A new procedure is described for cleft hands with absence of the middle finger associated with a palmar cleft. Three skin flaps are made within the cleft. The index ray is transferred ulnarward--not merely shifted ulnarly, but supinated--to prevent overlapping of the fingers when they flex. Length of the finger ray can be adjusted by shortening or lengthening the second metacarpal. The common extensor tendons to the index and ring fingers are interconnected with a free tendon graft to prevent the fingers from excessive spreading. The appearance is improved without disturbing the function of th

Septic peroneal tenosynovitis is a rare and significant challenge. A search of peer-reviewed published studies revealed only 5 case reports to guide treatment, none of which resulted in significant loss of both peroneal tendons necessitating reconstruction. No clear guidance is available regarding how to provide reliable reconstruction of both peroneal tendons after a significant loss secondary to septic tenosynovitis. In the present report, we describe the case of a young, active-duty soldier who underwent lateral ankle ligament reconstruction with a tendon allograft whose postoperative cours

BACKGROUND: The high retear rate post tendon-bone reconstructive surgery is attributed to scar tissue replacing fibrocartilage, which compromises mechanical properties. Macrophages play a pivotal role in this process. Bone marrow mesenchymal stem cells-derived exosome (BMSCs-Exo) have emerged as a promising biological therapeutic approach. However, their role and underlying mechanisms in regulating macrophage polarization and tendon-bone healing require further investigation.
METHODS: Primary macrophages were cultured, and a rat Achilles tendon-calcaneus reconstruction model was established,

PURPOSE: Injury to the distal triceps brachii tendon is rare. Imaging radiographs are used to confirm the findings of physical examination, classify the extent of injury, and guide treatment. Magnetic resonance imaging (MRI) is considered the gold standard of diagnostic imaging. However, no previous study has reported on the accuracy of differentiation between partial- and full-thickness triceps tendon tears. Our study's aim was to define the accuracy of MRI in differentiating partial- from full-thickness tear of the distal triceps tendon. We hypothesized that MRI has low accuracy in different

BACKGROUND: Secondary reconstruction of the extensor pollicis longus tendon (EPL) can be performed using an intercalated tendon graft (ITG) or a tendon transposition, preferably the extensor indicis transposition (EIT), since a secondary suture of the EPL tendon is not possible in most cases due to degenerative changes or defect injuries. A direct comparison of ITG and EIT in one single patient collective based on a standardized follow-up protocol has not been performed yet.
PATIENTS AND METHOD: For the first time, these two operative procedures were directly compared in a retrospective analy

Two patients with a deviated overlapping second toe due to rupture of the dorsal lateral metatarsophalangeal (MTP) collateral ligament and the lateral interosseous tendon were diagnosed, analyzed, and treated. Athletic activities were responsible for the tendon-ligament ruptures and the ensuing medial deviation of the second toe. Pain occurred in the second MTP joint, and a dorsal corn formed over the proximal interphalangeal joint (PIP). Reconstructive procedures included lengthening of the interosseous-lumbrical tendons and the collateral ligament on the medial side of the joint; a dorsal ca

OBJECTIVE. The purpose of this article is to review the anatomy and pathology of the pes anserinus to increase the accuracy of imaging interpretation of findings affecting these medial knee structures. CONCLUSION. The pes anserinus, consisting of the conjoined tendons of the sartorius, gracilis, and semitendinosus muscles and their insertions at the medial aspect of the knee, is often neglected during imaging assessment. Common pathologic conditions affecting the pes anserinus include overuse, acute trauma, iatrogenic disorders, and tumors and tumorlike lesions.

The purpose of this study was to determine the recovery potential of the posterior tibial muscle after late reconstruction following tendon rupture in stage II of posterior tibial tendon dysfunction. Fourteen patients (18 women, 6 men; mean age 59.8 years) were investigated 47 months (range, 24-76 months) after surgical reconstruction of a completely ruptured posterior tibial tendon (end-to-end anastomosis, side-to-side augmentation with the flexor digitorum longus tendon) in combination with a distal calcaneal osteotomy with a tricortical iliac crest bone graft for lengthening of the lateral

A proximally based transfer of a composite structure of the iliotibial tract, its osseous insertion, the lateral retinaculum, and part of the vastus lateralis was used in 111 anterior cruciate-deficient knees that were followed for two to seven years. No other ligament procedures were performed simultaneously. The clinical results included a negative Lachman test in 81 per cent (ninety) of the knees, an anterior drawer test of 1+ or less in 84 per cent (ninety-three knees), and a negative pivot-shift sign in 94 per cent (104 knees). Subjectively, 74 per cent (eighty-two) of the knees were rate

BACKGROUND: Irreparable subscapularis (SSc) tears alter the dynamic force coupling of the shoulder, resulting in pain, weakness, and impaired shoulder function. Pectoralis major (Pma), pectoralis minor (Pmi), and latissimus dorsi (LD) transfers are treatment options for irreparable SSc tears, but clinical outcomes vary. The purpose of this study was to compare the biomechanical properties of Pma, Pmi, and LD transfers in an SSc-deficient shoulder using a computational model.
METHODS: A computer shoulder model was used to investigate the moment arms of Pma, Pmi, and LD tendon transfers compare…

The EHL tendon transfers were performed in both cases, with favorable short-term results. Therefore, the EHL tendon transfer might be preferable in case of the EHL muscle contracture due to partial ACS.

Replantation still remains the best form of reconstruction following major upper limb amputations. Regaining a functional limb is a challenge in avulsion amputations when there is entire loss of a compartment as it happens in proximal third of forearm amputations or when the avulsion occurs through the musculotendinous junction. In these circumstances, primary repair of the long flexors or extensors is not possible and options of secondary tendon transfers do not exist due to lack of donor tendons. These factors could weigh in negatively in making the decision for replantation at the time of p…

BACKGROUND: Few studies have reported midterm outcomes after single-stage flexor digitorum longus (FDL) tendon transfer to the lateral foot for irreparable rupture of the peroneal tendons.
METHODS: Over a 7-year period (2008-2015), 25 consecutive patients underwent transfer of the FDL to the fifth metatarsal for irreparable peroneal tendon tears. Of these, 15 patients were available for inclusion with a mean follow-up of 53.7 ± 23.3 months, mean age at surgery of 48.4 years, and mean body mass index (BMI) of 29.8 kg/m2. Patients completed the pain visual analog scale (VAS), Foot Function Inde…

Trapeziometacarpal (TMC) dislocations are rare injuries that can lead to chronic instability, pain, and early degenerative changes if not properly addressed. While multiple surgical techniques exist for stabilizing the TMC joint, there is ongoing debate regarding the optimal approach. The dorsoradial ligament (DRL) has been increasingly recognized as the primary stabilizer of the TMC joint, and its insufficiency is a key contributor to post-traumatic instability. Traditional reconstruction techniques, such as the Eaton-Littler method, often involve tendon transfers, which introduce donor site …
