Hyperreactivity of neutrophils and abnormal T cell homeostasis: a new insight for pathogenesis of Behçet's disease.

Neutrophil hyperfunction of Behçet's disease is in part regulated by genetical factors, especially related to HLA-B51 genes and by immunological abnormalities. Regarding the latter points, Behçet's disease worsens with abnormal regulation by gamma delta T cells and alpha beta T cells. Indeed, our own studies and those of other laboratories suggest that human heat shock protein may be one of the triggering factors for activation of both the gamma delta T cells and alpha beta T cells. These activated T cells may produce proinflammatory and/or inflammatory cytokines, and lead to tissue injury pos

A control, double-blind comparison of mepivacaine injection versus saline injection for myofascial pain.

In a double-blind study 28 patients with acute, localised muscle pain received four local injections of mepivacaine 0.5%, and 25 patients with the same type of pain received local injections of an equivalent volume of physiological saline. The group receiving saline tended to have more relief of pain, especially after the first injection. The results thus show that pain relief is not due merely to the local anaesthetic. The study therefore raises questions about the mechanism by which local injections into muscle relieves pain, since there is the possibility that a similar effect might also be

pH-/Temperature-Triggered Gel Transition of Hyperbranched PEI-g-PDMAEMA as a Dual-Responsive Inhibitor for Clay Hydration Control.

To mitigate clay hydration and wellbore instability during deepwater drilling, a pH/temperature dual-responsive graft copolymer, hyperbranched polyethylenimine-g-poly(2-(dimethylamino)ethyl methacrylate) (HPEI-g-PDMAEMA), was designed and synthesized via free radical polymerization. Optimized synthesis at an HPEI/DMAEMA mass ratio of 1:2 with 2.4% AIBN at 70 °C for 10 h yielded a grafting ratio of 35.2% and a molecular weight of 84.3 kDa. The copolymer exhibits a tunable lower critical solution temperature (LCST) of approximately 48 °C at pH 8, decreasing with increasing pH due to tertiary ami

The Utility of Viscoelastic Testing in Guiding Intraoperative Transfusion Requirements in Pediatric Living Donor Liver Transplantation.

INTRODUCTION: Blood transfusion, though essential, is associated with increased morbidity following liver transplantation (LT). In pediatric liver transplantation (PLT), where physiological reserves are limited, optimal blood component transfusion (BCT) is critical. While viscoelastic testing (VET) has shown benefits in adult LT, pediatric evidence remains limited and transfusion triggers are poorly defined. This study evaluated the effectiveness of VET-guided BCT in pediatric living donor liver transplantation (LDLT). MATERIALS AND METHODS: A prospective analysis was conducted on children un

Sgk1 sensitivity of Na(+)/H(+) exchanger activity and cardiac remodeling following pressure overload.

Sustained increase of cardiac workload is known to trigger cardiac remodeling with eventual development of cardiac failure. Compelling evidence points to a critical role of enhanced cardiac Na(+)/H(+) exchanger (NHE1) activity in the underlying pathophysiology. The signaling triggering up-regulation of NHE1 remained, however, ill defined. The present study explored the involvement of the serum- and glucocorticoid-inducible kinase Sgk1 in cardiac remodeling due to transverse aortic constriction (TAC). To this end, experiments were performed in gene targeted mice lacking functional Sgk1 (sgk1 (-

Effects of extracorporeal shock wave lithotripsy on cardiac pacemakers and its safety in patients with implanted cardiac pacemakers.

UNLABELLED: Effects of extracorporeal shock wave lithotripsy (ESWL) were studied on 15 pacemakers (standard single chamber n = 5, dual chamber n = 6, rate responsive single chamber [Activitrax] n = 4). In-vitro testing involved suspending the pacemakers in a bath of degassified, deionized water firmly taped to a platform at the point of maximal pressure, i.e., second focal point (F2), where they received pressure shocks (means = 1300) from the HM3 Dornier lithotriptor. The pacemakers, programmed to their most sensitive setting, were continuously pacing at nominal outputs (atrial and ventricula

Unspecific Peroxygenases-Catalyzed Oxidation of Pharmaceutical Compounds Considered Emerging Contaminants.

Unspecific peroxygenase (UPO) is versatile fungal enzyme that carries out with high efficiency a plethora of C-H oxyfunctionalizations of organic compounds, simply triggered by H2O2. With the goal of making UPO an industrial biocatalyst, it has been subjected over the years to different directed evolution campaigns focused on expression, stability, activity, and selectivity. In this work, we tested the ability of PaDa-I and JaWa, evolved variants of the UPO from Agrocybe aegerita, to catalyze the oxidation of a series of pharmaceutical compounds, considered emerging contaminants (EC), or micro

Foliar dewdroplet-induced redox cascades promote early flowering in Brassicaceae plants.

Atmospheric water inputs such as dew are often overlooked in plant biology. We show that foliar dewdrops act as biochemical microreactors that trigger flowering in Arabidopsis thaliana, a small plant from the mustard family (Brassicaceae). These droplets generate reactive oxygen species that induce intracellular nitric oxide accumulation, initiating a redox cascade that suppresses the biosynthesis of abscisic acid, which contributes to the regulation of flowering time. This occurs via S-nitrosylation-mediated activation of histone deacetylase 19, which silences AAO3 and ABA2. Analysis of over

Pharmacogenetic considerations in therapy with novel antiplatelet and anticoagulant agents.

Antiplatelets and anticoagulants are extensively used in cardiovascular medicine for the prevention and treatment of thrombosis in the venous and arterial circulations. Wide inter-individual variability has been observed in response to antiplatelets and anticoagulants, which triggered researchers to investigate the genetic basis of this variability. Data from extensive pharmacogenetic studies pointed to strong evidence of association between polymorphisms in candidate genes and the pharmacokinetics and pharmacodynamic action and clinical response of the antiplatelets clopidogrel and the antico

Goodpasture syndrome: a rare case presenting with recurrent haemoptysis.

Goodpasture syndrome is a rare autoimmune disease which affects young adults with a male preponderance and can be triggered at any point in life with a classical clinical triad of rapidly progressive glomerulonephritis, diffuse pulmonary haemorrhage and circulating anti-glomerular basement membrane antibody (anti-GBM antibody). Here we are presenting a case of a young man with hypertension in his early 20s who presented with fatigue, recurrent haemoptysis, breathlessness and decreased urine output without features of infection. He was diagnosed at an early stage of the disease with the help of

Total Synthesis of Sculponin U through a Photoinduced Radical Cascade Cyclization.

We have accomplished the total synthesis of sculponin U, a polycyclic C-20-oxygenated kaurane diterpenoid featuring a 7,20-lactone-hemiketal bridge, through a radical cascade cyclization triggered by photoinduced electron transfer (PET) of a silyl enolate to form the cyclohexanone-fused bicyclo[3.2.1]octane skeleton. Other key points in our synthetic strategy encompass a Diels-Alder reaction to construct the middle six-membered ring of sculponin U, and an intramolecular radical cyclization induced by iron-catalyzed hydrogen atom transfer to close the western cyclohexane ring. Successful prepar

Unraveling the mysteries of spiral artery remodeling.

Spiral artery remodeling is the process by which the uterine vessels become large bore low resistance conduits, allowing delivery of high volumes of maternal blood to the placenta to nourish the developing fetus. Failure of this process is associated with the pathophysiology of most of the major obstetric complications, including late miscarriage, fetal growth restriction and pre-eclampsia. However, the point at which remodeling 'fails' in these pathological pregnancies is not yet clear. Spiral artery remodeling has predominantly been described in terms of its morphological features, however w

Updates on the diagnosis and management of celiac disease.

Celiac disease is a chronic autoimmune enteropathy affecting about 1% of the population. Gluten ingestion triggers an immune response in genetically susceptible patients, resulting in intestinal and extraintestinal disease manifestations. Current recommendations for diagnosis include serology for celiac-specific antibodies to transglutaminase, endomysium, and deamidated gliadin, and IgA serology. New highly accurate point-of-care tests can efficiently screen for celiac disease and improve the diagnostic timeframe. Definitive diagnosis is most commonly made via biopsy of the small bowel showing

Immune Cell Plasticity in Inflammation: Insights into Description and Regulation of Immune Cell Phenotypes.

Inflammation is a life-saving immune reaction occurring in response to invading pathogens. Nonetheless, inflammation can also occur in an uncontrolled, unrestricted manner, leading to chronic disease and organ damage. Mechanisms triggering an inflammatory response, hindering such a response, or leading to its resolution are well-studied but so far insufficiently elucidated with regard to precise therapeutic interventions. Notably, as an immune reaction evolves, requirements and environments for immune cells change, and thus cellular phenotypes adapt and shift, leading to the appearance of dist

The history of the drug-induced enterocolitis syndrome.

The diagnosis of drug-induced enterocolitis syndrome (DIES), resembling the typical findings of a well-known disease, the food protein-induced enterocolitis syndrome (FPIES), was acknowledged in the first publication on the topic in 2014. Ten cases of DIES have been described so far. Unanswered questions concerning DIES include its pathogenetic mechanism, natural history, the possible presence of predisposing genetic factors, and the potential existence of its atypical forms. DIES is a recently defined and intriguing clinical entity, similar to FPIES but triggered by drugs. It seems well-defin

(Un)triggering Anorexia: A Cognitive Literary Analysis of Lia "the Liar" in Wintergirls (2009).

The importance of authorial intention has been debated extensively in literary studies. In cognitive literary studies, however, the effects books provoke in readers are of greater relevance. With an unreliable intradiegetic narrator, ambivalent about her denial of hunger, Wintergirls (2009), a US YA anorexia novel, embodies the spiraling network of lies that feeds this condition. This essay takes Wintergirls as a starting point to discuss the therapeutic or harmful effects of literature, over and above the intentions of the writer. Adopting a cognitive literary perspective, this essay proposes

Malignant hyperthermia in a 16-day-old infant with congenital diaphragmatic hernia: a case report.

Malignant hyperthermia (MH) is a severe hypermetabolic disorder associated with dysregulation of calcium homeostasis and is triggered by inhalational anesthetics (isoflurane, sevoflurane, desflurane) and a depolarizing muscle relaxant (succinylcholine). We report the case of a 16-day-old infant undergoing laparoscopic surgery. The patient developed hyperthermia and hypercarbia with muscle rigidity. After the diagnosis of MH, dantrolene was administered with sufficient hydration. The patient was transferred to the pediatric intensive care unit for monitoring and treatment of acute renal injury

BMP signaling is essential for sustaining proximo-distal progression in regenerating axolotl limbs.

Amputation of a salamander limb triggers a regeneration process that is perfect. A limited number of genes have been studied in this context and even fewer have been analyzed functionally. In this work, we use the BMP signaling inhibitor LDN193189 on Ambystoma mexicanum to explore the role of BMPs in regeneration. We find that BMP signaling is required for proper expression of various patterning genes and that its inhibition causes major defects in the regenerated limbs. Fgf8 is downregulated when BMP signaling is blocked, but ectopic injection of either human or axolotl protein did not rescue

Tremor in Parkinson's Disease May Arise from Interactions of Central Rhythms with Spinal Reflex Loop Oscillations.

It is commonly believed that tremor, one of the cardinal signs of Parkinson's disease, is associated with cerebello-thalamo-cortical oscillations set off by the dopamine-depleted basal ganglia networks. The triggering mechanism has been, however, not entirely delineated. Several reports have pointed to the relevance of interactions with peripheral/spinal mechanisms to tremor generation. Investigations of motor unit synchronization and discharge patterns suggested that exaggerated beta-band oscillations may intermittently reach alpha-motoneurons and modulate low-amplitude membrane oscillations
