Clinical implementation of full Monte Carlo dose calculation in proton beam therapy.

The goal of this work was to facilitate the clinical use of Monte Carlo proton dose calculation to support routine treatment planning and delivery. The Monte Carlo code Geant4 was used to simulate the treatment head setup, including a time-dependent simulation of modulator wheels (for broad beam modulation) and magnetic field settings (for beam scanning). Any patient-field-specific setup can be modeled according to the treatment control system of the facility. The code was benchmarked against phantom measurements. Using a simulation of the ionization chamber reading in the treatment head allow

Spatial Distribution of Cortical Output Zones Affecting Combinations of Forelimb Muscles in the Monkey.

1. We investigated the dimensions of output zones affecting specific combinations of forelimb muscles in the precentral "motor" cortex of macaque monkeys. Single-pulse intracortical microstimulation (S-ICMS) was used to evoke subthreshold effects in multiple wrist and finger muscles. S-ICMS consisted of 15 Hz stimulus trains at low intensity (5 μA) to avoid spread of effects due to temporal summation and to activate circumscribed foci of cortical cells in the bank of the precentral gyrus. 2. To detect subthreshold effects on active muscles, stimuli were delivered during wrist movements against

Objective perimetry and diabetic retinopathy progression: a 10-year follow-up study.

PURPOSE: We investigated 10-year retinal function changes in persons with type 2 diabetes (PWT2D) using the objectiveFIELD Analyser (OFA) to measure per-region sensitivity and response delay. We also examined which baseline measures predicted diabetic retinopathy (DR) progression. METHODS: Participants underwent a comprehensive anterior and posterior segment examination at both visits. Participants were examined by four OFA test methods differing in visual field eccentricity and test duration, each producing per-region sensitivities and response delays. DR severity was graded using Early Trea

Personalized tourniquet pressure versus uniform tourniquet pressure in orthopedic trauma surgery of extremities: A prospective randomized controlled study protocol.

BACKGROUND: In the field of orthopedic surgery, tourniquets are often used to achieve a clear operative field, expedite operations, and minimize hemorrhagic events. However, determining the optimal tourniquet inflation pressure is a topic of debate. The current approach involves using a constant tourniquet pressure, although this is associated with the potential to augment the risk of tourniquet-associated complications. The Association of Surgical Technologists recommends a tourniquet pressure of systolic blood pressure plus 50 mm Hg for the upper limb and 100 mm Hg for the lower limb. Nevert

Towards the automation of early-stage human embryo development detection.

BACKGROUND: Infertility and subfertility affect a significant proportion of humanity. Assisted reproductive technology has been proven capable of alleviating infertility issues. In vitro fertilisation is one such option whose success is highly dependent on the selection of a high-quality embryo for transfer. This is typically done manually by analysing embryos under a microscope. However, evidence has shown that the success rate of manual selection remains low. The use of new incubators with integrated time-lapse imaging system is providing new possibilities for embryo assessment. As such, we

Complexity begets simplicity: self-supervised learning for palaeontological images with few or no labels.

Palaeontology has seen widespread and growing use of machine learning (ML) to classify and analyse large datasets of fossils. However, palaeontology is a challenging field in which to apply ML. Datasets may be small or unlabelled, images may be complex and different from standard datasets and palaeontologists may lack specialist training and access to necessary computational resources. We show how these challenges can be addressed by using recent developments in self-supervised learning (SSL). Using a frozen DINOv3 feature extractor and a simple linear classifier, with reduced data, we can ach

Selective and Fast Cortisol Sensing in Human Saliva Using Graphene-Based On-Chip Field-Effect Transistors.

Monitoring of biomarkers in body fluids is highly sought after for medical diagnostics and tracking health conditions. Conventional analytical methods require centralized facilities and complex protocols, limiting applicability for real-time or point-of-care use. In recent years, graphene-based transducers have shown high potential for miniaturized biosensing, however they have so far been hardly tested in real-case scenarios. In this work, electrolyte-gated graphene field-effect transistors (EG-GFETs) are developed for the detection of the primary stress hormone cortisol in human saliva. The

Development of an augmented virtuality framework using user-defined passthrough surfaces.

An augmented virtuality framework is presented in which the physical scene is admitted only through developer-defined geometric apertures, while all other pixels are rendered virtually. The scene initialises as a black field and uses surface-projected, overlay-style compositing so that the camera stream is visible only on circular or square meshes registered as projection surfaces. Tracking origin is configured to minimise unintended recentring, and the same method can operate either as a minimal black scene or within an optional three-dimensional environment. Main features of the framework ar

Time-resolved directional organization of brain networks: distinct strength and temporal signatures from neurophysiology to disease.

Brain function depends on time-resolved directional organization: influence is continuously redistributed across regions, shaping who drives whom, when, and for how long. Yet dynamic neuroimaging has largely emphasized undirected fluctuations in functional connectivity, limiting biological interpretation of asymmetric interactions. Here we show that directed influence can be resolved into two biologically distinct signatures: strength, reflecting the efficacy of influence, and duration, reflecting its temporal extent. To test this principle, we developed sliding-window prediction correlation (

Bringing usability testing to the front lines: IV smart pump evaluation with critical care nurses at a national conference.

Intravenous smart pump (IVSP) usability evaluations are often conducted in single-site laboratory or simulation settings, which can limit access to experienced frontline nurses. This methods article describes a field-deployable, conference-based approach for evaluating IVSP usability with practicing critical-care nurses. The method brought a standardized simulation environment to a national critical-care nursing conference while preserving experimental control through consistent pump stations, scripted clinical scenarios, counterbalanced device order, and common data-collection procedures. Pallidal alpha activity is an electrophysiological biomarker of symptom severity in obsessive-compulsive disorder.

Clinical efficacy of deep brain stimulation (DBS) for obsessive-compulsive disorder (OCD) remains limited by the absence of objective neural biomarkers to guide targeting, programming, and longitudinal optimization of therapy. Using sensing-enabled DBS devices, we analyzed intracranial local field potentials recorded longitudinally from thirteen patients with treatment-refractory OCD implanted in the ventral capsule/ventral striatum (VC/VS). We identify the periodic component of alpha-band activity (7-14 Hz) as a robust electrophysiological correlate of OCD symptom severity. At the initial cli

Optimization of enzyme-linked immunosorbent assay for analyzing surface structures of nervous necrosis virus and detecting virus-specific antibodies: A technical review.

Enzyme-linked immunosorbent assay (ELISA) is a quantitative immunoassay used to detect antigens and antibodies. In the field of fish diseases, the usefulness of ELISA has been largely overlooked due to its low reproducibility and high background optical density (OD). Nevertheless, ELISA is indispensable for evaluating specific immunity in vaccinated fishes, tracking infection history and analyzing the conformational structures and functions of the surface proteins of fish pathogens. Therefore, a highly quantitative and reproducible ELISA is important and necessary for fish disease research and

The ADVANCE toolkit: Automated descriptive video annotation in naturalistic child environments.

Video recordings are commonplace for observing human and animal behaviours, including interindividual interactions. In studies of humans, analyses for clinical applications remain particularly cumbersome, requiring human-based annotation that is time-consuming, bias-prone, and cost-ineffective. Attempts to use machine learning to address these limitations still oftentimes require highly standardised environments, scripted scenarios, and forward-facing individuals. Here, we provide the ADVANCE toolkit, an automated video annotation pipeline. The versatility of ADVANCE is demonstrated with schoo

A Rare Case of Hunter Syndrome (Mucopolysaccharidosis II) With Bilateral Maculopathy Associated With Rod-Cone Dystrophy.

Hunter syndrome is an X-linked recessive lysosomal storage disorder that is caused by a mutation in the iduronate sulfatase gene. Both anterior and posterior segment abnormalities are found as a result of the accumulation of glycosaminoglycans in ocular tissues. Retinal dystrophy, particularly rod-cone dystrophy, has a major effect on visual acuity, leading to significant visual impairment as the condition worsens. We report a case of a 53-year-old male patient of Asian descent previously diagnosed with Hunter syndrome, who presented with progressive difficulty in visual tracking and colour re

High-resolution tracking of unconfined zebrafish behavior reveals stimulatory and anxiolytic effects of psilocybin.

Serotonergic psychedelics are emerging therapeutics for psychiatric disorders, yet their underlying mechanisms of action in the brain remain largely elusive. Here, we developed a wide-field behavioral tracking system for larval zebrafish and investigated the effects of psilocybin, a psychedelic serotonin receptor agonist. Machine learning analyses of precise body kinematics identified latent behavioral states reflecting spontaneous exploration, visually-driven rapid swimming, and irregular swim patterns following stress exposure. Using this method, we found that acute psilocybin treatment has

Running-In Behavior and Failure Mechanism Between AgCuNi Alloy and Au-Electroplated Layer.

To avoid wear and tear of the slip ring due to electrical corrosion, the slip ring needs to undergo the running-in process under atmospheric conditions without current after assembly. To address the urgent demand for long-service capability space conductive slip rings in the aerospace field, the running-in behavior and failure mechanism between the AgCuNi alloy and Au-electroplated layer are investigated using a ball-on-disc tribometer in this paper. The results show that the transfer film composed of Au plays an important role in modifying the friction during the sliding process. With the acc

Geochemical Decoupling of Iron and Zinc during Transformation of Zn-Bearing Ferrihydrite in Reducing Sediments.

The transformation of the mineral ferrihydrite in reducing environments, and its impact on the mobility of incorporated trace metals, has been investigated in model laboratory studies, but studies using complex soil or sediment matrices are lacking. Here, we studied the transformation of zinc (Zn)-bearing ferrihydrite labeled with 57Fe and mixed with natural sediments, incubated in reducing conditions for up to six months. We tracked the evolution of Fe and Zn speciation with 57Fe Mössbauer spectroscopy and with bulk and micro-X-ray absorption spectroscopy. We show that Fe was readily reduced
