Predictors of postextubation stridor in pediatric trauma patients.

OBJECTIVE: To determine which factors are the best predictors of postextubation stridor in pediatric trauma patients. DESIGN: Prospective cohort study. SETTING: The Burn and Trauma ICUs at Harborview Medical Center from March to September 1989. PATIENTS: Children were eligible for the study if they were less than 15 yr old, were intubated for greater than 12 hr, and did not have underlying cardiopulmonary disease. The study included 25 patients with 30 extubations. RISK FACTORS ASSESSED: Age, type of injury (burn vs. trauma), location of intubation ("field" vs. hospital), endotracheal tube

Patterns of injury in children.

Trauma is the leading cause of death for children over 1 year of age. This study was undertaken to identify the patterns of injury among children admitted to a regional pediatric trauma center. During a 34-month period, 3,472 injured children were consecutively admitted to a regional pediatric trauma center. Data were collected on medical, etiological, and financial aspects of injury. Eight subgroups were defined by mechanism of injury: motor-vehicle crash occupants, pedestrian and cycle injuries, falls, child abuse, gunshot and stab wounds, burns, poisonings, and foreign body ingestions or as

[Trauma networks of the future: where is the trend headed?].

The idea of establishing certified trauma networks to ensure nationwide, high-quality care for severely injured patients emerged in the early 2000s. Since then, it has been continuously developed. Currently, 278 local, 240 regional, and 122 supraregional trauma centers in 48 trauma networks are certified in Germany. An increasing number of other countries are adopting the TraumaNetwork within their own structures. The far-reaching changes currently taking place in German health policy, the integration of other countries with different healthcare systems into the TraumaNetwork, and the growing …

Applying Trauma Informed Care in Multidisciplinary Cleft Practices: Defining Pediatric Cleft Trauma and Providing Actionable Recommendations.

Cleft lip and/or palate (CL/P) is a life-long condition that requires multidisciplinary care throughout childhood and beyond. Studies have examined the impact of CL/P on psychosocial outcomes including lower self-esteem, higher incidence of bullying, social isolation, and rejection. As a result, patients may be predisposed to experiencing mood disorders or trauma related to their facial difference. Moreover, access to dedicated cleft centers is limited in many areas in the United States. For children with a CL/P who present to the healthcare system, evaluating for trauma symptoms and incorpora

[Reflections on recent ruptures of the diaphragm. Apropos of 45 cases].

Trauma-induced ruptures of the diaphragm are very exceptional but are regularly encountered in trauma surgery units. Closed ruptures occur in 60 to 80% of diaphragm lesions in European series, 9 times out of 10 after traffic accidents. Large American trauma centers however, have had more experience with stab of firearm wounds. The diagnosis raises many problems and requires further exploration after the standard chest x-ray. We discuss the different explorations which can be contributive in both emergency and non-emergency situations. The most appropriate therapeutic approach is also discussed…

FAST and Furious About Quality: Impact of Continuous Review and Feedback on Point-of-Care Ultrasound Examinations.

Background: Focused Assessment with Sonography in Trauma (FAST) examination is a point-of-care ultrasound study used to evaluate for abdominal hemorrhage, pneumothorax, or pericardial blood in trauma patients as an adjunct to their initial assessment. The quality of the image can be limited, and its diagnostic value is heavily dependent on operator skill. Our objective was to determine whether a standardized review process improved image quality and reduced incidence of nondiagnostic or insufficient imaging by 10% over a 6-month period. Study Design: Between July 1, 2021, and March 31, 2022, w

Haemorrhage control beyond Advanced Trauma Life Support (ATLS) protocol in life threatening maxillofacial trauma - experience from a level Ⅰ trauma centre.

Maxillofacial injuries are usually not life-threatening and do not get priority over other associated injuries. However, some maxillofacial injuries with active oral or nasal bleeding need immediate management due to threatened airway and blood loss. In the case of major active vascular bleeding, measures such as local pressure, anterior nasal packing, posterior nasal packing, and balloon tamponade are ineffective. In these cases, angiography and transcatheter arterial embolisation (TAE) are used to treat life-threatening haemorrhage caused by maxillofacial trauma. We analysed the medical reco

Operative management of equine-related maxillofacial trauma presenting to a Melbourne level-one trauma centre over a six-year period.

The aim of this study was to examine the pattern of equine-related maxillofacial trauma that required operative treatment in patients who presented to a level-one trauma centre in Melbourne between 2011 and 2016. A total of 28 patients (16 female and 12 male, median (range) age 31 (16-76) years) were identified from a database of all operatively managed maxillofacial trauma cases, and data were collected on demographics, mechanisms and patterns of injury, and management. The most common mechanism was kicking (n=16), which was also the most likely to result in multiple injuries. Half the patien

Temporary arterial shunts in damage control: Experience and outcomes.

BACKGROUND: Arterial shunting is a well-described method to control hemorrhage and rapidly reestablish flow, but optimal shunt dwell times remain controversial. We hypothesized that prolonged shunt dwell times of more than 6 hours are related to adverse outcomes after major arterial injury. METHODS: A review (2005-2013) of all patients with arterial shunts placed after traumatic injury at our urban Level I trauma center was undertaken. Patients who died prior to shunt removal (n = 7) were excluded. Shunt complications were defined as dislodgement, thrombosis, and distal ischemia. Patients wer

Minimally invasive ventricular assist device implantation.

Durable mechanical circulatory support (MCS) systems are established therapy option in patients with end-stage heart failure, with increasing importance during the last years due to donor organ shortage. Left ventricular assist devices (LVADs) are traditionally implanted through median sternotomy (MS). However, improvement in the pump designs during the last years led to evolvement of new surgical approaches that aim to reduce the invasiveness of the procedure. Numerous reports and studies have shown the viability and possible advantages of less-invasive approach compared to the sternotomy app

Discharge functional status and predictors of all-cause geriatric trauma readmission across a mature trauma network.

INTRODUCTION: Readmission after trauma remains a significant challenge in the geriatric population. Few studies have looked at geriatric trauma readmission (GTR) across a mature trauma network including at level IV centers. Our objective was to determine if discharge functional status predicts GTR across all levels of trauma centers. Secondary objective was to determine incidence and reason for GTR. METHODS: Institutional trauma registries were queried for all geriatric trauma admissions across our network (2018-2023). These data were merged with all-cause network readmission data. Demographi…

Does Gender Matter: A Multi-Institutional Analysis of Viscoelastic Profiles for 1565 Trauma Patients With Severe Hemorrhage.

BACKGROUND: Viscoelastic tests including thromboelastography (TEG) and rotational thromboelastometry (ROTEM) are being used in patients with severe hemorrhage at trauma centers to guide resuscitation. Several recent studies demonstrated hypercoagulability in female trauma patients that was associated with a survival advantage. The objective of our study was to elucidate the effects of gender differences in TEG/ROTEM values on survival in trauma patients with severe hemorrhage. METHODS: A retrospective review of consecutive adult patients receiving massive transfusion protocol (MTP) at 7 Level…

Pediatric trauma systems.

The origins of trauma systems in the United States date to the 1960s when physicians returning from wars abroad realized that lessons learned from managing military casualties could be applied to civilian traumatic injury. Over the next several decades, trauma centers and then trauma systems began to be developed in an attempt to improve prehospital and acute care for these patients. Although studies of trauma system effectiveness are fraught with methodologic difficulties, several types of studies (panel reviews of preventable deaths, registry studies, and population-based studies), suggest t…

The Role of Child Maltreatment and Revictimization on Substance Use Trajectories Among Women Involved in the Criminal Justice System.

We explored the mechanisms underlying the relationship between trauma in childhood and adulthood and substance use initiation and recovery from opioid use disorder (OUD). We triangulated the perspectives of 20 women with current/previous OUD and 22 criminal legal and drug treatment professionals. Themes centered around the long-term effects of unaddressed childhood trauma, family- and partner-based trauma across the life course, sexual trauma, and traumatic events due to opioid use. Findings highlighted the unique trauma-related needs of women involved in the criminal legal system who also use
