Transesophageal echocardiographic screening before atrial flutter ablation: is it necessary for patient safety?

BACKGROUND: Transesophageal echocardiography (TEE) is commonly used before atrial flutter (AFl) ablation to detect atrial thrombus (AT) and thereby identify a heightened risk for systemic embolism both in patients with their initial episodes of AFl and in those with prior episodes whose anticoagulation has been inadequate. This treatment strategy has been extrapolated from guidelines for atrial fibrillation. In fact, limited data exist regarding the prevalence or clinical associations of AT and spontaneous echocardiographic contrast (SEC) in patients with AFl. Both AT and SEC are believed to r

Outpatient treatment of recent-onset atrial fibrillation with the "pill-in-the-pocket" approach.

BACKGROUND: In-hospital administration of flecainide and propafenone in a single oral loading dose has been shown to be effective and superior to placebo in terminating atrial fibrillation. We evaluated the feasibility and the safety of self-administered oral loading of flecainide and propafenone in terminating atrial fibrillation of recent onset outside the hospital. METHODS: We administered either flecainide or propafenone orally to restore sinus rhythm in 268 patients with mild heart disease or none who came to the emergency room with atrial fibrillation of recent onset that was hemodynami

Arrhythmia and impaired myocardial function in heritable thoracic aortic disease: An international retrospective cohort study.

BACKGROUND: Heritable thoracic aortic diseases (HTAD), typically entailing aortic complications, can be caused by pathogenic variants or likely pathogenic variants (PV/LPVs) in several genes, including fibrillin1 (FBN1), Actin Alpha2 (ACTA2) and genes encoding components of the transforming growth factor (TGF)-β signaling pathway. In addition to aortic complications, non-aortic cardiac disease such as impaired myocardial function and/or arrhythmia have been increasingly reported, mainly in Marfan syndrome with underlying FBN1 PV/LPVs and are acknowledged as additional causes of morbidity and m

Outcomes in Patients With Multiple Myeloma and Prescribed Urate-Lowering Therapy: A Propensity-Matched Study.

Introduction Multiple myeloma or MM is a plasma cell dyscrasia that causes monoclonal immunoglobulin accumulation and is associated with manifestations including hypercalcemia, anemia, bone pain, and renal dysfunction. Gout is also associated with electrolyte derangements that result in similar end-organ dysfunction. There is little information on outcomes in patients with both diagnoses. Methods Adult patients with MM were propensity-score matched according to age at index event, sex, demographics, and comorbidities. Patients were identified using the International Classification of Diseases

[Significance of a supraventricular arrhythmia precipitated during an electrophysiological study].

The clinical history and 24 hour Holter monitoring of 749 patients without ECG appearances of ventricular preexcitation were compared with the results of electrophysiological investigations to determine whether supraventricular arrhythmias initiated during endocavitary electrophysiological investigations had any pathological significance. Endocavitary studies were undertaken to investigate symptoms of dizziness, syncope and/or conduction defects except in the group of paroxysmal junctional tachycardia (PJT) where the indication was investigation of a tachycardia (78 cases). In 544 patients (Gr

Androgen-Modulating Drugs and Severe Complications After ST-Elevation Myocardial Infarction.

IMPORTANCE: Male sex and endogenous testosterone have been shown to exacerbate neutrophilia and cardiac injury in myocardial infarction (MI), raising questions about protective actions of androgen-modulating drugs in acute ST-elevation MI (STEMI). OBJECTIVE: To determine whether the use of 5-alpha reductase inhibitors (5αRIs) or androgen deprivation therapy (ADT), drugs that lower androgen actions, is associated with a reduced frequency of severe complications after percutaneous coronary intervention (PCI)-treated STEMI in men. DESIGN, SETTING, AND PARTICIPANTS: Nationwide, registry-based co

Cor Triatriatum Dexter: The Largest Comprehensive Review in the Field on 124 Worldwide Cases (1968-Now).

BACKGROUND: Cor triatriatum dexter (CTD) is a rare congenital heart defect where a membrane divides the right atrium into two chambers, resulting from the incomplete regression of the right valve of the sinus venosus. Due to its rarity, only individual case reports and a limited number of case series have been published to date. This study constitutes the most extensive comprehensive review conducted in this area. Eight factors were evaluated: age at diagnosis, sex, clinical presentation, electrocardiographic findings, imaging (ultrasound, CT, or MRI), associated cardiac anomalies, and patient

Long-Term Efficacy of Catheter Ablation for Reentrant Atrial Tachycardia in Dextro-Transposition of the Great Arteries Patients After Atrial Switch.

BACKGROUND: Reentrant atrial tachycardias (ATs) are frequent after atrial switch for dextro-transposition of the great arteries and are associated with adverse prognosis. Catheter ablation is first-line therapy, yet long-term outcomes and predictors of recurrence remain poorly defined. Our aim was to characterize induced AT circuits, describe acute and long-term catheter ablation outcomes, and identify recurrence predictors in dextro-transposition of the great arteries. METHODS: Single-center ambispective cohort of consecutive adults with dextro-transposition of the great arteries undergoing

Clinical Role of the Noninvasive Abdominal Fetal ECG in the Detection and Monitoring of Fetal Tachycardia.

BACKGROUND: Fetal tachycardias can cause adverse fetal outcomes including ventricular dysfunction, hydrops, and fetal demise. Postnatally, ECG is the gold standard, but, in fetal practice, echocardiography is used most frequently to diagnose and monitor fetal arrhythmias. Noninvasive extraction of the fetal ECG (fECG) may provide additional information about the electrophysiological mechanism and monitoring of intermittent arrhythmias. Signal processing advances could provide improved data quality impacting clinical translation. The aim of this study was to assess the fetus with known or suspe

Postoperative Atrial Fibrillation Following Anatomic Lung Resection: A Case Report and Narrative Review of Contemporary Pharmacologic Management.

Postoperative atrial fibrillation (POAF) is a common complication following anatomic lung resection, contributing to increased morbidity and mortality, prolonged hospital stays, and higher healthcare costs. Despite its frequency, there remains limited consensus on optimal pharmacologic management in this population, particularly in the context of balancing efficacy with the unique risks associated with thoracic surgery. This report aims to draw attention to the clinical significance of POAF in thoracic surgery, particularly following pulmonary resections, by presenting a representative case an

Fluoroless and contrast-free catheter ablation without a lead apron in routine clinical practice.

The technique of catheter ablation has been improved within the past few decades, especially by three-dimensional (3D) mapping system. 3D mapping system has reduced radiation exposure but ablation procedures still require fluoroscopy. Our previous study showed the safety and efficacy of catheter ablation based on intracardiac echogram combined with CARTOSOUND/CARTO3 system, however fluoroscopy use for an average of 16 min is required for this procedure. The present study was aimed to reduce radiation exposure to zero and establish a radiation free catheter ablation method with the goal of util
