation of the knee extensor mechanism, this can be difficult to obtain from the emergency department. Clinicians caring for patients with orthopedic i

Flexor Pollicis Longus Tendon Rupture and Carpal Tunnel Syndrome due to Scaphoid Nonunion Advanced Collapse: A Case Report.

CASE: We report a case of flexor pollicis longus (FPL) tendon rupture and carpal tunnel syndrome due to scaphoid nonunion advanced collapse deformity. Intraoperative findings showed disruption of the palmar joint capsule and a sharp proximal bone fragment protruding into the carpal tunnel. Removal of this proximal fragment and tendon grafting were performed. At the postoperative 2-year follow-up, the patient had no wrist pain, finger numbness, or restriction of thumb motion. CONCLUSION: Our results suggest that minimally invasive surgical procedures, such as proximal pole or osteophyte resect

A rare unilateral variation of extensor digitorum longus and fibularis tertius muscles.

PURPOSE: Both the extensor digitorum longus (EDL) and fibularis tertius (FT) muscles exhibit a wide range of morphological variations. METHODS: A 70-year-old male cadaver was subjected to routine anatomical dissection for research and teaching purposes. RESULTS: The EDL had a markedly smaller fleshy belly, whereas the FT appeared hypertrophic. The EDL tendon displayed an atypical elongation and bifurcated into only three distinct slips, which inserted into the dorsal digital expansions of the second, third, and fourth toes. In contrast, the FT tendon was remarkably well-developed and divided

Chondrogenic differentiation and lubricin expression of caprine infraspinatus tendon cells.

Reparative strategies for the treatment of injuries to tendons, including those of the rotator cuff of the shoulder, need to address the formation of the cartilage which serves as the attachment apparatus to bone and which forms at regions undergoing compressive loading. Moreover, recent work indicates that cells employed for rotator cuff repair may need to synthesize a lubricating glycoprotein, lubricin, which has recently been found to play a role in tendon tribology. The objective of the present study was to investigate the chondrogenic differentiation and lubricin expression of caprine inf

Tibialis posterior tendon transfer for persistent drop foot after peroneal nerve repair.

Despite advances in the surgical treatment of peroneal nerve injuries, a significant fraction of patients do not recover adequately. Among 35 patients who had previous repair of the peroneal nerve, 19 had permanent drop foot, and 16 of these patients underwent tibialis posterior (TP) tendon transfer. Mean duration of paralysis was 26.7 (range, 7 to 192) months. TP tendon was carried to the anterior compartment via the circumtibial route, and then attached to the tibialis anterior, extensor hallucis longus, extensor digitorum communis, and peroneus tertius tendons using tendon-tendon anastomosi

Hamstring muscle-tendon unit lengthening and activation in instep and cut-off kicking.

Hamstring muscle strain injury is one of the most common injuries in sports involving sprinting and kicking. Studies examining hamstring kinematics and activations are rich for sprinting but lacking for kicking. The purpose of this study was to examine kinematics and activations of hamstring muscles in instep and cut-off kicking tasks frequently performed in soccer. Videographic and electromyographic (EMG) data were collected for 11 male soccer-majored college students performing the two kicking tasks. Peak hamstring muscle-tendon unit lengths, elongation velocities, and maximum linear envelop

Knee Extensor Mechanism Repairs: Standard Suture Repair and Novel Augmentation Technique.

Patellar and quadriceps tendon ruptures are relatively common injuries. Rupture of the knee extensor mechanism may occur because of a forceful eccentric contraction of the quadriceps against a resisted flexed knee, though atraumatic cases have also been described. Patients at higher than normal risk for knee extensor mechanism rupture include those with systemic co-morbidities, fluoroquinolone use, and chronic tendinopathy. Early operative treatment and mobilization of acute extensor mechanism ruptures has proven effective, and numerous techniques have been described. A 46-year-old male on ana

Reliability of Continuous Shear Wave Elastography in the Pathological Patellar Tendon.

OBJECTIVES: Patellar tendon injuries occur via various mechanisms such as overuse, or due to surgical graft harvest for anterior cruciate ligament reconstruction (ACLR). Quantified patellar tendon stiffness after injury may help guide clinical care. Continuous shear wave elastography (cSWE) allows for the assessment of viscosity and shear modulus in tendons. The reliability of the measure, however, has not been established in the patellar tendon. The purpose of this study was to investigate the interrater reliability, intrarater reliability, and between-day stability of cSWE in both healthy an…

Sex Differences in Achilles Tendon Loading in Healthy Recreational Runners: A Cross-Sectional Study.

CONTEXT: Running-related Achilles tendon injuries have been observed more frequently in males, which may reflect sex differences in tendon morphology or mechanics during running. Males exhibit a larger Achilles tendon cross-sectional area and some measures of Achilles tendon loading in running, but sex differences in measures of tendon loading have not been thoroughly characterized. This study examines sex differences in the Achilles tendon cross-sectional area and Achilles tendon loading variables associated with running kinetics and kinematics. DESIGN: Cross-sectional. METHODS: Recruited f…

Surgical treatment of proximal ruptures of the rectus femoris in professional soccer players.

INTRODUCTION: Muscle injuries are very common in athletes. Depending on the type of sport, muscle injuries represent approximately 9-54% of total injuries. The rectus femoris muscle is very important in specific sports, such as soccer and American football, because it is the muscle involved in movements, such as sprinting and kicking the ball. Therefore, we believe that these disabling injuries need special and prompt treatment to enable athletes to return to their sporting activities. AIM: The aim of our study is to assess the results of surgical treatment of ruptures of the rectus femoris i
